    Comparison of MRI Features of Invasive Pleomorphic and Classical Lobular Carcinoma: Differentiation Is Possible?

    To evaluate breast MRI and DWI and demographic features of pleomorphic invasive lobular carcinoma (pILC) and classic invasive lobular carcinoma (cILC). Invasive lobular (ILC) is the second most common breast malignancy after invasive ductal carcinoma (IDC) and constitutes the 8-14% of all invasive breast cancers. ILC morphologically can be classified into the classic, alveolar, solid, tubulolobular, and pleomorphic subtypes according to WHO. This study was performed retrospectively. The MRI and demographic features of 18 patients with 23 pILC were compared with those 22 consecutive patients with 27 cILC. There was no significant difference in demographic features of patients, MR appearance, kinetics, and ADC values between two groups. pILC, an aggressive subtype of ILC, cannot be differentiated from cILC with breast MRI.

    A Six Sigma Trial For Reduction of Error Rates in Pathology Laboratory

    Objective: The aim of this study is to determine differences of levels of empathy among undergraduates in each year of their four-year programs of physiotherapy. Methods: During the 2014-2015 academic school year, 381 physiotherapy students were enlisted from two universities in Istanbul, one a foundation and the other a government university. The Turkish version of the Jefferson Scale of Physician Empathy was administered. Students were asked to indicate interest in particular physiotherapy specialties as well as their region of origin in Turkey. The Kruskal-Wallis analysis was used to determine differences among the four study years, and also to measure relationships between specialty interest, home-region, and empathy scores of the students. Empathy scores were also compared according to gender. Results: The difference of empathy scores between the students of the two universities was borderline significant (p=0.057). Empathy scores in both universities increased to a significant degree after school entrance and decreased in the final year. Levels of empathy did not change according to gender, specialty interest, or home-region (p=0.722, 0.524, and 0.309, respectively). Conclusions: This study points to the need for physiotherapy curricula that would enhance empathy and give students practice in exhibiting this valuable attribute. Additional studies are needed that would include larger study populations and track the same students year by year as to how and why their empathy levels change during their training.

    The role of PET and MRI in evaluating the feasibility of skin-sparing mastectomy following neoadjuvant therapy.

    Abstract Objective: To investigate the role of positron emission tomography (PET) and magnetic resonance imaging (MRI) in evaluating the feasibility of skin-sparing mastectomy in patients with locally-advanced breast cancer (LABC) who will undergo neoadjuvant chemotherapy (NAC) by evaluating the sensitivity and specificity of PET and MRI compared with skin biopsy results before and after NAC treatment. Methods: Patients with LABC who were treated with NAC between November 2013 and November 2015 were included in this study. Demographic, clinical, radiological and histopathological features of the patients were recorded. Results: A total of 30 patients were included in the study with a mean age of 52.6 years (range, 35– 70 years). Sensitivity and specificity for detecting skin involvement in LABC was 100%/10% (62%/ 85%) with MRI and 60%/80% (12%/92%) with PET before (after) NAC, respectively. When radiological skin involvement was assessed in relation to the final histopathological results, the preNAC PET results and histopathological skin involvement were not significantly different; and there was no difference between postNAC MRI and histopathological skin involvement. Conclusions: As preNAC PET and postNAC MRI more accurately determined skin involvement, it might be possible to use these two radiological evaluation methods together to assess patient suitability for skin-sparing mastectomy in selected patients.

    Mucous membrane plasmacytosis is a rare reactive process characterized by plasma cell infiltration of mucous membranes for unknown reasons. It may be confused with malignancy because of its exophytic structure and pavement appereances. A laryngeal polypoid lesion was sent to the pathology department, clinically called fibroma, and presented to emphasize the diagnostic and differential diagnostic criteria.

    Solitary Metastasis of Pulmonary Adenocarcinoma in Restricted Thyroid Gland Nodule

    Metastatic tumors of thyroid are rare and constitute 1.2% of all thyroid malignancies. Metastasis to thyroid gland from head and neck region occurs usually via direct spread. Hematogenous metastases are mostly seen in kidney, breast, and lung tumors and malignant melanomas. Thyroid gland metastasis is usually seen in widespread diseases. Solitary metastasis in this gland is extremely rare. A 68-year-old male patient was admitted to the general surgery clinic with swelling in the neck. The patient had undergone right lobectomy in another hospital due to lung cancer two years ago. Ultrasonography revealed the presence of nodules in both lobes with maximum diameter of 4.5 cm in the right lobe and 2 cm in the left lobe. Bilateral total thyroidectomy was performed. Nodules with diameter of 4.5 and 3.5 cm were observed in the right lobe. In the histopathologic examination of a small nodule, adenocarcinoma infiltration which was 2 cm in diameter was observed. Tumoral infiltration was not observed in nodule capsule and thyroid capsule. Immunohistochemically, tumor was positive for carcinoembryonic antigen (CEA), weakly positive for thyroid transcription factor-1 (TTF-1), and negative for thyroglobulin, chromogranin A, and calcitonin. Slides of lobectomy material were reexamined and it was observed that the two tumors had the same properties. The case was reported as -pulmonary adenocarcinoma metastasis in thyroid.- No other metastasis was observed in the whole body scan of the patient. In conclusion, in the presence of nodule in thyroid gland, possibility of metastasis should be considered if a history of malignancy exists.

    In vivo Efficacy of HIFU (High Intensity Focused Ultrasound) on Mice with Ehrlich Ascites Carcinoma

    Objective: Cavitation and coagulation necrosis due to high temperature caused by application of the high intensity focused ultrasound (HIFU) to a focal area occurs. This feature makes HIFU possible to use in the destruction of cancerous tissue. In this study, it was aimed to determine the effect of HIFU on Balb/c mice with Ehrlich ascites tumor model to reach effective usage of HIFU with its clinical and experimental applications in various types of cancer.

    Unpredictable behavior of breast phyllodes tumors

    – OBJECTIVE: Phyllodes Tumor is a rare fibroepithelial breast tumor with variable malignant potential. These tumors that currently accepted nomenclature are benign, borderline and malignant according to the World Health Organization. It tends to behave in a benign way but it may undergo a malignant transformation. The purpose of this study was to present our Case load of phyllodes tumor, and evaluate the management and follow-up the behavior of these tumors. PATIENTS AND METHODS: 36 patients with phyllodes tumor were treated in Bezmialem Vakif University and Okmeydani Training and Research Hospitals analyzed retrospectively. Demographics, presenting symptoms, history, local examination, preoperative clinical diagnosis, treatment, postoperative histopatology, followup period were evaluated retrospectively and studied. RESULTS: A total of 36 patients treated for phyllodes tumor were analyzed. Mean age of the patients was 38 (14-79) and all were females. The presentation of the patients was breast lump in all cases, either as a primary or recurrent lump presentation. The right and left breast were affected 58% and 42% respectively. 34 of these 36 cases are briefly mentioned in this paper but the two cases, one with sarcomatous malignant transformation and the other with fatal metastatic phyllodes tumor in an adolescent female are discussed in detail. The mean duration of the symptomatic mass was 9.8 ± 7.3 months (range 0.5 to 24). CONCLUSIONS: Phyllodes tumor is a type of tumor that may be benign or malignant. The type with malignant features may be fatal. The tumors diagnosed as benign may transform and become malignant because of unknown reasons and may be an aggressive tumor.

    A Pathological Process Should be Carefully Conducted in the Breast: Mucocele-Like Lesion

    Mucoccic-like lesion (MLL) of the breast has been firstly described as a benign lesion resembling mucinous carcinoma of the breast. Subsequently, it has also been reported to be associated with ductal hyperplasia or breast carcinoma. The risk of cancer development in the long term is unknown. Therefore, the approach to pure MLL of the breast detected during percutaneous biopsy is controversial. In this report, a mucocele-like breast lesion in a 47-year-old woman was first noticed during a core biopsy and later detected to be associated with ductal carcinoma in-situ focuses in excisional biopsy material is presented. The aim of this paper was to provide an overview of this rare and controversial lesion and its variabilities with regard of the presented case.
