Abstract

Enfeksiyon hastalıkları servisinde yatan hastalarda polifarmasi, komorbidite, antimikrobiyal tedavi kullanımı, böbrek fonksiyon değişiklikleri ve bakım geçişleri nedeniyle ilaç ile ilişkili sorunların (İLİS) sık görülebileceği düşünülmektedir. Bu çalışma ile enfeksiyon hastalıkları servisinde klinik eczacılık hizmetlerinin İLİS'lerin tespit edilmesi, önlenmesi ve azaltılması üzerindeki etkisinin değerlendirilmesi amaçlanmıştır. Araştırma, prospektif randomize kontrollü bir çalışma olarak iki grup şeklinde tasarlanmıştır. Çalışma, Bezmialem Vakıf Üniversitesi Tıp Fakültesi Hastanesi Enfeksiyon Hastalıkları Servisi'nde Kasım 2024-Kasım 2025 tarihleri arasında gerçekleştirilmiştir. Klinik eczacı tarafından kontrol grubundaki hastalar gözlemsel olarak izlenmiş, müdahale grubunda ise hastalara ilaç uzlaşısı yapılmış, günlük servis vizitlerine katılım sağlanmış, ilaç tedavileri değerlendirilmiş ve klinik açıdan anlamlı görülen İLİS'lere yönelik hekimlere önerilerde bulunulmuştur. İLİS sınıflandırması PCNE V9.1'e göre yapılmıştır. Çalışmaya toplam 206 hasta dahil edilmiş olup kontrol ve müdahale gruplarında 103'er hasta yer almıştır. Kontrol ve müdahale grubundaki hastaların yaş ortalaması sırasıyla 67,69±14,41 ve 69,27±14,18 yıl olarak bulunmuştur. Her iki grupta da hipertansiyon, diabetes mellitus, koroner arter hastalığı ve kronik böbrek hastalığı en sık görülen komorbiditeler arasında yer almıştır. Kontrol grubunda 581, müdahale grubunda 459 İLİS tespit edilmiştir. Klinik eczacı müdahalesi sonrasında toplam İLİS sayısı 106'ya düşmüş, hasta başına ortalama İLİS sayısı 4,46±3,88'den 1,03±1,20'ye gerilemiştir. İLİS saptanan hasta oranı ise %94,2'den %47,6'ya düşmüştür. Müdahale grubunda en sık tespit edilen sorun tedavi güvenliliği ile ilişkili bulunmuş; bunu tedavi etkililiği ve diğer sorunlar izlemiştir. Başlıca İLİS nedenleri ilaç seçimi ve doz seçimi ile ilişkili sorunlar olmuştur. Klinik eczacı müdahalesi sonrasında renal disfonksiyon ile ilişkili İLİS sayısı 72'den 0'a, antimikrobiyal ilaçlarla ilişkili İLİS sayısı 93'ten 16'ya, antitrombotik ilaçlarla ilişkili İLİS sayısı 81'den 7'ye ve diğer ilaç gruplarıyla ilişkili İLİS sayısı 205'ten 21'e gerilemiştir. Antidiyabetik ilaçlarla ilişkili İLİS sayısı ise 80'den 61'e düşmüştür. Müdahale grubunda klinik eczacı tarafından 363 öneride bulunulmuş ve bu önerilerin 353'ü (%97,2) hekimler tarafından kabul edilmiştir. Çalışmamızda enfeksiyon hastalıkları servisinde yatan hastalarda İLİS yükünün yüksek olduğu görülmüştür. Klinik eczacı tarafından gerçekleştirilen ilaç uzlaşısı, ilaç incelemesi, renal doz optimizasyonu, ilaç etkileşimi değerlendirmesi ve hekimlere sunulan öneriler sonucunda İLİS sayısında belirgin azalma sağlanmıştır. Bu sonuçlar, klinik eczacının enfeksiyon hastalıkları servisinde sağlık ekibine aktif olarak dahil edilmesinin ilaç güvenliğini artırabileceğini, tedavi sürecinin daha etkili yönetilmesine katkı sağlayabileceğini ve klinik eczacılık hizmetlerinin hastane servislerinde yaygınlaştırılmasının hasta bakım kalitesini güçlendirebileceğini göstermektedir.

Description

Hospitalized patients in infectious diseases wards are considered to be at high risk for drug-related problems (DRPs) due to polypharmacy, comorbidities, antimicrobial therapy use, renal function alterations, and transitions of care. This study aimed to evaluate the impact of clinical pharmacy services on the identification, prevention, and reduction of DRPs in an infectious diseases ward. The research was designed as a prospective randomized controlled study consisting of two groups. The study was conducted in the Infectious Diseases Ward of Bezmialem Vakıf University Faculty of Medicine Hospital between November 2024 and November 2025. Patients in the control group were observationally monitored by the clinical pharmacist, whereas in the intervention group, medication reconciliation was performed, participation in daily ward rounds was provided, medication therapies were evaluated, and recommendations were made to physicians regarding clinically significant DRPs. DRPs were classified according to PCNE V9.1. A total of 206 patients were included in the study, with 103 patients in both the control and intervention groups. The mean ages of patients in the control and intervention groups were 67.69±14.41 and 69.27±14.18 years, respectively. Hypertension, diabetes mellitus, coronary artery disease, and chronic kidney disease were among the most common comorbidities in both groups. A total of 581 DRPs were identified in the control group, while 459 DRPs were identified in the intervention group. Following clinical pharmacist intervention, the total number of DRPs decreased to 106, and the mean number of DRPs per patient declined from 4.46±3.88 to 1.03±1.20. The proportion of patients with at least one DRP decreased from 94.2% to 47.6%. In the intervention group, the most frequently identified problems were related to treatment safety, followed by treatment effectiveness and other problems. The primary causes of DRPs were associated with drug selection and dose selection. After clinical pharmacist intervention, the number of renal dysfunction-related DRPs decreased from 72 to 0, antimicrobial-related DRPs from 93 to 16, antithrombotic-related DRPs from 81 to 7, and DRPs related to other drug groups from 205 to 21. DRPs associated with antidiabetic medications decreased from 80 to 61. In the intervention group, the clinical pharmacist made 363 recommendations, of which 353 (97.2%) were accepted by physicians. In our study, the burden of DRPs among hospitalized patients in the infectious diseases ward was found to be high. Medication reconciliation, medication review, renal dose optimization, drug interaction assessment, and physician-directed recommendations performed by the clinical pharmacist resulted in a substantial reduction in DRPs. These findings suggest that the active involvement of clinical pharmacists in the healthcare team within infectious diseases wards may improve medication safety, contribute to more effective management of the treatment process, and enhance the quality of patient care through the wider implementation of clinical pharmacy services.
