    Serum endocan levels in women with restless legs syndrome.

    Background: Endocan is a recently introduced marker of endothelial dysfunction. The objective of this study was to compare serum endocan levels in patients with restless legs syndrome (RLS) and control subjects in order to elucidate whether RLS is associated with endothelial dysfunction. Methods: A total of 31 drug naïve female patients with RLS and 31 age- and BMI-matched women were included in the study. Patients with pathological or physiological conditions or with a history of medication use that could potentially influence endothelial functions were excluded, as well as those with alcohol or drug abuse history. The two groups were compared with routine blood tests and serum endocan levels. Results: Patients with RLS had lower serum endocan levels than the controls (P=0.037). There was a negative bivariate correlation between RLS severity score and serum endocan levels (r=-0.406, P=0.023). While white blood cell count was significantly higher in RLS group, 25-hydroxy vitamin D3, vitamin B12, transferrin saturation rate, and HDL-cholesterol were significantly lower. Creatininemia and diastolic blood pressure were also marginally insignificantly lower in RLS group. Due to the presence of differences between two groups in these variables, a linear regression analysis was performed that showed a positive association between endocan and creatininemia (β=0.310, P=0.022), and a negative association between endocan and RLS (β=-0.502, P,0.001). Conclusion: The results of this study seem to suggest that patients with RLS may have better endothelial functions when compared with the general population and that these patients may be better protected against atherosclerosis

    Association of calcium channel blocker use with lower hemoglobin levels in chronic kidney disease

    Aim: To search whether calcium channel blockers (CCBs) are associated with lower hemoglobin levels in chronic kidney disease (CKD) patients who are not on renal replacement therapy (RRT), vitamin D and anti-anemic treatment. Patients and methods: CKD patients were classified into two groups. Patients on CCBs treatment (103 patients) and patients not using CCBs (104 patients) were compared cross-sectionally regarding clinical findings, complete blood count (CBC), biochemistry and regular medication use. Patients with polycystic kidney disease, comorbidities that could influence CBC other than iron deficiency of obscure origin, patients receiving RRT, erythropoietin (EPO), vitamin D, phosphate binders and drugs that could influence CBC were excluded. Under dependent variable of CCB use, all significantly different independent variables were subjected to multivariate binary logistic regression analysis (MBLRA). Results: Lower hemoglobin, lower bilirubinemia, higher serum EPO, higher systolic blood pressure were observed in CCB users. Two groups were similar concerning age, gender, BMI, CKD etiology, CKD stage, pretibial edema prevalence, cardiothoracic index, diastolic blood pressure, corrected reticulocyte count, BUN, creatinine, eGFR, proBNP, parathormone, alkaline phosphatase, phosphorous, corrected calcemia, sCRP, relative EPO deficiency and prevalence of relative EPO deficient patients. Groups were comparable regarding comorbidities, types and usage frequencies of all antihypertensive medications other than CCBs. Higher systolic blood pressure and lower hemoglobin were significantly associated with CCB use after MBLRA. Conclusions: Hemoglobin was significantly lower in CCB users compared to non-users, among CKD patients who did not receive RRT, EPO, phosphate binders, vitamin D, iron, vitamin B12 and folic acid.

    Generation of Bone Tissue Using Adipose Tissue-derived Stem Cells

    Objective: Bone grafts and even bone substitutes do not meet all of the requirements of bony reconstructions. The aim of this study was to generate bone tissue from autologous adipose tissue-derived mesenchymal stem cells (ATDMSCs) and decellularised bone allografts. Methods: A 1.5 cm bone defect developed in the middle third of the rabbit's ulna. Reconstructions were carried out using miniplate and screws and interpositional autogenous bone grafts according to the designs of the groups: (1) No touch, (2) cryopreserved, (3) decellularised and (4) ATDMSCs-implanted decellularised bones. Before implantation, ATDMSCs in the last group were labelled with Q-dot and identified microscopically. Results: Graft recovery and irregular callus formation were observed in the first, second and forth groups. In the first group, the organisation of Haversian systems, the structure of the lacunae and the presence of canaliculi ossiums were observed; in the second group, approximately 40% of the Haversian canals contained blood vessels, and canaliculi ossiums in the form of thin filaments were found in 90% of the microscopically examined areas; in the third group, most Haversian canals were empty, most osteocyte canals were devoid of cells, and canaliculi ossiums were absent; in the fourth group, some of the Haversian canals contained blood vessels, and there were partly lacunae containing cells due to decellularisation, whereas in approximately 50% of the examined microscope areas, the presence of canaliculi ossiums with evidence of mesenchymal stem cells differentiated into osteocytes was demonstrated by Q-dot traced cells. Conclusion: In this study, the establishment of a proper niche environment for adipose-derived mesenchymal cells promotes their development into osteogenic cells.

    Boneless Maxillary Reconstruction

    Objective: Although the midface is supported by maxillary bone, reconstruction of maxillary bony defects does not always require osseous reconstruction. A series of maxillary facial defects of moderate to severe sizes (Brown and Shaw 4-6) uniformly reconstructed with myocutaneous free flaps is presented. Methods: Retrospective analysis was done for consecutive 10 cases between 2013-18. Seven of the cases had defects caused by oncological resection and 3 defects caused by trauma. Free vertically oriented rectus abdominis (fVRAM) flap was used to reconstruct these defects. The mean age of patients was 42.9 years (range=20-71 years). All flaps survived and 2 flaps had small skin island problems. Rapid recovery, low morbidity, acceptable oral competency, and satisfactory cosmetic appearance were obtained with the reconstruction using fVRAM flaps. Conclusion: According to our limited experience, acceptable reconstruction of extensive maxillary defects can be done with using only soft tissue flap, which is versatile and safe.

    Gastroenterology cases of cutaneous leukocytoclastic vasculitis.

    Rarely, leukocytoclastic vasculitis can result from ischemic colitis, inflammatory bowel disease, and cryoglobulinemia. There is no established standard for the treatment of leukocytoclastic vasculitis associated with gastroenterologic diseases. This paper presents three cases of leukoytoclastic vasculitis, each of which is associated with a different gastroenterologic condition: ischemic colitis, Crohn’s disease, and chronic hepatitis C. Each condition went into remission by treatment of leukocytoclastic vasculitis, regardless of the underlying disease.

    The Effect of Vitamin D3 on Skeletal Muscle in Rat after Ischemia Reperfusion Injury: Preliminary Report

    Objective: Vitamin D is a vitamin that has gained popularity in recent years and has an anti-inflammatory and immunmodular effect. In the literature review we conducted, there was no study investigating the effect of vitamin D to prevent ischemia reperfusion (I/R) injury in skeletal muscle. The aim of this study was to investigate the effect of vitamin D on skeletal muscle in I/R injury. Methods: Six rats were used in each group including ischemia group (group I) and experimental group (group D given vitamin D). In the histopathological examination, inflammation and apoptosis levels were studied in gastrocnemius muscle. In biochemical analysis, total oxidative stress and total antioxidant and catalase levels were evaluated in anterior tibialis muscle. Results: The number of apoptotic cells in group D were found significantly lower than group I. In the terms of total antioxidants and catalase levels, there were significant difference between group D and group I. Conclusion: Our results support the hypothesis that vitamin D is an agent that can be used to prevent I/R injury.

    Pedicled latissimus dorsi myocutaneous flap in the reconstruction of the head and neck region: Experience with 17 patients

    Objective: To assess the efficacy of a modified pedicled latis-simus dorsi myocutaneous flap (LDMF) in the reconstruction of defects of the head and neck region. Methods: The retrospective analysis of 17 consecutive patients operated between 2010 and 2014 in the Plastic and Reconstructive Surgery Department of a tertiary care center was performed. Among them, four (23.5%) were initially diagnosed with trauma, while 13 (76.5%) had squamous cell carcinoma originating from the head and neck region. Tumor recurrence, necrosis of the free flap, advanced age, presence of a comorbidity, and previous history of radiotherapy were indicators for a pedicled LDMF. The pedicled LDMF technique was performed via the modified subcutaneous tunnel method in all patients. Demographic, clinical, and peroperative data as well as complications in and a survey of our patients were documented. Results: Our series comprised 11 males (64.7%) and six females (35.3%); the average age was 64.29 +/- 5.43 (range, 51 to 72) years. Seroma (5/17, 29.4%) and partial flap necrosis (3/17, 17.6%) were the most common complications, while the rate of mortality during the follow-up period was 17.6% (3/17). One patient died during the early postoperative period, while two died during their oncological treatment and follow-up. No remarkable functional disabilities or restrictions were postoperatively observed. Conclusion: Our results showed that a pedicled LDMF can constitute a safe and effective alternative for the reconstruction of head and neck defects in selected patients. However, further controlled clinical trials on larger series are warranted to precisely unveil the indications, contraindications, and outcomes of a pedicled LDMF.

    A new concept for mandible reconstruction after oncological resection: Multisegment virtual surgical planning

    Purpose: Virtual surgical planning (VSP) is good for three dimensional reconstructions in maxillofacial surgery, but it is not problem-free completely especially when the resection margins cannot be affirmed in preoperative period. We aimed to obtain an ideal reconstruction with elaborating VSP to be prepared for adverse conditions during surgery and to proceed the oncological resections step- by- step with A, B, and C resection planes. Patients and methods: Four patients undergoing multisegment VSP for the primary mandible malignancies were included in the study. The first resection margin was detected as plan A in VSP, and plans of B and C were also prepared considering the tumor- positive result of intraoperative frozen section procedure. Results: Following the tumor resection, margins were extended to the plan B in two patients, and plan C in one patient in accordance with the results of the frozen section procedure.Histogram comparison of the localizations of osteotomies in mandible and fibula, and positions of the implants were calculated at a confidence level of 95 % (p > 0.95) and mean difference was found -0.55 mm, while standard deviation was 1.76 mm. Conclusion: Multisegment virtual surgical planning seems to achieve the optimal reconstruction with the staged resection preventing redundant removal of tumor- free structures like bone and teeth.
