| dc.description | Metastatic gastric cancer is associated with high mortality, and systemic therapy remains the cornerstone of treatment. However, the role of gastrectomy directed at the primary tumor and its potential contribution to overall survival remain controversial. Gastrectomy may be performed for palliation of tumor-related complications such as bleeding, obstruction, or perforation, and reduction of tumor burden has also been proposed as a potential mechanism for improving survival. Nevertheless, patients undergoing surgery often represent a selected population with better performance status, lower comorbidity burden, and more favorable clinical characteristics, which may introduce substantial selection bias in retrospective studies. Therefore, the association between gastrectomy and survival should be evaluated while accounting for relevant clinical and prognostic characteristics. This study aimed to evaluate the association between gastrectomy and overall survival in patients with metastatic gastric cancer compared with patients receiving systemic therapy alone. METHODS In this retrospective cohort study, the electronic medical records and archived files of 755 patients who were followed and treated at the Medical Oncology Clinic of Bezmialem Vakıf University between 2013 and 2025 were reviewed. A total of 247 patients with metastatic gastric adenocarcinoma who met the predefined inclusion and exclusion criteria were included in the study. The primary endpoint was overall survival, defined as the time from diagnosis to death or last follow-up. Patients who were alive at the last follow-up were censored at that date. Survival was estimated using the Kaplan–Meier method, and groups were compared using the log-rank test. The association between gastrectomy and overall survival was evaluated using Cox regression models adjusted for clinical and prognostic variables. Time-dependent Cox regression, a 3-month landmark analysis, and propensity score–based sensitivity analyses were performed to assess potential biases related to treatment selection and the timing of surgery. RESULTS Of the 247 patients included in the study, 85 underwent gastrectomy and 162 received systemic therapy alone. During follow-up, 228 deaths occurred, while 19 patients were alive or censored at the last follow-up. In the primary Kaplan–Meier analysis, median overall survival was 20.8 months (95% CI: 16.7–28.4) in the gastrectomy group and 9.7 months (95% CI: 8.2–11.0) in the systemic therapy–only group. The 12- and 24- month overall survival rates were 82.2% and 44.4%, respectively, in the gastrectomy group, compared with 38.9% and 12.5% in the systemic therapy–only group (log-rank p<0.001). In univariable analysis, gastrectomy was associated with longer overall survival (HR: 0.34; 95% CI: 0.25–0.46; p<0.001). In the multivariable Cox regression model adjusted for age, sex, ECOG performance status, Charlson comorbidity index, baseline AJCC stage IV status, metastatic site, prognostic nutritional index, systemic immune- xvii inflammation index, and HER2 status, gastrectomy remained associated with longer overall survival (HR: 0.39; 95% CI: 0.22–0.68; p<0.001). The direction of the association was maintained in the analysis using the date of metastasis as the time origin (adjusted HR: 0.41; 95% CI: 0.30–0.57; p<0.001), the 3- month landmark analysis (HR: 0.56; 95% CI: 0.40–0.78; p=0.001), the time-dependent Cox model (HR: 0.57; 95% CI: 0.35–0.91; p=0.018), the analysis restricted to patients with baseline AJCC stage IV disease (HR: 0.32; 95% CI: 0.17–0.62; p<0.001), and the propensity score covariate-adjusted analysis (HR: 0.51; 95% CI: 0.32–0.81; p=0.005). Restricted mean survival time analyses at 24 and 36 months demonstrated between- group differences favoring gastrectomy of 7.1 and 10.4 months, respectively (both p<0.001). CONCLUSION In this retrospective cohort study, gastrectomy was associated with longer overall survival compared with systemic therapy alone in patients with metastatic gastric cancer. This association persisted after multivariable adjustment for measured clinical and prognostic variables and remained directionally consistent across different sensitivity analyses. Nevertheless, substantial differences in baseline characteristics and treatment selection between the groups could not be fully eliminated because of the retrospective observational design. Residual confounding due to unmeasured or incompletely controlled variables therefore cannot be excluded, and the findings should not be interpreted as evidence that gastrectomy provides a causal survival benefit. Rather, they indicate an observational association between gastrectomy and longer overall survival that persisted after adjustment for measured clinical and prognostic factors. These findings emphasize the importance of careful patient selection and multidisciplinary decision-making when considering surgery in metastatic gastric cancer. Prospective controlled studies incorporating surgical morbidity, symptom control, quality of life, metastatic disease burden, response to systemic therapy, and the timing of surgery are needed to better define the patient population in whom gastrectomy may be appropriate. | dc.description.abstract | Metastatik mide kanseri, yüksek mortalite ile seyreden ve tedavisinin temelini sistemik tedavinin oluşturduğu malignitelerden biridir. Bununla birlikte, primer tümöre yönelik gastrektominin bu hasta grubundaki yeri ve genel sağkalıma olası katkısı ise tartışmalıdır. Gastrektomi, kanama, obstrüksiyon veya perforasyon gibi tümöre bağlı komplikasyonların kontrolü amacıyla palyatif olarak uygulanabilmekte; tümör yükünün azaltılması yoluyla sağkalım üzerinde olumlu bir etkisi olabileceği de öne sürülmektedir. Ancak cerrahi uygulanan hastaların çoğunlukla daha iyi performans durumu, daha düşük komorbidite yükü ve daha uygun klinik özelliklere sahip seçilmiş bir grubu oluşturması, retrospektif çalışmalarda seçim yanlılığı olasılığını gündeme getirmektedir. Bu nedenle gastrektominin sağkalım üzerindeki olası etkisinin, hastaların klinik ve prognostik özellikleri dikkate alınarak değerlendirilmesi önem taşımaktadır. Bu çalışmada, metastatik mide kanseri tanılı hastalarda gastrektomi uygulanmasının, yalnızca sistemik tedavi alan hastalarla karşılaştırıldığında genel sağkalım ile ilişkisinin değerlendirilmesi amaçlandı. YÖNTEM Bu retrospektif kohort çalışmasında Bezmialem Vakıf Üniversitesi Medikal Onkoloji Kliniği'nde 2013–2025 yılları arasında takip ve tedavisi yürütülmüş 755 hastanın sistem ve dosya arşivi incelenmiş; önceden belirlenen dahil etme ve dışlama kriterlerini karşılayan metastatik mide adenokarsinomu tanılı 247 hasta çalışmaya dahil edilmiştir. Çalışmanın birincil sonlanım noktası genel sağkalım olarak belirlenmiştir. Genel sağkalım, tanı tarihinden ölüm tarihine veya son kontrol tarihine kadar geçen süre ay olarak hesaplanmış; ölüm olayı gerçekleşmeyen hastalar son kontrol tarihinde sansürlenmiştir. Sağkalım analizleri Kaplan–Meier yöntemi ile gerçekleştirilmiş ve gruplar log-rank testi ile karşılaştırılmıştır. Gastrektomi ile genel sağkalım arasındaki ilişki; yaş, cinsiyet, ECOG performans durumu, Charlson komorbidite indeksi, metastaz bölgesi, prognostik nutrisyonel indeks, sistemik immün-inflamasyon indeksi ve HER2 durumu gibi klinik ve prognostik değişkenler için ayarlanmış Cox regresyon modelleri ile değerlendirilmiş; cerrahi zamanlaması ve hasta seçimine bağlı olası yanlılıkları incelemek amacıyla zaman-bağımlı Cox, 3. ay landmark ve eğilim skoru temelli duyarlılık analizleri uygulanmıştır. BULGULAR Çalışmaya dahil edilen 247 hastanın 85'ine gastrektomi, 162'sine yalnızca sistemik tedavi uygulandı. İzlem sürecinde 228 ölüm gerçekleşti; 19 hasta son takip tarihinde sağ veya sansürlü idi. Ana Kaplan–Meier analizinde medyan genel sağkalım gastrektomi grubunda 20,8 ay (%95 GA: 16,7–28,4), yalnızca sistemik tedavi grubunda ise 9,7 ay (%95 GA: 8,2–11,0) olarak saptandı. Gastrektomi grubunda 12 ve 24 aylık genel sağkalım oranları sırasıyla %82,2 ve %44,4 iken, yalnızca sistemik tedavi grubunda bu oranlar sırasıyla %38,9 ve %12,5 idi (log-rank p<0,001). Tek değişkenli analizde gastrektomi daha uzun genel sağkalım ile ilişkili bulundu (HR: 0,34; %95 GA: 0,25–0,46; p<0,001). Çok değişkenli Cox regresyon analizinde yaş, xv cinsiyet, ECOG performans durumu, Charlson komorbidite indeksi, başlangıç AJCC evre IV durumu, metastaz bölgesi, prognostik nutrisyonel indeks, sistemik immün- inflamasyon indeksi ve HER2 durumu için düzeltme sonrasında gastrektomi uygulanması daha uzun genel sağkalım ile ilişkili kaldı (HR: 0,39; %95 GA: 0,22– 0,68; p<0,001). Bu ilişkinin yönü metastaz tarihinden itibaren yapılan analizde (ayarlı HR: 0,41; %95 GA: 0,30–0,57; p<0,001), üç aylık landmark analizinde (HR: 0,56; %95 GA: 0,40– 0,78; p=0,001), zaman-bağımlı Cox modelinde (HR: 0,57; %95 GA: 0,35–0,91; p=0,018), başlangıç AJCC evre IV hastalarla sınırlı analizde (HR: 0,32; %95 GA: 0,17–0,62; p<0,001) ve eğilim skoru kovaryat ayarlı analizde (HR: 0,51; %95 GA: 0,32–0,81; p=0,005) korundu. Kısıtlanmış ortalama sağkalım süresi analizinde, gastrektomi grubunda yalnızca sistemik tedavi alan hastalara kıyasla 24 ve 36 aylık zaman sınırlarında sırasıyla 7,1 ve 10,4 aylık sağkalım farkı saptanmıştır (her iki analiz için p<0,001). SONUÇ Bu retrospektif kohort çalışmasında, metastatik mide kanseri tanılı hastalarda gastrektomi uygulanması, yalnızca sistemik tedavi alan hastalara kıyasla daha uzun genel sağkalım ile ilişkili bulunmuştur. Bu ilişkinin, klinik ve prognostik değişkenler için yapılan çok değişkenli düzeltme sonrasında ve farklı duyarlılık analizlerinde tutarlı biçimde sürmesi, bulgunun istatistiksel sağlamlığını desteklemektedir. Bununla birlikte, retrospektif tasarımın doğası gereği gruplar arasındaki başlangıç klinik özellikleri ve tedavi seçimine ilişkin belirgin farklılıklar tamamen ortadan kaldırılamamıştır. Ölçülmeyen ya da mevcut modellerde yeterince kontrol edilemeyen potansiyel değişkenlerin sonuçlar üzerindeki etkisi dışlanamayacağından, elde edilen bulgular gastrektominin nedensel bir sağkalım avantajı sağladığının kanıtı olarak değerlendirilmemelidir. Bulgular, bu hasta kohortunda gastrektomi uygulanması ile daha uzun genel sağkalım arasında, ölçülen klinik ve prognostik değişkenler için yapılan ayarlama sonrasında da devam eden gözlemsel bir ilişki bulunduğunu göstermektedir. Bu bulgular, metastatik mide kanserinde cerrahi kararının hasta seçimini, performans durumunu, hastalık yükünü, sistemik tedaviye yanıtı ve cerrahi riskleri birlikte değerlendirilerek multidisipliner biçimde ele alınması gerektiğini düşündürmektedir. Cerrahi morbidite, semptomatik yarar, yaşam kalitesi ve sistemik tedavinin zamanlamasını birlikte değerlendirecek prospektif kontrollü çalışmalara ihtiyaç vardır.
