    Parathyroid allotransplantation in rabbits without cultivation.

    Permanent hypoparathyroidism is a serious clinical situation. Allotransplantation of the parathyroid cells is relatively new approach to treatment. Non-cultivated allotransplantation in rabbits is not tried before. In this research parathyroidectomy was performed in six female New Zealand white rabbits. After division of surgically removed tissues into two, cryopreservation after cell isolation was done. Non-cultivated cross allotransplantation was performed under immunosuppression. Serum calcium and phosphorus levels were observed 15 days and histopathological analyses of the transplanted parathyroid tissues were studied. Significant changes in serum calcium and phosphorus levels during the experiment were observed (p=0.001 for both). Calcium levels which were significantly dropped to 6.66±0.7 mg/dL after parathyroidectomy and progressively increased up to 15.98±1.25 mg/dL at the end of the experiment (p=0.004). Phosphorus levels which were increased to 9.38±0.63 mg/dL after parathyroidectomy and stabilized to 4.46±1.06 mg/dL at the end of the experiment (p=0.007). All allotransplanted parathyroid tissues showed normal tissue architecture without evidence of cellular rejection. In conclusion allotransplantation of the parathyroid tissues without cultivation may be considered as an alternative and safe approach for the treatment of permanent hypoparathyroidism.

    Efficacy of glycerol and flax seed oil as anti-adhesive barriers after thyroidectomy.

    We evaluated the effects of local flax seed oil and glycerol application for reducing adhesion formation after thyroidectomy. Material/Method: We randomly assigned 18 female Wistar albino rats (median weight: 275 g, median age: 4.5 mth) to 3 groups: 0.1 ml 0.9% NaCl, glycerol, and flax seed oil were sprayed in a perithyroidal area after thyroidectomy operation on all animals as anti-adhesive barriers. Rats were sacrificed on the postoperative 14th day and adhesions were evaluated macroscopically and microscopically. Results: The median macroscopic adhesion score was 3.0±0.0 in the 0.9% NaCl group, 1.33±0.52 in the glycerol group, and 1.67±0.53 in the flax seed oil group. The median histopathological fibrosis scores were 2.33±0.82, 0.67±0.52, and 0.83±0.75, respectively. Both glycerol and flaxseed oil group macroscopic and microscopic scores were significantly lower than the 0.9% NaCl group (p<0.05). There was no significant difference among the groups (p>0.05). Conclusions: Glycerol and flax seed oil both decrease the incidence of post-thyroidectomy adhesion in rats, but glycerol is more effective.

    Safe thyroidectomy with intraoperative methylene blue spraying.

    Background: We aimed to minimalize operative complications by spraying of methylene blue stain on thyroid glands and the perithyroidal area. Material and methods: The intra-operative methylene blue spraying technique was used prospectively on a total of 56 patients who had undergone primary (not recurrent) thyroid surgery for a variety of thyroid diseases. Bilateral total thyroidectomy was performed in all cases. After superior but before inferior pole ligation, 0.5ml of methylene blue was sprayed over the thyroid lobe and perilober area. Tissues, especially parathyroides, the recurrent laryngeal nerve, and the inferior thyroid artery, were identified and evaluated. Results: Recurrent laryngeal nerve and arteries were not stained and thus they remained white in all cases while all other tissues were stained blue. Within three minutes parathyroid glands washed out the blue stain and the original yellow color was regained. Thyroid tissue wash-out time was not less than 15 minutes; perithyroideal muscles, tendinous and lipoid structures took no less than 25 minutes. Conclusion: The safety of intravascular methylene blue guidance on thyroid surgery is known. This research demonstrates the effectiveness of the spraying technique, a new technique which ensures not only identification of parathyroid glands within three minutes, but also identification of recurrent laryngeal nerves and inferior thyroid arteries.

    Parathyroid Allotransplant With a New Technique: A Prospective Clinical Trial.

    Objectives: Parathyroid allotransplant is a valuable alternative in treating permanent hypoparathyroidism. However, it is a difficult process that requires several trained staff and advanced laboratory equipment, which makes the costs high. Here, we identify a new parathyroid allotransplant technique. Materials and Methods: After obtaining informed consent from patients, parathyroid cell suspensions obtained from 4 donors who had undergone a subtotal parathyroidectomy owing to chronic renal failure were transplanted in 10 patients with permanent hypoparathyroidism after short-term cell cultivation. Prednisolone were used as immunosuppressant for the first 10 days and discontinued thereafter. Results: Allograft function was observed in 7 patients (70%) at a mean follow-up of 12 months. Daily oral calcium and vitamin D supplementations discontinued totally in 7 patients. No major or minor complication was observed. Conclusions: Our technique is simple, fast, and has a low cost, with a 70% success rate at a mean follow-up of 12 months. It requires few staff, minimal equipment, and short-term immunosuppressant use for main tenance. The technical developments of parathyroid allotransplant, as mentioned in this study, may be important in treating permanent hypoparathyroidism

    Parathyroid Allotransplant for Persistent Hypocalcaemia: A New Technique Involving Short-Term Culture.

    Objectives: To develop a new parathyroid allotransplant method for the treatment of permanent hypoparathyroidism. Materials and methods: Parathyroid cells 50 × 10(6) derived from a parathyroid hyperplasia patient were transferred to a 61-year-old patient who had thyroidectomy 17 years earlier, allowing to papillary thyroid cancer; he was admitted to our outpatient clinic with symptomatic chronic hypocalcemia. Cell isolation, cryopreservation, and culturing were conducted according to a new protocol. Results: During a follow-up of 5 months, the patient had no complications that could indicate rejection, and clinical symptoms completely resolved without requiring any drug supplementation. Conclusions: Here, we report a new method, enabling fast and cost-effective parathyroid allotransplant with maintained tissue viability sufficient to treat persistent hypocalcemia.

    Intersphincteric resection and coloanal anastomosis in treatment of distal rectal cancer.

    In the treatment of distal rectal cancer, abdominoperineal resection is traditionally performed. However, the recognition of shorter safe distal resection line, intersphincteric resection technique has given a chance of sphincter-saving surgery for patients with distal rectal cancer during last two decades and still is being performed as an alternative choice of abdominoperineal resection. The first aim of this study is to assess the morbidity, mortality, oncological, and functional outcomes of intersphincteric resection. The second aim is to compare outcomes of patients who underwent intersphincteric resection with the outcomes of patients who underwent abdominoperineal resection.

    Body weight reducing effect of oral boric acid intake.

    Background: Boric acid is widely used in biology, but its body weight reducing effect is not researched. Methods: Twenty mice were divided into two equal groups. Control group mice drank standard tap water, but study group mice drank 0.28mg/250ml boric acid added tap water over five days. Total body weight changes, major organ histopathology, blood biochemistry, urine and feces analyses were compared. Results: Study group mice lost body weight mean 28.1% but in control group no weight loss and also weight gained mean 0.09% (p<0.001). Total drinking water and urine outputs were not statistically different. Cholesterol, LDL, AST, ALT, LDH, amylase and urobilinogen levels were statistically significantly high in the study group. Other variables were not statistically different. No histopathologic differences were detected in evaluations of all resected major organs. Conclusion: Low dose oral boric acid intake cause serious body weight reduction. Blood and urine analyses support high glucose, lipid and middle protein catabolisms, but the mechanism is unclear.

    Cancer preventive effects of whole cell type immunization against mice Ehrlich tumors.

    Background: Effects of whole cell type immunization on mice Ehrlich tumours were evaluated. Materials and Methods: After preliminary study, mice were divided two major groups; 1x1000 and 100x1000 live Ehrlich cell transferred major groups, each divided into four subgroups (n: 10). Study groups were immunized with Ehrlich cell lysates in 0, 3, 7, 14th days and after 30 days of last immunization, live Ehrlich cells were transferred. Mice were observed for six months and evaluated for total and cancer free days. Results: Out of 100x1000 cell transferred solid type study group, all study group mean and tumour free periods were statistically longer than control groups. All 1x1000 Ehrlich cell transferred study groups survived significantly longer than 100x1000 Ehrlich cell transferred groups. Conclusions: Ehrlich mice tumours were prevented and survival prolonged with whole cell type immunization. Effects are related to the number of transferred tumor cells.
