    Merkel cell carcinoma (MCC) is a rare tumor that arises from mechanoreceptor Merkel cells. Ultraviolet exposure, immunosuppression and Merkel cell polyoma virus play a significant role in tumor pathogenesis. Although it typically presents as an initially indolent growing, painless solitary lesion, the course of MCC may be aggressive due to the nodal invasion, distant metastasis and high recurrence rates. We presented a case of MCC with a background history of rheumatoid arthritis treated with immunosuppressive therapy for many years who had necrotizing granulomatous lymphadenitis.

    ÖZETAlt hava yollarında hava kaçağına, dolayısıyla pnömotoraks veya pnömomediyastinuma, neden olan patolojiler veya travma nedeniyle akciğer parankiminin zedelenmesi ile oluşan posttravmatik psoudokistler ve akciğer kontüzyonları gibi patolojiler basit, klinik gözlem ve destek tedavi ile tedavi edilebilecek derecede hafif olabileceği gibi, cerrahi müdahale gerektirebilecek şiddette olabilir. Bazı hastalar patolojinin şiddeti ve eşlik eden başka hastalık veya organ yaralanması nedeniyle hayati tehlike arz edebilirler. Spontan veya travmatik, her ne şekilde olursa olsun, havayolu ve akciğer parankimi yaralanması nedeniyle ortaya çıkan pnömotoraks, pnömomediyastinum gibi hava kaçağı ile giden durumlar erken tanınmalıdır. Kontüzyonlar ve travmatik parankim kistlerinin ayırıcı tanısı titizlikle yapılmalıdır. Tanıda, radyolojik incelemeler ilk plandadır ve önemi büyüktür.     Rhabdomyosarcoma (RMS) is a malignant form of neoplasm that originates from skeletal muscle. RMSs can exist anywhere in the human body but are more commonly detected in the neck region and extremities. The alveolar type is one of the subtypes of RMS that has a poor prognosis. Because the clinical manifestation of a tumour can be a painless mass, symptoms might be non-contributary to the diagnosis. Herein, a four-month-old girl was admitted to the emergency department with complaints of respiratory distress without a runny nose, cough, and fever. Recurrent effusions subsided with subsequent tube thoracostomy. Video-assisted thoracoscopic surgery (VATS) was performed to determine the aetiology of the recurrent effusion. The Tru-Cut biopsy obtained during VATS resulted in the diagnosis of alveolar rhabdomyosarcoma. Pleural effusion decreased, and the tube drainage was stopped rapidly after first vincristine, actinomycin-D, and cyclophosphamide chemotherapy cycle. Persistent and recurrent pleural effusions should alert physicians to rule out unusual diagnoses like that of our case.

    Self-inflicted chest stabbing complicated with tension pneumothorax (TPX) is a rare condition. We report two stabbing cases which were complicated with pneumothorax and were successfully treated.

    Sternal metastasis from uterine leiomyosarcoma treated by near-total sternectomy and reconstruction with titanium sternal stabilization and fixation system

    Uterine leiomyosarcomas are rare soft tissue neoplasms. Although they have a tendency to metastasize to distant organs, most commonly to the lung, liver and brain, rarely, sternal metastasis from a uterine leiomyosarcoma can occur. In this article, we report a 50-year-old female patient whose uterine leiomyosarcoma was treated with total abdominal hysterectomy and bilateral salphingo-oopherectomy, and who developed sternal metastasis five years later, which was completely resected and successfully repaired during the same session.
