    PHYSIOTHERAPISTS ARE ERGONOMICALLY AT RISK

    ProblemMusculoskeletal problems are frequently encountered related to the occupational workload and unsuitable physical features of workplace. Ergonomic analysis of both people and the workplace to reveal occupational risk factors prevents the musculoskeletal problems and makes people more efficient in their workplaces. We aimed to determine occupational risk factors by ergonomically analyzing physiotherapists and their workplace.MethodsAll physiotherapists (6 male, 2 female with the mean age of 30±1.2) who work in the intensive care unit, neurology, hand surgery, orthopedics and pediatrics departments of Bezmialem Vakif University which is quite new established were included in this study. Their posture analysis were done during working. Rapid Entire Body Assessment (REBA) during patient assessment and treatment was used for ergonomic analysis of the participants. General Social Survey 2010 were used to question participants work-related quality of life. Parameters of workplace ergonomic analysis were room temperature, noise, lighting, and scent.ResultsParticipants mean REBA score was 7.13±2.23 (4-10). This indicated that physiotherapists were at medium risk and they should take precautions. Physiotherapists working at neurology unit had more REBA score (10) than the others and those of intensive care unit had the lowest score (4) among them. We found that their workplace was noisy, lack of air condition and enough lightening. According to the results of General Social Survey 2010, physiotherapists liked what they were doing, but they complained about working conditions and determined that their workplace limited them and had negative effect on their physical and mental health.ConclusionParallel to the literature, the outcomes of this study showed that physiotherapists at this university hospital were ergonomically at risk. The results of this study should be considered to design workplaces ergonomically and also to educate physiotherapists with a mind of ergonomics.

    Comparison of Non-specific Low Back Pain Between Hospital and Home Care Health Providers

    Objective: Non-specific low back pain has become a major public health problem worldwide. Depending on theoccupational loading, non-specific low back pain is frequently encountered in health care providers. The aim of this study is to comparenon-specific low back pain between hospital and home care health providers.Methods: Thirty health care providers (physiotherapist, ergotherapist, nurse, physical therapy assistant) were included to study. Health care providers in hospital was referred as group 1, health careproviders in home care was referred as group 2. The relation between non-specific low back pain and body mass index (BMI), working-years, daily working hours, and their area of speciality (orthopedic, neurologic, cardiopulmonary, geriatrics) were recorded. Non-specific low back pain was measured by Quebec Back Pain Disability Scale (QUEBEC). A visual analog scale (VAS) was used to score the intensity of the pain.Results: The mean age and Body Mass Index (BMI) scores for group 1 were 35.73±10.95 years and 24.8±3.19 kg/m2; for group 2 were 30.93±6.49 years and 24.41±4.32 kg/m2, respectively. Comparison between groups, VAS activity and night scores were statistically significiant(p<0.01). Group 1 had higher VAS scores. The correlation between the QUEBEC and the VAS activity scores were excellent(r 0.5, p<0.01), but the QUEBEC and the VAS rest scores were correlated poorly.Conclusions: Non-specific low back pain is a serious problem among health care providers. These findings demonstrated that health care providers working in hospital have higher rate of non-specific low back pain. Daily working hours, working-years, and BMI may be associated with non-specific low back pain. Working at hospital cause greater risk because of hard working conditions. There is still need for proper training and precautions to decrease of non-specific low back pain.

    Kemirgenlerin Primer Somatoduyusal Korteksinde Özel Bir Yapı: Barrel Korteks Gelişimi, Anatomik ve Fonksiyonel Organizasyonu

    Bu derlemenin amacı, kemirgenlerde bıyıklara ait barrel korteks yapısının gelişimi, anatomisi ve fonksiyonel organizasyonuna ait genel bir çerçeve sunmaktır. Duyu sistemleri, doğumdan sonraki ilk aylarda hızlı bir şekilde gelişerek beynin davranışsal bir tepki seçmek için yorumlaması gereken dış çevre hakkında ham bilgi sağlamaktadır. Tüm duyuların arasında özellikle sosyal davranış ve mekânsal öğrenmede önemli olan taktil uyarılar deri üzerinde bulunan farklı somatoduyusal reseptörlerin aktive edilmesi ile etkilerini göstermektedir. Kemirgenlerin burunlarının her iki yanında 5 sıra halinde dizilmiş bıyıklardan elde edilen taktil duyusal sinyaller; nesnelere ve çevreye ait -ne- ve -nerede- bilgilerinin elde edilmesi için kortekste nöral temsiller oluşturmaktadır. Bu duyu sisteminin en çarpıcı alanı, her bir bıyık için 4. katmanda ayrı ve iyi tanımlanmış bir yapı ile temsil edilen primer somatoduyusal -barrel- kortekstir. Her bir bıyık hareketi, kıl folikülünü inerve eden mekanik kapılı iyon kanallarını açarak depolarizasyona neden olmaktadır. Ortaya çıkan depolarizasyon, trigeminal sinirin infraorbital dalının duyu nöronlarında aksiyon potansiyelini tetikleyerek beyin sapında yer alan ilk sinapsta glutamat salınımını uyarır. Beyin sapı nöronları, talamusa duyusal bilgi gönderir; burada ikinci bir glutamaterjik sinaps, birincil somatoduyusal barrel korteksine yansıyan talamokortikal nöronları uyararak bu somatotopik haritanın temelini oluşturan katman 4’e iletim gerçekleşir. Woolsey ve Loos tarafından ilk tanımlanmasından bu yana geçen yarım yüzyıldır, kemirgenlerin primer somatoduyusal korteksinde bıyıklara özgü net topografik organizasyonu sunan barrel korteks sistemi, duyusal işleme, deneyime bağlı plastisite ve davranış arasındaki ilişkiyi incelemek için sıklıkla tercih edilen bir model sistem haline gelmiştir.
