dc.descriptionObjective: This study aimed to investigate the impact of anticholinergic and sedative drug burden, calculated using the Drug Burden Index (DBI), on mortality and comprehensive geriatric assessment parameters in older adults diagnosed with Alzheimer's disease (AD) and dementia with Lewy bodies (DLB). Materials and Methods: This retrospective cohort study included patients aged 65 years and older diagnosed with AD or DLB who were followed at the Geriatrics Outpatient Clinic of Bezmialem Vakıf University Faculty of Medicine Hospital. Comprehensive geriatric assessment included evaluation of cognitive function using the Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment (MoCA); functional status using the Barthel Index of Activities of Daily Living and the Lawton–Brody Instrumental Activities of Daily Living Scale; nutritional status using the Mini Nutritional Assessment (MNA); sarcopenia risk using the SARC-F questionnaire; and balance and gait performance using the Tinetti Performance- Oriented Mobility Assessment and the Timed Up and Go (TUG) test. Anticholinergic and sedative drug burden was quantified using the Drug Burden Index (DBI), and patients were classified into three groups: no burden, low burden, and high burden. The primary outcome was all-cause mortality. Results: A total of 392 patients were included in the study, comprising 270 (68.9%) patients with AD and 122 (31.1%) patients with DLB. The mean age of the participants was 85.17 ± 7.01 years, and 74.7% were female. In the AD group, higher sarcopenia risk (SARC-F, p = 0.009), greater functional dependence (Barthel Index, p = 0.007), impaired balance and gait performance (Tinetti, p = 0.049), and poorer nutritional status (MNA, p = 0.036). In contrast, no significant differences were observed between the burden groups regarding cognitive performance (MMSE), depressive symptoms (Geriatric Depression Scale), or history of falls. Among patients with DLB, no significant associations were identified between anticholinergic drug burden and any comprehensive geriatric assessment parameter. The mean overall survival for the entire cohort was 47.07 months (SE = 1.54; 95% CI: 40.90–57.10). The mean survival was 50.10 months (SE = 1.85; 95% CI: 43.62– 68.38) in the AD group and 40.22 months (SE = 2.62; 95% CI: 20.84–51.16) in the DLB group, with significantly shorter survival observed in patients with DLB (log- rank p = 0.003). No significant association was found between anticholinergic drug burden and survival in either the AD group (p = 0.147) or the DLB group (p = 0.943). Conclusion: Anticholinergic and sedative drug burden, as measured by the Drug Burden Index, was not associated with mortality in patients with Alzheimer's disease or dementia with Lewy bodies. However, in patients with AD, a higher anticholinergic drug burden was associated with increased frailty, greater sarcopenia risk, greater functional dependence, impaired balance and gait performance, and poorer nutritional status. In contrast, no significant associations were observed between anticholinergic drug burden and either mortality or comprehensive geriatric xvi assessment parameters in patients with DLB. Although survival was significantly shorter in patients with DLB than in those with AD, this difference was not explained by anticholinergic drug burden. These findings suggest that, in older adults with dementia, anticholinergic drug burden may have greater clinical relevance for functional and nutritional outcomes than for mortality, underscoring the importance of regular medication review strategies in this population. dc.description.abstractAmaç: Bu çalışmada, Alzheimer hastalığı (AH) ve Lewy cisimcikli demans (LCD) tanılı yaşlı bireylerde Drug Burden Index (DBI) ile hesaplanan antikolinerjik ve sedatif ilaç yükünün mortalite ve kapsamlı geriatrik değerlendirme parametreleri üzerindeki etkisinin incelenmesi amaçlandı. Gereç ve Yöntem: Bu retrospektif kohort çalışmaya, Bezmialem Vakıf Üniversitesi Tıp Fakültesi Hastanesi Geriatri Polikliniği'nde takip edilen 65 yaş ve üzeri AH ve LCD tanılı hastalar dahil edildi. Kapsamlı geriatrik değerlendirme kapsamında bilişsel durum Mini Mental Durum Değerlendirme Testi (MMSE) ve Montreal Bilişsel Değerlendirme Ölçeği (MoCA), fonksiyonel durum Barthel Günlük Yaşam Aktiviteleri ve Lawton-Brody Enstrümantal Günlük Yaşam Aktiviteleri ölçekleri, beslenme durumu Mini Nütrisyonel Değerlendirme (MNA), sarkopeni riski SARC-F ve denge-yürüme performansı Tinetti ile Timed Up and Go testleri kullanılarak değerlendirildi. Antikolinerjik ve sedatif etkiye sahip ilaçlara maruz kalma düzeyini niceliksel olarak değerlendiren Drug Burden Index (DBI) ile hastalar yük yok, düşük yük ve yüksek yük gruplarına ayrıldı. Birincil sonlanım noktası tüm nedenlere bağlı mortalite olarak belirlendi. Bulgular: Çalışmaya toplam 392 hasta dahil edildi; bunların 270'i (%68,9) AH ve 122'si (%31,1) LCD tanılıydı. Hastaların ortalama yaşı 85,17±7,01 yıl olup katılımcıların %74,7'si kadındı. AH grubunda artan antikolinerjik yük; daha yüksek sarkopeni riski (SARC-F, p=0,009), artmış fonksiyonel bağımlılık (Barthel, p=0,007), bozulmuş denge ve yürüme performansı (Tinetti, p=0,049) ve kötüleşmiş beslenme durumu (MNA, p=0,036) ile ilişkili bulundu. Buna karşın bilişsel durum (MMSE), depresif belirtiler (GDS) ve düşme öyküsü açısından gruplar arasında anlamlı farklılık saptanmadı. LCD grubunda ise antikolinerjik yük ile geriatrik değerlendirme parametreleri arasında anlamlı ilişki izlenmedi. Tüm örneklemde ortalama sağkalım süresi 47,07 ay (SE=1,54; %95 GA: 40,90–57,10) olarak bulundu. AH hastalarında ortalama sağkalım süresi 50,10 ay (SE=1,85; %95 GA: 43,62–68,38), LCD hastalarında ise 40,22 ay (SE=2,62; %95 GA: 20,84–51,16) olup LCD grubunda sağkalım anlamlı olarak daha kısa bulundu (log-rank p=0,003). Antikolinerjik ilaç yükü ile sağkalım arasında ne AH grubunda (p=0,147) ne de LCD grubunda (p=0,943) anlamlı ilişki saptandı. Sonuç: DBI ile hesaplanan antikolinerjik ve sedatif ilaç yükü, Alzheimer hastalığı ve Lewy cisimcikli demansta mortalite ile ilişkili bulunmamıştır. Bununla birlikte özellikle Alzheimer hastalarında yüksek antikolinerjik yük; kırılganlık, sarkopeni riski, fonksiyonel bağımlılık, denge-yürüme performansında bozulma ve malnütrisyon ile ilişkilidir. Buna karşılık Lewy cisimcikli demans hastalarında antikolinerjik yük ile ne mortalite ne de geriatrik değerlendirme parametreleri arasında anlamlı ilişki gösterilememiştir. Lewy cisimcikli demanslı hastalarda sağkalım Alzheimer hastalarına göre belirgin olarak daha kısa olmasına rağmen, antikolinerjik yük bu farklılığı açıklamamaktadır. Bu bulgular, demanslı yaşlı bireylerde antikolinerjik ilaç yükünün mortaliteden çok fonksiyonel ve nutrisyonel sonuçlar üzerinde klinik önem xiv taşıdığını düşündürmekte ve düzenli ilaç gözden geçirme stratejilerinin gerekliliğini desteklemektedir.
