    Intussusception usually occurs in children due to benign causes, whereas in adults, it is mostly caused by malign causes in the colon. On the other hand, the causes of small bowel intussusceptions in adulthood are usually benign, such as polyps, lipomas, adenomas, and Meckel-s diverticulum. Intestinal lipomas are an uncommon entity among benign tumors. Here, we present MR enterography features of jejunojejunal intussusception caused by an intramural lipoma.

    Diagnostic performance of apparent diffusion coefficient values for the differentiation of intrahepatic cholangiocarcinoma from gastrointestinal adenocarcinoma liver metastases

    Background: We aimed to investigate whether there is a difference between intrahepatic cholangiocarcinoma (IHCC) and liver metastases of gastrointestinal system (GIS) adenocarcinoma in terms of apparent diffusion coefficient (ADC) values. Patients and methods: From January 2018 to January 2020, we retrospectively examined 64 consecutive patients with liver metastases due to gastrointestinal system adenocarcinomas and 13 consecutive IHCC in our hospital's medical records. After exclusions, fifty-three patients with 53 liver metastases and 10 IHCC were included in our study. We divided the patients into two groups as IHCC and liver metastases of GIS adenocarcinoma. For mean apparent diffusion coefficient (ADCmean) values, the region of interests (ROI) was placed in solid portions of the lesions. ADCmean values of groups were compared. Results: The mean age of IHCC group was 62.50 ± 13.49 and mean age of metastases group was 61.15 ± 9.18. ADCmean values were significantly higher in the IHCC group compared to the metastatic group (p < 0.001). ROC curves method showed high diagnostic accuracy (AUC = 0.879) with cut-off value of < 1178 x 10-6 mm2/s for ADCmean (Sensitivity = 90.57, Specificity = 70.0, positive predictive value [PPV] = 94.1, negative predictive value [NPV] = 58.3) in differentiating adenocarcinoma metastases from IHCC. Conclusions: The present study results suggest that ADC values have a potential role for differentiation between IHCC and GIS adenocarcinoma liver metastases which may be valuable for patient management.

    Uncommon Testicular Germ Cell Tumor Thrombus in Azygos Vein Detected by FDG PET/CT

    A 40-year old man admitted to the nuclear medicine department to undergo PET/CT for restaging of known testicular germ cell tumor. The patient had a history of orchiectomy for left testis and left nephrectomy with retroperitoneal lymph node dissection. Histopathology revealed testicular germ cell tumor. During the follow-up period, there is a rapidly expanding mass in posterior mediastinum. PET reveals increased FDG activity in the azygos vein. Contrast-enhanced CT of the thorax showed distended azygos vein with heterogeneously enhancing thrombus and peripheral fat stranding. Findings are consistent with azygos vein tumor thrombosis related to testicular germ cell tumor.

    Groove (oluk) pankreatit ilk olarak 1970’li yıllarda tanımlanmış, pankreas başı, duodenum ve koledok tarafından çevrelenmiş anatomik alanın (pankreatoduodenal oluk) fibrotik skarlaşmasıyla karakterize segmental bir kronik pankreatit formudur. altta yatan patolojinin duodenum duvarında heterotopik pankreas dokusunun bulunması olduğu düşünülmektedir. merkezimizde 2013-2018 yılları arasında anahtar sözcük -groove-, -paraduodenal- yazılarak retrospektif olarak hastane veritabanından veri süzülerek toplam 18 hastanın bilgilerine ulaşıldı. hastaların temel demografik özellikleri, laboratuvar test sonuçları, CT, MRI, EUS, ERCP bulguları, yapılan tedavi ve hastaların prognozları değerlendirmeye alındı. yaş ortalaması 58 (25-87), erkek hasta sayısı 13 (%72.2), sigara kullanımı 5 (%27.8), alkol ve sigara kullanımı 6 (%33.3) hastada mevcuttu. en sık görülen semptom üst karın ağrısı idi 9(%50). bunu 5 (%27.8) hasta ile mide çıkış yolu obstrüksiyonu izledi. Serum CRP ortalaması 2.59(1-17), T.Bil: 0.635 (0.11-8.92), AST: 29(9-206), Amilaz 76(33-1359). tüm hastalara; 18(%100) EUS yapılırken, 9(%50) hastanın ERCP, 15(%83) hastanın CT, 9(%50) hastanın MR görüntülemesi mevcuttu. en sık bulgu 11(%61.1) hasta ile ödematöz duodenum duvarı idi. Bunu 7(%38.8) hastada pankreas başında ödem ve 7(%38.8) hastada pankreas başında kitle izledi. EUS-FNA yapılan 14 hastanın 8(%44.4)’inde benign sitoloji, 3(%16.6) Atipi, 3(%16.6) adeno CA olarak geldi. 8(%44) hastaya konservatif yaklaşım yapılırken, 3(%16.6) kolesistektomi, 6(%33.3) biliyer stentleme,1(%5.5) duodenal stentleme, 1(%5.5) whipple prosedürü uygulandı. ortalama takip süresi 14 (2-70) ay. 8(%44.4) hastanın semptomları gerilerken 5(%27.7) hastada rekürren pankreatit atakları oldu. 5(%%27.7) hasta takip sürecinde exitus oldu. Sonuç olarakpankreatitin bu özel bir formu olan groove pankreatitin klinik,radyolojik ve patolojik olarak tanısının konulması ve olası maligniteden ayırt edilmesi tedavi planlamasında hayati önem arz etmektedir.     Effect of cerebral microhemorrhages on neurocognitive functions in patients with end-stage renal disease.

    Background: Death is the most serious complication of intracerebral hemorrhage. Microbleeding can be a precursor of intracerebral hemorrhage. Susceptibility weighted imaging (SWI) should be included in imaging protocols for some specific groups such diabetic hemodialysis patients in terms of prediction of macrohemorrhages. Purpose: To investigate intracerebral microbleeding in hemodialysis patients and the correlation between microbleeding and neurocognitive impairment. Material and Methods: Forty-nine hemodialysis cases were involved in the study. Locations of microbleeding, correlation between microbleeding and hypertension, diabetes mellitus (DM), age, and duration of dialysis were analyzed. Standardized mini-mental test was performed. The tested cases were divided into two groups: intracerebral microbleeding (group 1, n¼26) and without intracerebral microbleeding (group 2, n¼17). Results: Incidence of microbleeding and macrohemorrhage was noted as 59% and 14%, respectively, in all cases. All macrohemorrhagic cases also have microbleeding. In group 1, neurocognitive impairment was detected in 10 (38.4%) cases: six and four cases with moderate and mild impairment, respectively. In group 2, neurocognitive impairment was detected in 2 (11.7%) cases, both with mild impairment. A significant positive correlation was detected between microbleeding and neurocognitive impairment (P¼0.031). Although there was no correlation between attention disorder and microbleeding, a positive correlation was detected between close memory impairment and microbleeding (P¼0.027). A positive correlation was detected between DM and microbleeding (P¼0.027). Conclusion: In hemodialysis patients, microbleeding can be a cause of neurocognitive impairment which will be important for guide to treatment protocols. SWI should be included in the imaging protocol of diabetic hemodialysis patients with neurocognitive deterioration.

    Unusual Cerebral Involvement of Rheumatoid Arthritis Mimicking a Tumor

    Background: The central nervous system-s involvement in Rheumatoid Arthritis (RA) is infrequent and can be life-threatening. Mass-like CNS involvement is an unusual presentation. A 45 year old man had suffered seropositive rheumatoid arthritis for five years referred to our hospital with one-week history of right-sided facial paralysis, left hemiparesis and headache.

    Primary penile lymphoma is extremely rare and there are less than 30 reported cases worldwide. Frequently penile lymphoma is seen as a secondary involvement of the penis by lymphoma, most likely by Non-Hodgkin lymphoma (NHL). Here, we report a case with penile lymphoma caused by previously known NHL.
