Abstract

Amaç: Bu çalışmanın amacı, diyabetik retinopatili (DRP) hastalarda serum soluble ürokinaz plazminojen aktivatör reseptörü (suPAR) düzeylerini ölçmek ve retinopati olmayan diyabetes mellitus hastaları (DM) ve sağlıklı gönüllülerin serum düzeyleri ile karşılaştırmaktır. Materyal ve Metod: Bezmialem Vakıf Üniversitesi Hastanesi Göz Hastalıkları Anabilim Dalı'nda Şubat–Nisan 2026 tarihleri arasında yürütülen bu kesitsel çalışmaya toplam 156 katılımcı dahil edildi. Katılımcılar; DRP'li hastalar (n=52), DM'li hastalar (n=58) ve sağlıklı kontroller (n=53) olmak üzere üç gruba ayrıldı. Tüm katılımcılarda serum suPAR, IL-1β, VEGF, CRP ve HbA1c düzeyleri ölçüldü; OKT ile santral maküla kalınlığı (SMK) ve koroid vasküler indeksi (KVİ) değerlendirildi. İstatistiksel analizde Kruskal-Wallis, One-Way ANOVA, Spearman korelasyonu ve çok değişkenli lineer regresyon yöntemleri kullanıldı. Bulgular: Serum suPAR düzeyleri DRP grubunda (medyan: 11,11 ng/mL), hem DM grubuna (7,69 ng/mL) hem de kontrol grubuna (6,62 ng/mL) kıyasla istatistiksel olarak ileri derecede anlamlı biçimde yüksek bulundu (p<0,001). suPAR; IL-1β (rs=0,496), VEGF (rs=0,509), HbA1c (rs=0,450) ve SMK (rs=0,222) ile anlamlı pozitif korelasyon gösterdi. Çok değişkenli lineer regresyon analizinde DRP varlığı (β=0,492, p<0,001), IL-1β (β=0,375, p<0,001) ve VEGF (β=0,173, p=0,034), suPAR'ın bağımsız belirleyicileri olarak saptandı ve bu üç değişken birlikte suPAR varyansının %62,6'sını açıkladı. CRP her iki diyabetik grupta yüksek olmakla birlikte DRP ile DM grupları arasında ayrım yapamadı (p=0,541). KVİ gruplar arasında anlamlı farklılık göstermedi (p=0,972). Sonuç: Bu çalışma, suPAR'ın diyabetik retinopatide demografik, klinik ve biyokimyasal değişkenlerden bağımsız olarak yükselebildiğini; inflamatuvar (IL-1β) ve anjiyojenik (VEGF) süreçlerle güçlü korelasyon sergilediğini ortaya koymaktadır. suPAR, retinopatisi olmayan diyabetik bireylerden retinal mikrovasküler tutulumu olanları ayırt edebilme kapasitesiyle CRP'ye kıyasla daha özgül bir biyobelirteç adayı olarak öne çıkmaktadır. Bu bulguların prospektif ve daha geniş örneklemli çalışmalarla doğrulanması, suPAR'ın klinik izlemdeki rolünün belirlenmesi açısından gereklidir. Objective: The aim of this study was to measure serum soluble urokinase plasminogen activator receptor (suPAR) levels in patients with diabetic retinopathy (DR) and to compare them with the serum levels of diabetes mellitus (DM) patients without retinopathy and of healthy volunteers. Materials and Methods: A total of 156 participants were included in this cross-sectional study, conducted at the Department of Ophthalmology, Bezmialem Vakıf University Hospital, between February and April 2026. Participants were divided into three groups: patients with DR (n=52), patients with DM (n=58), and healthy controls (n=53). Serum suPAR, IL-1β, VEGF, CRP, and HbA1c levels were measured in all participants; central macular thickness (CMT) and choroidal vascularity index (CVI) were assessed by OCT. Kruskal-Wallis, one-way ANOVA, Spearman correlation, and multivariate linear regression methods were used in the statistical analysis. Results: Serum suPAR levels were found to be statistically highly significantly elevated in the DR group (median: 11.11 ng/mL) compared with both the DM group (7.69 ng/mL) and the control group (6.62 ng/mL) (p<0.001). suPAR showed significant positive correlations with IL-1β (rs=0.496), VEGF (rs=0.509), HbA1c (rs=0.450), and CMT (rs=0.222). In multivariate linear regression analysis, the presence of DR (β=0.492, p<0.001), IL-1β (β=0.375, p<0.001), and VEGF (β=0.173, p=0.034) were identified as independent predictors of suPAR, and these three variables together explained 62.6% of the variance in suPAR. Although CRP was elevated in both diabetic groups, it could not discriminate between the DR and DM groups (p=0.541). CVI showed no significant difference among the groups (p=0.972). Conclusion: This study demonstrates that suPAR can be elevated in diabetic retinopathy independently of demographic, clinical, and biochemical variables, and that it exhibits strong correlations with inflammatory (IL-1β) and angiogenic (VEGF) processes. With its capacity to distinguish diabetic individuals with retinal microvascular involvement from those without retinopathy, suPAR stands out as a more specific candidate biomarker compared with CRP. Validation of these findings in prospective studies with larger samples is needed to determine the role of suPAR in clinical monitoring.
