    Comparison of EtView (TM) tracheoscopic ventilation tube and video-assisted fiberoptic bronchoscopy during percutaneous dilatational tracheostomy

    Fiberoptic bronchoscopy (FOB) via endotracheal tube (ETT) is the most frequent utilized technique for monitoring of percutaneous dilatational tracheostomy (PDT) procedure while maintaining mechanical ventilation. Endoscopic guidance has increased the safety of this procedure; nevertheless, the use of a bronchoscope via ETT potentially may deteriorate ventilation and lead to hypercarbia and/or hypoxia. EtView tracheoscopic ventilation tube (EtView TVT) is a standard endotracheal tube with a camera and light source embedded at the tip. The objectives of this study are to introduce EtView TVT as a monitoring tool during PDT and to compare it with video assisted FOB via ETT. We hypothesized that using EtView TVT during PDT may obtain similar visualization; also may have advantages regarding better mechanical ventilation conditions when compared with video-assisted FOB via ETT. Patients, 18-75 years of age requiring mechanical ventilation scheduled for PDT were randomly allocated into two groups for airway monitorization to guide PDT procedure either with FOB via ETT (Group FOB, n = 12) or EtView TVT (Group EtView, n = 12). After standard anesthesia protocol, alveolar recruitment maneuver was applied and all patients were mechanically ventilated at pressure-controlled ventilation mode with same pressure levels. The primary outcome variable was the reduction in arterial oxygen partial pressure (PaO2) values during the procedure. Other respiratory variables and the effectiveness (the visualization and identification of relevant airway structures) of two techniques were the secondary outcome variables. Patients in both groups were comparable with respect to demographic characteristics and initial respiratory variables. Visualization and identification of relevant airway structures in any steps of the PDT procedure were also comparable. The decrease in minute ventilation in Group FOB was higher when compared with Group EtView (51 +/- 4 % vs. 12 +/- 7.3 %, p < 0.05). The decrease in PaO2 from initial levels during (34 +/- 21 % vs. 5 +/- 7 % decrease) and after (26 +/- 27 % vs. 2.8 +/- 16 % decrease) the procedure was higher in Group FOB when compared with Group EtView (p < 0.05). Considering comparable features in monitorization and advantageous features over mechanical ventilation when compared with video bronchoscopy; EtView TVT would be a good alternative for airway monitorization during PDT especially for patients with poor pulmonary reserve.

    Effects of Mandatory School-time Practice on the Preparation of Vocational Medical College Students for their Profession

    Objective: The goal of this study was to evaluate the effects of mandatory school-time practice on the preparation of second-year anesthesia nurse students for their future profession. Methods: A questionnaire was filled out by second-year anesthesia nurse students for situational awareness evaluation at the beginning of the term. The questionnaire form consisted of 19 questions and was prepared according to the basic practice on anesthesia course. Certain practical anesthesia procedures were obliged to make a certain minimum number for anesthesia nurse students. At the end of the year, the same questionnaire form was filled out again. Results: At the beginning of the study period, the ratio of students who did not feel were ready for their future professional life was 60%, whereas this ratio decreased to 20% by the end of the study period. Conclusion: Application of mandatory practical procedures for anesthesia nurse students seem to be very important in preparing them for their future professional life.

    Rat Hipotalamusunda Deksmedetomidinin İndüklediği Hipertermi Mekanizmasının Proteomiks Analizi İle Araştırılması

    Giriş / Amaç: Alfa 2 selektif adrenerjik reseptör agonisti deksmedetomidin yoğun bakımlarda kullanımına bağlı vücut sıcaklığı artışına sebep olabilmektedir.(1) Deksmedetomidine bağlı hipertermi bir dışlama tanısı olup yüksek maliyetli testler gerektirmektedir.(2-3) İlaca bağlı ateşin santral mekanizmasını ve tanıda olası biyobelirteçleri tanımlamak için hipotalamus hücrelerinde proteomiks ve biyoinformatik analiz ile değişen protein profilleri gösterilmek istenmiştir. Gereç ve Yöntem: Etik kurul onayı sonrası iki gruba ayrılan ratlara (n:5) 0,15 µg/gr deksmedetomidin ve kontrol grubuna aynı volümde serum fizyolojik intraperitoneal uygulandı. %70 karbondioksit inhalasyonu ve giyotinle sakrifiye edildi. Diseke edilen hipotalamus dokusu Filter-Aided Sample Preparation protokolü ile ayrıştırıldı. Sıvı Kromatografi-Kütle Spektrometresi ile yüksek çözünürlüklü proteomiks analizi yapıldı. PLGS Threshold Inspector biyoinformatik aracıyla protein tayini yapıldı, karşılaştırmalı analiz için Progenesis QIP yazılımı kullanıldı. Gen Seti Zenginleştirme Analizi ile insan ortologlarına çevirilen proteinler ve normalleştirilmiş zenginleştirme skorlarına (NES) göre ön plana çıkan yolaklar belirlendi. Bulgular: NES’e göre STIP 1 (Stress-induced-phosphoprotein 1) proteini ve temsil edildiği eksternal uyarlara hücresel cevap yolağı, MTCO2 (Sitokrom c oksidaz subunite 2) ve temsil edildiği biyolojik oksidasyon yolağı, PSMC6 (26S proteosome subunite) ve temsil edildiği proteozom yolağı up regülasyon göstermiştir. Oksidatif fosforilasyon yolağı ise down regülasyon göstermiştir. Tartışma / Sonuç: Sonuçlarımızda ön plana çıkan yolaklar ve anlamlı proteinler litaratürde hipertermi- hipotalamus üzerinde çalışılmış makalelerle karşılaştırıldı. Proteomiks analizi sonuçlarına göre deksmedetomidin kullanımına bağlı hipotamustaki anlamlı protein değişiklikleri, fizyolojik bir stres olan ateşin varlığını düşündürmektedir. Hipotalamusta santral ateşe dair bulgular elde etmiş olsak da hedef mekanizmaya dair sorular hala yanıt aramaktadır.     Usefulness of oxidative stress marker evaluation at admission to the intensive care unit in patients with COVID-19

    Objective:Two critical processes in the coronavirus disease 2019 (COVID-19) pandemic involve assessing patients- intensive care needs and predicting disease progression during patients- intensive care unit (ICU) stay. We aimed to evaluate oxidative stress marker status at ICU admission and ICU discharge status in patients with COVID-19.Methods:We included patients in a tertiary referral center ICU during June-December 2020. Scores of Acute Physiology and Chronic Health Evaluation II (APACHE II), Sequential Organ Failure Assessment (SOFA), and clinical severity, radiologic scores, and healthy discharge status were noted. We collected peripheral blood samples at ICU admission to evaluate total antioxidants, total oxidants, catalase, and myeloperoxidase levels.Results:Thirty-one (24 male, 7 female) patients were included. At ICU admission, patients- mean APACHE II score at ICU admission was 17.61 ± 8.9; the mean SOFA score was 6.29 ± 3.16. There was no significant relationship between clinical severity and oxidative stress (OS) markers nor between radiological imaging and COVID-19 data classification and OS levels. Differences in OS levels between patients with healthy and exitus discharge status were not significant.Conclusions:We found no significant relationship between oxidative stress marker status in patients with COVID-19 at ICU admission and patients- ICU discharge status.
