    This study aims to investigate the relationship between disease duration and psychological burden in PsA and to identify the risk factors associated with psychological distress. Patients with PsA who met CASPAR classifcation criteria enrolled by Turkish League Against Rheumatism (TLAR)-Network. Patients were categorized into three groups based on disease duration: early stage (<5 years), middle stage (≥5,<10 years), and late stage (≥10 years). All patients underwent clinical and laboratory assessment using standardized protocol and case report forms. The associations between psychological variables and clinical parameters were assessed by a multivariate analysis. Of the 1113 patients with PsA (63.9% female), 564 (%50.7) had high risk for depression and 263 (%23.6) for anxiety. The risk of psychological burden was similar across all PsA groups, and patients with a higher risk of depression and anxiety also experienced greater disease activity, poorer quality of life, and physical disability. Multivariate logistic regression revealed that female gender (OR=1.52), PsAQoL (OR=1.13), HAQ (OR=1.99), FiRST score (OR=1.14), unemployment/retired (OR=1.48) and PASI head score (OR=1.41) were factors that infuenced the risk of depression, whereas the current or past enthesitis (OR=1.45), PsAQoL (OR=1.19), and FiRST score (OR=1.26) were factors that infuenced the risk of anxiety. PsA patients can experience a comparable level of psychological burden throughout the course of their disease. Several socio-demographic and disease-related factors may contribute to mental disorders in PsA. In the present era of personalized treatment for PsA, evaluating psychiatric distress can guide tailored interventions that improve overall well-being and reduce disease burden.

    The reliability and validity of the Turkish version of the Quality of Life Questionnaire of the European Foundation for Osteoporosis-31 (QUALEFFO-31)

    This study reveals the Turkish version of QUALEFFO-31 has a sufficient level of reliability, validity, and psychometric properties. The questionnaire, which is potentially capable of differentiating patients with fractures, does not appear to have the differential capacity in terms of osteoporosis. Purpose This study aims to conduct the reliability and validity study of QUALEFFO-31 in Turkish and to evaluate the capacity of the questionnaire to distinguish patients in terms of osteoporosis. Methods The original English version was translated into Turkish by two translators whose native language was Turkish. Subsequently, this Turkish version was translated back into English by two different bilingual translators whose native language was English. After this preliminary questionnaire was tested in 30 patients, words, terminology, information errors, and parts difficult to understand were revised, and the questionnaire was finalized. Internal consistency and test-retest analyses were used for the reliability study. For the validity study, convergent-discriminant validity, concurrent validity, factor analysis, known-group validity, and receiver operating characteristic (ROC) analyses were performed. Results A total of 111 patients were evaluated. Internal consistency levels were optimal except for the mental function. ICC coefficients showed good retest reliability for all domains and total tests. The convergent and discriminant validity ratios for the mental function domain were 78% for both and 100% for the other domains. There was a moderate and good negative correlation between QUALEFFO-31 and SF-36 domains which had similar names. Exploratory factor analysis revealed 3 structures. However, there was a spread to the other factors in physical function domain items. Confirmatory factor analysis (CFA) markers were not at a very good fitting level except for the relative chi-square index. When CFA was performed according to the assumed model, the fitting level increased in all analyses. There was no significant differential capacity in terms of osteoporosis or fracture for either QUALEFFO-31 or SF-36. Conclusion The Turkish version of QUALEFFO-31 has a sufficient level of reliability, validity, and psychometric properties. Nevertheless, improvements in pain and mental function domains and some changes applying to the model may increase the psychometric capacity of the questionnaire. The questionnaire, which is potentially capable of differentiating patients with fractures, does not appear to have the differential capacity in terms of osteoporosis.

    Cross-cultural adaptation and validation of the Behavioral Avoidance Test-Back Pain (BAT-Back) to the Turkish language

    Objective: Pain-avoidance is considered to be one of the major leading factors to develop a chronic low back pain (CLBP). In this study, we aimed to translate the Behavioral Avoidance Test-Back Pain (BAT-Back) into Turkish and evaluate its psychometric properties in patients with CLBP. Methods: 115 patients with CLBP filled the provided socio-demographic information form, the "Oswestry Disability Index (ODI)", the "Fear-Avoidance Beliefs Questionnaire (FABQ)", the "Tampa Scale for Kinesiophobia (TSK)", and the "Hospital Anxiety Depression Scale (HADS)". All patients and 40 pain free controls (PFC) were administered the Turkish version (TrBAT-Back) of the "Behavioral Avoidance Test-Back Pain" test. The internal consistency was evaluated with Cronbach's α coefficient. The test-retest reliability was assessed with the intraclass correlation coefficient (ICC). To evaluate the structural validity of TrBAT-Back, its correlation with FABQ, TSK, ODI, and HADS was examined. Results: The internal consistency of TrBAT-Back was excellent (Cronbach's α = 0.97) and its test-retest reliability was good (ICC = 0.87). Its high correlation with the FABQ-physical activity scores and moderate correlation with the TSK scores supported the structural validity. The TrBAT-Back scores showed a statistically significant moderate correlation with the higher level of pain and disability. Despite the low degree of positive correlation, a high level of anxiety (HADS-anxiety) is potentially associated with avoidance behaviour. CLBP patients and controls differed significantly on TrBAT-Back avoidance scores. Conclusion: We are of the opinion that, compared to the self-report scales, TrBAT-Back will provide more objective data in detecting avoidance behaviour associated with pain in Turkish speaking patients with CLBP.

    Fibromiyalji Sendromu ile Bağırsak Mikrobiyotası Arasındaki İlişkinin Araştırılması

    Fibromiyalji sendromu (FMS), yetişkin popülasyonda bildirilen prevalansı %3-10 olan, kronik yaygın ağrının sık görülen formlarından biridir. Tipik ağrının klinik görünümü ve ilişkili somatik ve psikolojik semptomların varlığı tanıya temel oluşturur. FMS sinir sistemi disfonksiyonu ile ilişkilidir ve nörotransmitterler fibromiyalji için onaylanmış bir dizi ilacın hedefi olarak rol oynamaktadır. Bununla birlikte, FMS’de altta yatan mekanizmalar kesin olarak henüz bilinmemekle birlikte birçok hipotez ortaya konmuştur. Fibromiyalji ve irritabl bağırsak sendromu (IBS) arasındaki ilişki göz önüne alındığında, değişen bağırsak mikrobiyomu fibromiyalji ile ilişkili olabilir. Bu çalışmada, FMS tanılı hastalarda sağlıklı kontrollere göre değişen bağırsak mikrobiyom düzeylerinin araştırılması amaçlanmıştır. Mikrobiyom çalışması için FMS tanılı 54 hastadan ve 36 sağlıklı kişiden oluşan kohorttan fekal örnekler toplanmıştır. Kontrol grubunda herhangi bir mental ve/veya fiziksel hastalığı olanlar çalışma dışında bırakılmıştır. FMS’li hasta grubu, -American College of Rheumatology (ACR)- 2010 tanı kriterlerine bağlı kalınarak belirlenmiştir. Fekal örnekler, kullanılana kadar -80°C’de muhafaza edilmiş ve buz üzerinde çözülmüş; her ekstraksiyon için 0.3 g feçes tartılmıştır. DNA izolasyonu, ticari kit ile üreticinin protokolüne uygun olarak gerçekleştirilmiştir. Örnekler, gerçek zamanlı polimeraz zincir yöntemi ile Bacteroidetes, Firmicutes, Enterobacter, Lactobacillus, Streptococcus ve Bifidobacterium’a özgül primerler ile 16S rRNA gen amplifikasyonu yapılarak karşılaştırılmıştır. Sonuçlara göre, hasta grubunda Bacteroidetes ve Bifidobacterium istatistiksel olarak anlamlı bir şekilde artarken (p< 0.05), Firmicutes’in azaldığı saptanmıştır (p< 0.001). Enterobacter, Streptococcus ve Lactobacillus için istatistiksel olarak anlamlı sonuç bulunamamıştır (p> 0.05). Bakteriler arasındaki ilişki değerlendirildiğinde, Bacteroidetes ile Firmicutes yüzdesi arasında istatistiksel olarak yüksek anlamlı ve negatif korelasyon bulunurken (r= -0.778, p< 0.001), Enterobacter ve Bifidobacterium yüzdesi arasında orta derecede istatistiksel anlamlılık ve pozitif korelasyon gözlenmiştir (r= 0.460, p= 0.005). Sonuçlar, bağırsak mikrobiyotasının fibromiyaljide rol oynayabileceğini göstermektedir. Bağırsaktaki Firmicutes ve Bacteroidetes filumlarının dengesinin bağırsak homeostazı için önemli etkilere sahip olduğu bilinmektedir. Özetle, daha büyük kohortlarda yapılacak büyük ölçekli araştırmaların, bağırsak mikrobiyomu ve FMS arasındaki ilişkiyi anlamada ve olası tedavi seçeneklerini değerlendirmede etkili olacağı açıktır.
