Publication 120-W GreenLight Laser Photoselective Vaporization of Prostate for Benign Prostatic Hyperplasia: Midterm Outcomes(01.07.2011) Tasci, Ali Ihsan; Ilbey, Yusuf Ozlem; Luleci, Huseyin; Cicekler, Olcay; Sahin, Selcuk; Cevik, Cem; Tugcu, VolkanOBJECTIVES To evaluate the safety and efficacy of 120-W high-performance system (HPS) laser photoselective vaporization of the prostate (PVP) in the treatment of patients with lower urinary tract symptoms secondary to benign prostatic hyperplasia. METHODS Two experienced surgeons performed 120-W HPS laser PVP. The baseline characteristics, perioperative data, complications, and postoperative outcomes were evaluated at 3, 6, 12, 24, and 36 months postoperatively. RESULTS A total of 550 consecutive patients, with a mean age of 67.6 years, underwent PVP with a 120-W HPS laser. The mean prostate volume was 72.93 cm(3), with a mean prostate-specific antigen level of 3.57 ng/mL. The mean operative duration and the mean applied energy was 61.3 minutes and 164.06 kJ, respectively. No major complication occurred intraoperatively or postoperatively. The mean follow-up was 17.80 months. Significant improvements were observed postoperatively in the mean International Prostate Symptom Score, quality of life score, maximal urinary flow rate, and postvoid residual urine volume. The mean catheterization time was 18.5 hours. The complications included delayed hematuria in 26 (4.8%), recatheterization in 24 (4.4%), reoperation owing to residual prostatic adenoma in 46 (8.5%), urethral stricture in 19 (3.5%), and bladder neck contracture in 6 (1.1%) patients. CONCLUSIONS PVP with a 120-W HPS laser for benign prostatic hyperplasia has been proved to be a safe and effective procedure for our patients, including those treated with oral anticoagulants. The functional outcome in larger prostates was similar to that in smaller glands. UROLOGY 78: 134-141, 2011. VOCAL programme, echogenicity, number of follicles and blood flow parameters were evaluated. The patients with PCOS had a higher total ovarian volume, mean stromal volume and stromal echogenicity (18.6 +/- 4.75 to 10.2 +/- 3.4p .05). 3 D power Doppler parameters included VI, FI, and VFI values of the patients with PCOS were higher when compared to those of the patients with multifollicular ovary (3.82 +/- 2.65 to 1.78 +/- 1.2,p < .01; 50.76 +/- 4.45 to 40.6 +/- 3.64,p = .03; and 2.34 +/- 1.02 to 1.12 +/- 0.65,p = .02, respectively). Our results revealed that total ovarian volume, stromal volume and echogenicity; VFI, VI, and FI could be useful for differential diagnosis in women with PCOS and multifollicular ovaries.Impact statement What is already known on this subject?Ultrasonography is considered the new diagnostic tool for PCOS. Enlarged ovaries with multiple small follicles peripherally located around increased ovarian stroma with increased stromal echogenicity are the sonographic features of polycystic ovaries. What do the results of this study add?3-D Doppler ultrasonography may be more specific in the determination of multifollicular and polycystic ovaries when compared to RI and PI in 2-D Doppler ultrasonography. Moreover, 3-D power Doppler ultrasonography could be useful for differential diagnosis in women with PCOS and multifollicular ovaries. What are the implications of these findings for clinical practice and/or further research? In clinical practice, the differentiation of multifollicular ovaries and polycystic ovaries (PCO) is difficult with the use of 2-D sonography alone. Therefore, 3-D ultrasound and power Doppler may also be used in addition to 2-D ultrasound for the differentiation of multifollicular ovaries and PCO.Publication 3-miktoarm star terpolymers using triple click reactions: Diels-Alder, copper-catalyzed azide-alkyne cycloaddition, and nitroxide radical coupling reactions(2012-02-15T00:00:00Z) Günay, Ufuk Saim; Durmaz, Hakan; Gungor, Eda; Dag, Aydan; Hızal, Gürkan; Tunca, Ümit; DAĞ, AYDANWell-defined linear furan-protected maleimide-terminated poly(ethylene glycol) (PEG-MI), tetramethylpiperidine-1-oxyl-terminated poly(epsilon-caprolactone) (PCL-TEMPO), and azide-terminated polystyrene (PS-N-3) or -poly(N-butyl oxanorbornene imide) (PONB-N-3) were ligated to an orthogonally functionalized core (1) in a two-step reaction mode through triple click reactions. In a first step, DielsAlder click reaction of PEG-MI with 1 was performed in toluene at 110 degrees C for 24 h to afford alpha-alkyne-alpha-bromide-terminated PEG (PEG-alkyne/Br). As a second step, this precursor was subsequently ligated with the PCL-TEMPO and PS-N-3 or PONB-N-3 in N,N-dimethylformamide at room temperature for 12 h catalyzed by Cu(0)/Cu(I) through copper-catalyzed azide-alkyne cycloaddition and nitroxide radical coupling click reactions, yield resulting ABC miktoarm star polymers in a one-pot mode. There are a few reported cases of overdose for suicide attempts or abuse with methylphenidate in current literature. Majority of these cases fully recovered after supportive therapy. We present a patient who ingested 3240 mg long acting methylphenidate for suicide attempt. He experienced increase in heart beat rate and blood pressure but he did not have additional symptoms. Although no serious neurological or cardiovascular event happened in this case, the supervised stimulant use is needed in child and adolescent population. Further studies are needed to evaluate the effect of methylphenidate overdose.Publication 57PD NEOADJUVANT CHEMORADIOTHERAPY FOR LOCALLY ADVANCED NON SMALL CELL LUNG CANCER(2013-04-01) ZİYADE, SEDAT; ZİYADE, SEDATPublication A clinical and microbiological comparison of Er: YAG laser and mechanical debridement for the nonsurgical periodontal therapy: a randomized controlled clinical study(2018-06-01) ZENGİN ÇELİK, TUĞBA; ERCAN, ÇİLEM; AKBAŞ, FAHRİ; SAĞLAM, EBRU; Nazaroğlu, KENAN; TUNALI, MUSTAFA; ZENGİN ÇELİK, TUĞBA; ERCAN, ÇİLEM; AKBAŞ, FAHRİ; TUNALI, MUSTAFAPublication A 13-Year-Old Boy Who Has Kawasaki Disease Shock Syndrome Presents with Parotitis(2020-03-01T00:00:00Z) YOZGAT, Yılmaz; UZUNER, SELÇUK; Demir, Aysegul Dogan; OĞUR, MUSTAFA; Yozgat, Can Yilmaz; TÜREL, Özden; YOZGAT, YILMAZ; UZUNER, SELÇUK; OĞUR, MUSTAFA; TÜREL, ÖZDENWe report a 13-year-old boy who (initially) had symptoms of toxic shock-like syndrome and mumps. Then, the patient was hospitalized in the pediatric intensive care unit (PICU) because of his ongoing hemodynamic instability (low blood pressure of 70/30 mm Hg and capillary refill time of > 4 seconds). During his stay in the PICU, the patient was treated with fluid resuscitation and vasoactive infusion and at the same time was diagnosed with Kawasaki disease shock syndrome (KDSS), when giant right coronary artery aneurysms were detected on echocardiographic examination. This case illustrates the risk of KDSS in patient who carries both parotitis and toxic shock-like syndrome. The clinicians should be cautious about detecting any types of coronary artery aneurysms in such patients. This is the first case of KDSS associated with parotitis reported in the literature.Publication A 17-Year-Old Female with Systemic Lupus Presents with Complex Movement Disorder: Possible Relationship with Antiribosomal P Antibodies(2013-01-01T00:00:00Z) Ozcan, Muhammed Emin; Altinoz, Meric Adil; Karadeli, Hasan Huseyin; ASİL, Talip; Kocer, Abdulkadir; ASİL, TALIPComplex movement disorder is a relatively rare presentation of neurolupus. Antiphospholipid antibodies are associated with movement disorders likely via aberrant neuronal stimulation. Antiribosomal P antibodies have been previously associated with neuropsychiatric disorders but their correlation with movement disorder was not previously established. Our case report involves a 17-year-old Caucasian female patient positive for only antiribosomal P antibody and lupus anticoagulant who presented with a sudden onset of complex movement disorder. After complete cessation of physical signs with olanzapine, anticardiolipin and anti-beta 2 glycoprotein I antibodies became positive which indicates a likely discordance between movement disorder and antiphospholipid antibodies. This also indicates a potential causal role of antiribosomal P antibodies in inducing movement disorder.Publication A 4-year clinical evaluation of direct composite build-ups for space closure after orthodontic treatment(2015-12-01) Demirci, Mustafa; Tuncer, Safa; Oztas, Evren; TEKÇE, NESLİHAN; Uysal, Omer; UYSAL, ÖMERTo evaluate the medium-term clinical performance of direct composite build-ups for diastema closures and teeth recontouring using a nano and a nanohybrid composite in combination with three- or two-step etch-and-rinse adhesives following treatment with fixed orthodontic appliances.Publication A Basic Algorithmic Surgical Approach for Nicolau Syndrome(2020-04-01T00:00:00Z) Yeniocak, Ali; Güneren, Ethem; Özkan, Mustafa; Temel, Metin; Kelahmetoğlu, Osman; YENİOCAK, ALİ; GÜNEREN, ETHEM; KELAHMETOĞLU, OSMANBackground: Nicolau syndrome (NS) is a rare complication that develops after the administration of intramuscular diclofenac sodium. The etiology and surgical treatments of 11 patients with NS were evaluated and studies in the literature were examined. The aim of this study was to compose a basic algorithm for surgical approaches to treat NS. Materials and methods: Eleven patients were evaluated for NS between December 2013 and January 2018. Two patients did not accept treatment, and nine patients underwent surgical debridement of necrotic tissues. The tissue defects of five patients were closed with a fasciocutaneous flap and, in four patients, the defects were repaired primarily. Results: No complications, such as wound infection, wound dehiscence, seroma, or flap necrosis, were encountered. Of the seven patients who received concurrent antibiotic therapy, no patient had any problems at their follow-up (2-30 months). The results were satisfactory from an aesthetic and functional point of view. Conclusion: NS was more frequent in women with a high body mass index and high fat in gluteal regions. We considered that any kind of medication could lead to NS. Different methods are discussed for treatment.Publication A bibliometric analysis of rheumatology and COVID-19 researches(2021-07-01T00:00:00Z) PASİN, ÖZGE; Pasin, Tugce; PASİN, ÖZGEObjective COVID-19 has had a substantial impact on rheumatology. The most important clinical findings are anemia, leucopenia and trombocytopenia or their combinations. The MDS patients could have chronic asymptomatic cytopenias or symptomatic anemia, infections and bleeding disorders and there is a variable risk of transformation to acute leukemia. It is generally an elderly disease and though, its- etiology is unknown most of the time; chemotherapy and radiotherapy may exert a mutant effect on the stem cells. The incidence of MDS with unknown etiology is 4/100000 and this ratio exceeds that of acute myeloid leukemia.Publication A brief overview to ageing-related organ damage: A light and electron microscopic approach to several systems(2022-06-01T00:00:00Z) Eşrefoğlu, Mukaddes; EŞREFOĞLU, MUKADDESPublication A Brief Reconnoitre about Effects of MMP9 on Aortic Dissection(2021-11-01T00:00:00Z) Uluganyan, Mahmut; ULUGANYAN, MAHMUTPublication A CADASIL Case Presenting with Progressive Bulbar Palsy Caused by Acute Simultaneous Multiple Subcortical Infarcts(2019-04-01T00:00:00Z) GÖKÇAL, ELİF; KOLUKISA, MEHMET; Mustafayev, Nihat; ASİL, Talip; GÖKÇAL, ELİF; KOLUKISA, MEHMET; ASİL, TALIPCerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is an adult-onset inherited small vessel disease of the brain caused by NOTCH3 mutations. Clinical characteristics of CADASIL include recurrent infarctions, migraine with aura, mood disturbances and cognitive impairment. We report a 35 year-old migraine-free patient with unusual presentation with acute simultaneous multiple subcortical infarctions causing progressive bulbar palsy as the initial manifestation. Hepresented with slurred speech progressing to anarthria and bilateral palsy of lower motor cranial nerves within 24 hour. Initial acute subcortical infarcts enlarged and new infarcts developed in bilateral hemispheres. There was diffuse leukoencephalopathy involving temporal horns and the patient had positive familiy history of migraine and young stroke. CADASIL was diagnosed genetically when NOTCH3 mutationwas shown. The patient improved after treatment with liquid hydration and antiplatelet agent. We experienced with the patient that CADASIL may present with unusual progressive manifestations and bilateral simultaneous infarctions. Irrespective of clinical presentation, patients with leukoencephalopathy and positive familiy history should be tested for NOTCH3 mutation.Publication A case of angioedema-like atypic scleromyxedema responding to treatment with steroid(2016-01-01T00:00:00Z) Polat, Aysegul; KAPICIOĞLU, YELDA; Sahin, NURHAN; Yilmaz, Mikail; ŞAHİN, NURHANLichen myxedematosus is a chronic, inflammatory, systemic dermatose characterized by dermal musin deposition and increased fibroblasts in the absence of thyroid dysfunction. It is usually seen together with paraproteinemia. It is clinically classified as scleromyxedema (papular mucinosis), localized lichen myxedematosus, and atypical lichen myxedematosus. Etiopathogenesis of the disease which is very difficult to treat is still unknown. Herein, we present a case of atypical scleromyxedema without monoclonal gammopathy mimicking angioedema, rapidly developing and responding to steroid treatment.
