    Autosomal dominant polycystic kidney disease (ADPKD) is the most frequently occurring hereditary kidney disease, and axial spondyloarthritis (SpA) is one of the most frequently occurring rheumatic diseases. Treatment-related decisions for axial SpA may pose a challenge in case of renal involvement. -e authors describe two siblings with cooccurrence of these two diseases. -e association of these two diseases is not well known.     Fear Assessment in Inflammatory Rheumatic diseases (FAIR) questionnaire: a cross-cultural adaptation and validation to the Turkish language

    In chronic inflammatory rheumatic diseases (CIRD), it is important to understand patients- fears towards their disease in order to improve patient-physician dialog, to raise the quality of care offered, and to optimize treatment adherence. In this study, we aimed to translate the Fear Assessment in Inflammatory Rheumatic diseases (FAIR) questionnaire into Turkish and evaluate its psychometric properties in patients with CIRD. One hundred fifteen patients filled the provided socio-demographic information form, FAIR-Tr questionnaire, Hospital Anxiety and Depression Scale (HADS), and Beck-s Hopelessness Scale (BHS). For the analysis of short-term reliability, 50 patients re-filled the FAIR-Tr questionnaire 1week later. Internal consistency was evaluated with Cronbach-s coefficient and test-retest reliability was evaluated with intraclass correlation coefficients (ICC). Construct validity analysis was investigated based on the correlation with HADS and BHS. All patients found FAIR-Tr easily understandable and acceptable. FAIR-Tr internal consistency (Cronbach-s =0.93) and test-retest reliability (ICC=0.91) were excellent. Psychometric validation was proved upon observing high correlation with HADS (Anxiety, r=0.77; Depression, r=0.70) and moderate correlation with BHS (r=0.65). FAIR-Tr is a questionnaire that has excellent internal consistency and test-retest reliability. The successful correlation with HADS and BHS supported its psychometric validity in terms of evaluating the fear in CIRD cases. We think that FAIR-Tr is a specific scale that can help to evaluate the disease- and treatment-related fears of the Turkish patients with CIRD and may be useful in both routine practice and clinical studies.

    This study aims to investigate the relationship between disease duration and psychological burden in PsA and to identify the risk factors associated with psychological distress. Patients with PsA who met CASPAR classifcation criteria enrolled by Turkish League Against Rheumatism (TLAR)-Network. Patients were categorized into three groups based on disease duration: early stage (<5 years), middle stage (≥5,<10 years), and late stage (≥10 years). All patients underwent clinical and laboratory assessment using standardized protocol and case report forms. The associations between psychological variables and clinical parameters were assessed by a multivariate analysis. Of the 1113 patients with PsA (63.9% female), 564 (%50.7) had high risk for depression and 263 (%23.6) for anxiety. The risk of psychological burden was similar across all PsA groups, and patients with a higher risk of depression and anxiety also experienced greater disease activity, poorer quality of life, and physical disability. Multivariate logistic regression revealed that female gender (OR=1.52), PsAQoL (OR=1.13), HAQ (OR=1.99), FiRST score (OR=1.14), unemployment/retired (OR=1.48) and PASI head score (OR=1.41) were factors that infuenced the risk of depression, whereas the current or past enthesitis (OR=1.45), PsAQoL (OR=1.19), and FiRST score (OR=1.26) were factors that infuenced the risk of anxiety. PsA patients can experience a comparable level of psychological burden throughout the course of their disease. Several socio-demographic and disease-related factors may contribute to mental disorders in PsA. In the present era of personalized treatment for PsA, evaluating psychiatric distress can guide tailored interventions that improve overall well-being and reduce disease burden.

    Amaç:Geleneksel ve tamamlayıcı tıp (GETAT) uygulamaları dünyada gerek primer tedavi, gerek kültürel ve tarihi etkilerin yansıması, gerekse de modern tıbbi tedavilerin tamamlayıcısı olarak kullanılmaktadır. Kronik kas-iskelet sistemi hastalıkları ve ağrı GETAT-a başvuru nedenleri arasında önemli bir yer kaplamaktadır. Bu çalışmanın amacı, İstanbul Metin Sabancı Baltalimanı Kemik Hastalıkları Eğitim ve Araştırma Hastanesi, Fiziksel Tıp ve Rehabilitasyon Polikliniğine ağrı ile başvuran hastaların GETAT uygulamaları ile ilgili farkındalığını değerlendirmektir.Gereç ve Yöntemler:Bu tanımlayıcı çalışmada veriler yüz yüze görüşme tekniği ile doldurulan anketle elde edilmiştir.Bulgular:Üç yüz on sekiz hastanın %73,3-ü GETAT uygulamaları ile ilgili bilgi sahibidir, %32,1-i daha önce GETAT yaptırmıştır. En çok bilinen ve yaptırılan uygulamalar sırasıyla hacamat, masaj ve sülüktür. Bitkisel tedaviler ve gıda takviyelerinin kullanım oranı %43,7-dir. Hastaların büyük bir kısmı kronik ağrı nedeniyle GETAT-a başvurmuş olup en sık başvuru nedeni sırt ağrısıdır. GETAT uygulamalarıyla ilgili bilgilere en çok kitle iletişim araçları ve internet aracılığıyla ulaşılmaktadır. Hastaların %49,4-ü sağlık çalışanlarından GETAT ile ilgili yeterli bilgi alamadığını ifade etmiştir.Sonuç:Bu çalışma ile GETAT uygulamalarına yönelme durumunun bu konuda bilgi sahibi olma ile yakından ilişkili olduğu gözlemlenmiştir. Kronik hastalıkların yarattığı psikososyal yük ve modern tıbbi tedavilerden yeterli yanıt alınamaması gibi nedenlerle GETAT uygulamalarına duyulan ilgi artmaktadır. Bu ilginin artışında son dönemde sosyal mecralar ve internet gibi bilgiye ulaşma yollarının artması da etkili olmaktadır. Dolayısıyla doğru bilgiye ulaşılması, doğru tedavi yöntemlerinin seçimi ve istenmeyen etkilerin önlenmesi açısından hastaların sağlık çalışanları tarafından bilgilendirilmesi önem taşımaktadır.

    Fibromyalgia syndrome (FMS) is one of the frequent forms of chronic widespread pain, with a reported prevalence of 2% to 4% in the adult population. Nervous system dysfunction is implicated in FMS and neurotransmitters are the targets of a number of drugs approved for fibromyalgia. However exact underlying mechanisms of FMS are unknown yet and multiple hypotheses being suggested. Considering the relation between fibromyalgia and irritable bowel syndrome, altered gut microbiome could be associated with fibromyalgia. Herein, altered gut microbiome levels were investigated in patients with FMS compared to healthy controls. We collected faeces to study the microbiome, from a cohort of 54 patients with FMS and 36 healthy individuals. Extraction of DNA was carried out with commercial kit according to the manufacturer’s recommendations. Samples were compared using 16S rRNA gene amplification with specific primers of Bacteroidetes, Firmicutes, Enterobacteria, Lactobacillus, Streptococcus and Bifidobacterium by the Real-Time PCR instrument. According to our results, Bacteroidetes and Bifidobacteria were increased statistically significant (p<0.05), Firmicutes was decreased (p<0.001) in the patient group. No statistically significant results were found for Enterobacterium, Streptococcus and Lactobacillus (p>0.05). When the relationship between bacteria was evaluated, statistically significant and negative correlation was found between Bacteroidetes and Firmicutes percentage (r =-0.778, p <0.001). A moderately positive correlation was observed between the percentage of Enterobacter and Bifidobacter (r = 0.460, p = 0.005). The results suggest that the gut microbiota may play a role in fibromyalgia.t the gut microbiota may play a role in fibromyalgia. In sum, it is clear that large-scale further research in larger cohorts will be efficient in understanding the relationship of gut microbiome and FMS and evaluating possible treatment options.
