    A Six Sigma Trial For Reduction of Error Rates in Pathology Laboratory

    Objective: The aim of this study is to determine differences of levels of empathy among undergraduates in each year of their four-year programs of physiotherapy. Methods: During the 2014-2015 academic school year, 381 physiotherapy students were enlisted from two universities in Istanbul, one a foundation and the other a government university. The Turkish version of the Jefferson Scale of Physician Empathy was administered. Students were asked to indicate interest in particular physiotherapy specialties as well as their region of origin in Turkey. The Kruskal-Wallis analysis was used to determine differences among the four study years, and also to measure relationships between specialty interest, home-region, and empathy scores of the students. Empathy scores were also compared according to gender. Results: The difference of empathy scores between the students of the two universities was borderline significant (p=0.057). Empathy scores in both universities increased to a significant degree after school entrance and decreased in the final year. Levels of empathy did not change according to gender, specialty interest, or home-region (p=0.722, 0.524, and 0.309, respectively). Conclusions: This study points to the need for physiotherapy curricula that would enhance empathy and give students practice in exhibiting this valuable attribute. Additional studies are needed that would include larger study populations and track the same students year by year as to how and why their empathy levels change during their training.

    Yields of Dual Therapy Containing High-Dose Proton Pump Inhibitor in Eradication of H. Pylori Positive Dyspeptic Patients

    Background/Aims: H. pylon eradication has been recommended for dyspeptic patients in high prevalance regions. Triple therapies are still prescribed mostly because culture and antibiotic susceptibility tests aren-t widely available in the world. Dual therapy with high-dose proton pump inhibitors reported to have higher eradication rates. Our objective was to determine eradication success and cost-effectivity of dual therapy in dyspeptic patients. Methodology: Patients were treated orally with either dual (n:74,omeprazole 20mg q.i.d and amoxicillin 1g b.i.d) or triple therapy (n:116,omeprazole 20mg b.i.d and amoxicillin 1g b.i.d and clarithromycin 500mg b.i.d) for 14 days. HpSA was requested 3 months later. The results were evaluated statistically, p values <0,05 were considered significant. Results: Patients (n:190) were included the study((80 female,110 male, mean age: 35.6 11yearr). Alcohol/smoking, endoscopic findings and H. pylon rates with pathological examinations were not significantly different between groups whereas there was significant difference in HpFast tests(p<0.01). Whe examined with HpSA tests 3 months after the treatment eradication rate was 81.1% in the dual therapy group versus 63.8% in the triple therapy group (p:0.011) Dual therapy was economic than triple therapy (144USDvs.107USD,p<0.001). Conclusions: Dual therapy seems more successful cost-effective and is less risky in terms of side effects compared to standard triple therapy in patients with dyspepsia.

    Diffusion MRI on lymph node staging of gastric adenocarcinoma

    Objective: The purpose of this study was to evaluate the accuracy of diffusion weighted magnetic resonance imaging (MRI) in preoperative assessment of metastatic lymph nodes of gastric cancer. Methods: A total of 23 gastric cancer patients with a mean age of 59.4±10.9 years were analyzed. Lymph nodes were grouped as perigastric lesser curvature (Group Ia), perigastric greater curvature (Group Ib), D1+/D2 lymph nodes (Group II). Identification of histologically metastatic lymph nodes by diffusion weighted MRI was regarded as the main outcome. Results: A total of 1,056 lymph nodes including 180 histologically proven metastatic lymph nodes were dissected. Although diffusion weighted MRI could identify the metastatic lymph nodes in 18 out of 23 patients (77.8%), only 69 of total 1,056 nodes (6.53%), either metastatic or non-metastatic, could be detected. There was no correlation between histopathology and diffusion weighted MRI with regard to lymph node groups (P>0.05 for all). Overall accuracy was calculated as 69.56, 65.21 and 52.17 for Groups II, Ib and Ia lymph nodes, respectively. Apparent diffusion coefficient (ADC) values could not be helpful to differentiate metastatic lymph nodes (P=0.673). Conclusions: Diffusion weighted MRI has low accuracy to detect or to differentiate metastatic and non-metastatic lymph nodes based on their ADC values in gastric cancer.

    Mature ovarian cystic teratoma containing well-differentiated cerebellar tissue

    A 38-year-old female patient experienced groin pain; ultrasound imaging revealed a dermoid cystic mass in the right ovary and a cystectomy was then performed. Unusually, a mature cerebellum is found in the cyst wall. The pathological diagnosis was -mature cystic teratoma with well-differentiated cerebral and cerebellar tissue-. Glial tissue is a common neural component of teratomas, but a cerebellum is extremely rare in mature ovarian cystic teratomas. The authors report the case because of its rare component; they acknowledge that a cystic teratoma is the most common neoplasm of ovarian germ cells.

    Background: In many tumors including ovarian cancer, cell proliferation and apoptosis are important in pathogenesis and there are many alterations in most of the genes related to the cell cycle. This study was designed to evaluate immunohistochemistry with apoptotic-antiapoptotic proteins (p53, p21, bax, and bcl-2), c-kit, telomerase, and metallothionein as a diagnostic aid in typing of benign, borderline, and malignant serous and mucinous ovarian tumors.

    Immunoexpression of TTF-1 in Non-Lung Tumors

    Objective: Thyroid transcription factor-1 (TTF-1) immunoexpression is frequently determined in small-cell lung carcinomas, as well as primary lung adenocarcinomas. While dealing with metastatic carcinomas, TTF-1 immunoexpression is a significant indicator of primary lung carcinomas. Recent studies have revealed that TTF-1 immunoexpression is also defined in non-lung cancers, such as squamous cell carcinomas of different sites and certain neuroendocrine tumors. The verified data obtained from these studies indicate that a straightforward diagnosis of primary lung carcinoma in cases with positive TTF-1 immunoexpression can cause diagnostic contradictions. The aim of our study is to investigate the immunoexpression status of TTF-1 in common non-lung tumors. Methods: A total of 85 cases that were diagnosed in our institute between the years 2011-2012 were included in our study. After a review of the pathological slides prepared from these tumors [colon adenocarcinoma (n: 15), renal cell carcinoma (n: 15), prostate adenocarcinoma (n: 15), invasive papillary urothelial carcinoma (n: 15), invasive ductal carcinoma of breast (n: 15), and neuroendocrine tumors (n: 10)] TTF-1 immunohistochemistry was applied. Semiquantative evaluation based on the distribution and intensity of the staining was performed by two pathologists, respectively. Results: Positive immunostaining was evident in only 1 case of colon adenocarcinoma out of 85 cases. Conclusion: Our study demonstrated that TTF-1 immunoexpression is a very rare finding (1%) in non-lung tumors. This result provides that anti-TTF-1 is a reliable antibody in the interpretation of primary lung carcinomas. In fact, further studies with a large number of cases are needed to confirm the sensitivity and specificity of TTF-1.
