    Diş Hekimliğinde Genel Anestezi Uygulamaları

    emporomandibuler eklem (TME) değerlendirmesi pre-operatif havayolu muayenesinin önemli bir parçası olarak kabul edilmektedir. TME disfonksiyonu zor entübasyona neden olabilir veya genel anestezi işlemleri sırasında TME yapıları hasar görebilir. TME ankilozu ya da kilitlenme gösteren disk deplasmanları ağız açıklığında şiddetli kısıtlılığa neden olarak anestezi için uyanık fiberoptik entübasyon gibi özel teknikler gerektirirler. Buna karşılık anestezi indüksiyonu, laringoskop kullanımı veya entübasyon manevralarına bağlı olarak TME’de dislokasyon, disk deplasmanları ve postoperatif dönemde kas ağrıları meydana gelebilir. Geçmişte TME problemi hikayesi olan hastalarda ise mevcut semptomların artması söz konusudur. Bu nedenle, hastalar genel anestezi sonrası oluşabilecek TME komplikasyonları bakımından ayrıntılı biçimde bilgilendirilmeli, mevcut problemlerin işlem öncesi giderilmesi için gerekli tedavilerin başlatılması sağlanmalıdır. Anestezi hekimleri, genel anestezi sonrası meydana gelebilecek TME komplikasyonlarının teşhis ve acil tedavisi konusunda bilgi sahibi olmalı, gerekli durumlarda diş hekimliği konsültasyonu istemelidir. Bu derlemede, genel anestezi işlemleri ile ilişkili TME problemleri tartışılmaktadır.

    Bistüri Olmadan Dental Cerrahi Mümkün Müdür?

    Bu çalışmada oral patolojilerin tedavilerinde cerrahi yöntemlerin gerekliliği veya zamanlaması hakkında güncel bilgilerin sunulması hedeflenmiştir. Gereç ve Yöntem: Çalışmada oral patolojisi olup da cerrahi endikasyonu olan ve opere olan veya opere olamayan hastalar incelenmiş olup, hastaların tedavilerinin prognozlarını nasıl etkilediğini gösterilmiştir. Bulgular ve Sonuç: Yapılan incelemeler sonucunda takibi yapılan hastalar arasında malignite ile karşılaşılan bir hastanın, bir yıl sonra mortaliteyle sonuçlandığı ve bir vakanın da hiç bir dental müdahele yapılmamasına rağmen tamamen iyileştiği gözlenmiştir. Bu sonuçlar değerlendirildiğinde planlanan cerrahilerin kararını verirken geniş ve birkaç bölgeden biyopsi yapılma ihtiyacıyla beraber, bazı özel durumlarda da cerrahi kararından vazgeçmenin hastanın oral patolojisinin tedavisi için doğru endikasyon olabileceği konusu rapor edilmiştir.

    Maxillofacial Surgery and Craniofacial Deformity: Practices and Updates

    Body dysmorphic disorder (BDD) may be related to the appearance of a body part or may sometimes arise from concerns about a body function. Currently, this disorder was included in contemporary classification systems with DSM-5. The majority of BDD patients first consult dermatologists, surgeons, and more often plastic surgeons, rather than psychiatrists. Therefore, it is difficult to determine the prevalence of this disorder in the psychiatric society. The oral and maxillofacial region is highly associated with face deformities, and the patients with BDD are applying to those clinics even without self-awareness of their disorders. It has been reported that most of the orthognathic surgical patients are associated with the facial appearance of surgical motivations and will have similar psychological motivations to cosmetic surgery patients. Moreover, the orthodontics, prosthetic and restorative dentistry are the branches of dentistry that mostly the patients come with esthetic complaints. Studies on BDD have not yet received the value they deserve concerning the prevalence and severity. Researches in dentistry and oral and maxillofacial surgery are much less, and the individuals suffering from BDD are not well-known among dentists/oral and maxillofacial surgeons; therefore, the frequency of BDD patients is not noticed and treated properly.

    Evaluation of Dental Anxiety Levels of Students from Different Departments

    Objective: This study aimed to evaluate and compare the anxiety levels among dentistry students and students from other departments and to assess the effects of classes at pre-clinic labs or dentistry clinics on the dental fear of dentistry students. Materials and Methods: A total of 993 students, including 744 (75%) women and 249 (25%) men, were asked to answer the Modified Dental Anxiety scale (MDAS) and Dental Fear scale. Four groups were created according to the departments: Dentistry (group 1), medicine (group 2), health-related (group 3) and others (group 4). The participants scored their anxiety levels from 1 (no anxiety) to 5 (high anxiety). Statistical analysis was performed and values of p<0.05 were considered statistically significant. Results: The dentistry students scored lower than the other three groups in the MDAS (p<0.001). For the education process of dentistry, no significant difference was observed in other questions regarding pre-clinic dentistry and dentistry clinic periods (p>0.05). Conclusion: Results of this study showed that the dental anxiety levels of dentistry students were lower than those of students from other departments and that preclinic or dentistry clinic classes did not affect dental anxiety.

    Osteochondroma of The Mandibular Condyle: A Case Report

    Objective: Osteochondroma or osteocartilaginous exostosis, is a cartilage-capped exophytic lesion that arises from the cortex of a bone. Although osteochondroma is considered as the most common tumor of skeletal bones, it is relatively rare in the jaw. The osteochondroma of the mandible occurs at the condyle or the tip of the coronoid process whereas called Jacob’s Disease when mandibular coronoid process is involved. In this report, we present an osteochondroma case extending from the mandibular condyle to the cranial base. Case: A 55-year male patient was referred to Department of Oral and Maxillofacial Surgery of Bezmialem Vakif University Dentistry Faculty with complaints of mouth opening limitation and facial asymmetry. The distance of maximum mouth opening was 9 mm in length. A computed tomography scan showed the right condylar processes hyperplasia. The condyle and pathological segment resections were performed with interpositional gap arthroplasty and abdominal fat graft was preferred. After the operation, the patient’s maximal incisal opening was 45 mm. Pathological examination revealed irregularly arranged fibrous, cartilaginous and bony elements. Based on histological features, a final diagnosis of osteochondroma was made. No complaints were reported by the patient during 1-year follow-up. Conclusion: Osteochondroma is rarely seen on the mandibular condyle. Clinical symptoms may be similar in many temporomandibular joint diseases such as ankylosis, Jacob’s Disease etc. Therefore the differential diagnosis should be well done. Gap arthroplasty is commonly recommended as the best treatment option in such cases.
