    Evaluation of cerebral hemodynamic status in patients with unilateral symptomatic carotid artery stenosis during motor tasks, through use of transcranial Doppler sonography

    Background: Carotid artery stenosis increases cerebral ischemic event risk through changing different cerebral hemodynamic parameters. Objective: To investigate how cerebral hemodynamics in the M1 segment of middle cerebral artery change in patients with carotid artery stenosis, after motor tasks using transcranial Doppler sonography (TCD). Methods: Thirty-two healthy subjects and 30 patients with unilateral symptomatic carotid artery stenosis were recruited. The patient population was divided into three groups according to the degree of stenosis (group 1: ≥50 to 69%, group 2: 70 to 89% and group 3: ≥90 to 99%). TCD was used to measure the pulsatility index (PI) and cerebral vasomotor reactivity (CVR). Results: In the patient group, significant differences for symptomatic side PI values (p=0.01) and mean CVR increases (p=0.05) were observed, compared with the healthy controls. However, the difference was not statistically significant for asymptomatic side PI values and mean CVR increases. The results from the intergroup comparison showed significantly higher percentages of symptomatic and asymptomatic side CVR increases in group 1, compared with groups 2 and 3 (p=0.001 and p=0.002, respectively). Conclusions: Our study showed that cerebral autoregulation and hemodynamic mechanisms are impaired in patients with carotid artery stenosis. Furthermore, the impairment of PI and CVR tends to get worse with increasing degrees of stenosis. In addition, this study demonstrated that assessment of these two hemodynamic parameters in clinical practice might be helpful for monitoring the progress of carotid artery stenosis.

    Co-occurrence of Acute Coronary Syndrome and Stroke: Trousseau-s Syndrome

    In 1865, Trousseau remarked that there was a possibility for an unforeseen or mobile thrombophlebitis to be a sign of an occult visceral malignancy. Trousseau-s syndrome is often a comorbid of mucin-positive carcinomas in patients who have cancer and it includes chronic disseminated intravascular coagulopathy associated with microangiopathy, verrucous endocarditis and arterial emboli. 86 year old male patient who had a prostate cancer for 6 years came to our emergency department with a complaint of not being able to use his right hand. We founded that there was diffusion limitation in MR diffusion and cerebrovascular disease was diagnosed by MR diffusion. There was R segment losses in the inferior and anteroseptal leads of patients- ECG, had a QS pattern in the inferior leads. Serum samples had high levels of troponin. His treatment was started with 300 mg of acetylsalicylic acid and medical treatments were added. For patients who have had malignancy diagnosis, Trousseau-s syndrome should be kept in mind and doctors should be alert for thrombotic events that may develop secondarily.

    Effects of stroke etiology on angiographic success in patients undergoing mechanical thrombectomy

    Objective In this study, we investigated the relationship between stroke etiology and recanalization success with endovascular treatment in patients with acute ischemic stroke. Material and methods A total of 109 patients with anterior circulation stroke who underwent mechanical thrombectomy between August 2017 and June 2019 were included in the study retrospectively. Stroke etiologies of the patients were determined according to the TOAST criteria (Trial of Org 10,172 acute stroke treatment), and the relationship between stroke etiology and successful first-pass recanalization rate with endovascular treatment, total successful recanalization rate, and procedure time was evaluated. Results The data of 109 patients who presented with anterior circulation stroke and underwent mechanical thrombectomy were retrospectively analyzed. Fifty-five (50.5%) of the patients were female and 54 (49.5%) were male, and mean age was 67.3 +/- 12.9. When the stroke etiologies of the patients were evaluated, it was found that 47 (43.1%) were due to large vessel ateherosclerotic disease (LVAD), and 62 (56.9%) were cardioembolic-related. Recanalization success and clinical outcomes did not differ significantly in patients with LVAD and those with cardioembolic etiology (p > 0.05). In addition, the number of patients with modified Rankin score (mRS) 6 in the atherosclerotic group was significantly higher than in the cardioembolic group (p = 0.022). Conclusion Recanalization success and clinical outcomes did not differ significantly in patients with LVAD and those with cardioembolic etiology. However, mortality rate was higher in patients with atherosclerotic etiology, due to the complexity of the procedure and the high rate of reocclusion.

Bu çalışmada, Covid-19 pandemisi sırasında kliniğimizde yatırılarak takip ve tedavi edilen, Covid-19 enfeksiyonu veya aşısı ile zamansal ilişkisi saptanan hastaların, klinik özelliklerinin belgelenmesi amaçlanmıştır.Gereç ve Yöntem:Türkiye’de pandeminin başladığı 11 Mart 2020 tarihinden pandeminin sona erdiği 31 Mayıs 2023’e kadar yatırılarak izlenen 1905 hastadan, belirti ve bulgularının başlamasından önceki son 6 hafta içinde, 1) PCR test pozitifliği ve/veya Covid-19 enfeksiyonuyla uyumlu görüntüleme ve klinik bulguları ile Covid-19 tanısı almış, 2) Covid-19 aşısı olmuş hastalar değerlendirmeye alındı.Bulgular:Kırkbeş (%0.02) hastada Covid-19 enfeksiyonu (5’inde post-enfeksiyöz ensefalit, 2’sinde kronik inflamatuar demiyelinizan polinöropati, 1’inde 6. kranial sinir paralizisi, 4’ünde ensefalopati, 1’inde dermatomiyozit, 2’sinde diplopi, 3’ünde polinöropati, 4’ünde demiyelinizan hastalık, 3’ünde Guillain-Barre sendromu, 1’inde miyastenia gravis, 1’inde otoimmün ensefalit, 1’inde santral pontin miyelinolizis, 12’sinde iskemik inme, 2’sinde sinüs ven trombozu, 3’ünde transvers miyelit), 4 (%0.002) hastada Covid-19 aşısı (2’sinde Guillain-Barre sendromu,1’inde 6. kranial sinir paralizisi, 1’inde dermatomiyozit ) ile nörolojik tablo arasında zamansal ilişki saptandı. Aşı ile ilişkili olguların tümü Biontech aşısından sonra gelişmişti.Sonuç:Covid-19 ile ilşkilendirilebilecek nörolojik bozukluklar değerlendirildiğinde, meta-analiz çalışmalarında miyalji, tat ve/veya koku kaybı, baş ağrısı ve özellikle yaşlı hastalarda akut konfüzyon-deliryum gibi nörolojik bulgular ile yakın ilişki kurulabilmiştir. İnme prevelansında İnfluenza çalışmaları ile kıyaslanarak artış olduğu düşünülmektedir, fakat genel popülasyon ile karşılaştırma yapılamamaktadır. Geçirilen enfeksiyonun şiddeti ile de nörolojik durumlar arasında net bir ilişki kurulamamıştır. 39 aylık süreçte kliniğimizde yatırılarak incelenen hastalarda da belli bir hastalık grubu için özel bir sıklık artışı dikkati çekmemiştir.
