    THE EFFECT OF INTERMITTENT FASTING ON DIABETES MELLITUS

    Diabetes mellitus (DM) is a chronic disease characterized by high blood sugar levels, which occurs as a result of insufficient or insufficient effect of the hormone insulin secreted by the pancreas. In this disease, symptoms such as dry mouth, excessive drinking of water and consequent frequent urination, numbness in the feet, foot cramps, weakness and fatigue may be observed. Increased frequency of urination at night is also one of the symptoms of diabetes. According to scientific data, intermittent fasting has gained popularity recently which can improve brain health, help weight control, protect against diseases and prolong life. The most commonly used models of intermittent fasting, which is also known as a diet, are the 16:8 and 5:2 methods. In the absence of caloric intake for a certain period of time, there is a healthy decrease in blood sugar, insulin levels drop, insulin resistance decreases, and metabolism switches to fat-burning mode when fasting for more than 14 hours. In addition, when fasting for more than 16 hours, the process of autophagy begins in cells. A reported meta-analysis suggests that intermittent fasting may be an appropriate dietary strategy in patients with Type 2 DM (T2DM). However, the risk of hypoglycemia during fasting may still be a significant barrier in people on insulin therapy. Further studies are needed to assess whether intermittent fasting is safe in patients with diabetes. In this review work, we will discuss in detail the effects of intermittent fasting in diabetic patients, the role of this type of diet on DM disease and the mechanisms by which it works in the body.     Otizm Spektrum Bozukluğunda Beslenme Yaklaşımları

    İlk kez \"infantil otizm\" olarak adlandırılan, Otizm Spektrum Bozukluğu (OSB) çocuklarda en yaygın görülen nörogelişimsel bozukluklardan biridir. Ortalama 3 yaşlarında tanı koyulmaktadır ve sıklıkla morfolojik, fizyolojik (gastrointestinal sorunlar ve uyku bozuklukları) ve psikiyatrik durumları içeren hastalıkların eşliğiyle yaşam boyu devam etmektedir. İletişim bozuklukları, takıntılı ve tekrarlanan davranışlar, hafızada zayıflık, dikkat dağınıklığı ve motor becerileri iyi kullanamama görülen semptomlardandır. Bu temel semptomların yanı sıra, otizmli kişilerde eşlik eden psikiyatrik veya nörolojik bozukluklar da bulunmaktadır. Otizmli bireylerde görülen semptomların azaltabilmesi ve daha kaliteli bir yaşam sürebilmeleri için de çeşitli beslenme programları uygulanmaktadır. Eliminasyon diyeti, glutensiz beslenme, ketojenik diyet, candida ve feingold diyeti bu beslenme programları arasında yer alır. OSB’li çocuklarda uygulanmış beslenme programlarının sonucunda gelişimleri, yaşamsal faaliyetleri ve ilerleyen yıllardaki değişimleri gözlenmiştir. Yapılan çalışmalara göre OSB’li çocuklarda aşırı şekerli ve paketli besinler kısıtlandığında çocuklarda konsantrasyon artabilmektedir. Ketojenik diyet ve kazeinsiz-glutensiz diyetin de iyileştirici etkileri olduğu görülmüştür. Bu derlemede otizmli bireylerin sağlık sorunlarına yönelik çeşitli beslenme yaklaşımlarının etkilerinin araştırılması amaçlanmıştır.

    Distal femur morphology and the suitability of standard guides for knee arthroplasty in the Turkish population

    Objective: The aim of this study was to evaluate the posterior condylar angle (PCA) and condylar twist angle (CTA) of the distal femur in the Turkish population and its concordance with the current standard prosthesis guides used in total knee arthroplasty (TKA). Methods: Two hundred and forty knees of 120 Turkish subjects (60 male and 60 female) were included in this study. PCA, CTA, femoral mediolateral lengths (fML), medial femoral anteroposterior lengths (fMAP), lateral femoral anteroposterior lengths (fLAP), distances between the trochlear groove and fMAP (DBTG–fMAP), distances between the trochlear groove and fLAP (DBTG–fLAP), medial posterior condylar cartilage thickness (MPCCT) and lateral posterior condylar cartilage thicknesses (LPCCT) were measured on magnetic resonance imaging (MRI). Results: The median CTA was 7° (range: 0°-13.0°) and the median PCA was 4° (range 0°-11.0°) (P <.0001). The median fML was 79.5 mm (range: 65.7-98.9). The median length of the fMAP was 58.2 mm (range: 46.8-69.0) and the median length of fLAP was 58.2 mm (range: 48.4-73.0). The DBTG–fMAP was 15.2 mm (range: 5.2-23.2), and DBTG–fLAP length was 21.9mm (range: 16.4-29.4). The median MPCCT and LPCCT were 2.4 mm (range: 1.6-3.6) and 2.3 mm (range: 1.2-2.8), respectively. The intraclass correlation coefficient for quantifying interobserver and intraobserver reliability showed excellent agreement regarding the PCA and CTA. Conclusion: This study has shown us that PCA and CTA may be higher in the Turkish population. Although it is not known whether these results have any clinical utility, it may be useful for surgeons to keep this in mind to prevent femoral component malposition.     DESIGN AND EVALUATION OF PROPOLIS-HPC/PVP ELECTROSPUN NANOFIBERS FOR POSSIBLE CUTANEOUS APPLICATIONS

    The aim of the study was to prepare propolis loaded hydroxypropyl cellulose and polyvinyl pyrrolidone (HPC/PVP) nanofibers, suitable for cutaneous applications. LC-HRMS analyses were applied for the identification and quantification of the bio-compounds in the propolis extract. Electrospinning was applied in order to prepare propolis HPC/PVP nanofibers with increasing concentration of propolis 10%, 20% and 30 % (v/v) and subsequently characterized in terms of morphology, wettability, thermal degradation and biological properties. SEM images showed homogenous features of the nanofibers and average diameter between 197.2-268.4 nm, high antioxidant activity, very good biocompatibility and no cytotoxicity. Particularly, with the highest propolis concentration, 30-PE/HPC/PVP nanofiber formulation is a promising alternative to conventional treatments for cutaneous applications.

    The role of oxidative stress and inflammation biomarkers in pre- and postoperative monitoring of prostate cancer patients

    IntroductionProstate Cancer (PC) is a global health concern affecting men worldwide. Oxidative stress is believed to contribute to the initiation of early-stage PC lesions. Additionally, inflammation has long been acknowledged as a factor in the development of PC. We aimed to examine the biomarkers of oxidative stress and inflammation in PC patients before and after surgery.Patients and methodsA cross-sectional study was conducted at the Urology Outpatient Clinic of Bezmialem Vakif University Hospital. A total of 150 individuals were included in the study, divided into five groups: 50 Healthy controls, 25 patients with Benign Prostatic Hyperplasia (BPH), 25 patients with Low-Risk Prostate Cancer (LRPC), 25 patients with Medium-Risk Prostate Cancer (MRPC), and 25 patients with High-Risk Prostate Cancer (HRPC). Measurements of Total Oxidant Status (TOS), Total Antioxidant Status (TAS), Total Thiol (TT), and Native Thiol (NT) were performed using photometric methods. Oxidative Stress Index (OSI) and Disulfide (DIS) levels were calculated mathematically. Levels of Interleukin-10 (IL-10), Interleukin-1beta (IL-1 beta), Tumor Necrosis Factor-alpha (TNF-alpha), Interleukin-6 (IL-6), and Presepsin were determined using commercially available enzyme-linked immunosorbent assay (ELISA) kits.ResultsCompared to the healthy control group, the results indicated a statistically significant increase in both oxidative stress and inflammation levels. In the groups receiving both pharmaceutical therapy and surgical treatment (PC), a significant decrease in oxidative stress and inflammation levels was observed.ConclusionConsequently, it is suggested that the assessment of oxidative stress and inflammatory biomarkers should be incorporated in the pre- and postoperative monitoring of patients with PC. Total Antioxidant Status (TAS) levels are found to be statistically lower in all PC groups, indicating a correlation between oxidative stress and the progression of PC.Levels of inflammatory biomarkers (IL-1 beta, IL-6, IL-10, TNF-alpha) were found to be higher before and after surgery in PC groups, and their variation correlated with tumor grade and size.Post-surgery, a decrease in presepsin levels is associated with a reduced likelihood of sepsis in PC patients.Reductions in oxidative stress and inflammation levels postoperatively suggest the effectiveness of surgical intervention in mitigating these factors.The potential for personalized medicine to decrease PC mortality is highlighted by better understanding the functional relationship coordinating inflammatory signatures in the tumor microenvironment.

    Metabolik sendrom; vücuttaki bir dizi risk faktörünün birleşimiyle ortaya çıkan karmaşık bir durumdur abdominal yağlanma, yüksek kan basıncı, insülin direnci ve dislipidemi gibi faktörlerin bir araya gelmesiyle tanımlanır. Son yıllarda, sindirim sistemindeki mikroorganizmaların (mikrobiyota) metabolik sendromun gelişimi üzerindeki etkileri üzerine yapılan araştırmalar önemli bir ilgi odağı haline gelmiştir. Metabolik sendromun patogenezi, genetik ve çevresel faktörlerin etkileşimiyle karmaşık bir şekilde şekillenir. Özellikle, barsak mikrobiyotasının dengesizliği, metabolik sendromun oluşumunda potansiyel bir etken olarak öne çıkmaktadır. Mikrobiyotanın dengesizliği, barsak bariyerinde bozulmaya, düşük dereceli inflamasyona ve metabolik süreçlerde değişikliklere yol açarak insülin direnci gibi temel bileşenlerle ilişkilendirilmiştir. Bu alandaki araştırmalar, sağlıklı bir mikrobiyota profilinin metabolik sendrom riskini azaltabileceğini ve belirtilerini hafifletebileceğini öne sürmektedir. Bununla birlikte, spesifik mikrobiyota bileşenlerinin ve metabolik sendrom arasındaki korelasyonun tam olarak anlaşılması için daha fazla çalışma gerekmektedir. Mikrobiyota manipülasyonunun, metabolik sendromun önlenmesi ve tedavisinde potansiyel bir strateji olabileceği düşünülmektedir. Probiyotikler, prebiyotikler ve diyet değişiklikleri gibi yaklaşımlar, mikrobiyota sağlığını iyileştirmeye ve metabolik sendrom belirtilerini yönetmeye yönelik umut verici bulgular sunmaktadır. Sonuç olarak, metabolik sendrom ile mikrobiyota arasındaki ilişki karmaşıktır ve henüz tam olarak anlaşılmamıştır. Ancak, bu alandaki araştırmaların ilerlemesi, metabolik sendromun önlenmesi ve tedavisinde yeni ve etkili stratejiler geliştirilmesine ışık tutabilir. Bu derleme, literatürden yararlanarak metabolik sendrom ile mikrobiyota arasındaki ilişkinin karmaşıklığını ve potansiyel tedavi yöntemlerini vurgulamayı amaçlarken detayları belirtmek yerine genel bir bakış sunarak, bu alandaki araştırmaların önemini vurgulamayı hedeflemektedir.
