    Sağlık Çalışanlarının COVID-19 Pandemi Sürecindeki Psikososyal Uyumları: Asistan Doktorlar, Hemşireler ve Hasta Bakıcıların Ekstra İlgiye İhtiyacı Var

    Amaç: Bu çalışmada Koronavirüs hastalığı-2019 (COVID-19) pandemi sürecinde sağlık çalışanlarının psikososyal uyumu ile meslek, hastane çalışma birimi, sosyal destek ve COVID-19 tutumu arasındaki ilişkinin incelemesini amaçladık. Gereç ve Yöntem: Kesitsel ve tanımlayıcı olan bu çalışma 2 pandemi hastanesinde yürütüldü. Katılımcılar, tüm meslek guruplarından ve tüm hastane çalışma birimlerinden alınan toplam 557 sağlık çalışanı idi. Sosyo-demografik özellikler ile COVID-19 tutum ve bilgi düzeyini değerlendirmek için araştırmacılar tarafından geliştirilmiş veri formu kullanıldı. Psikososyal uyum ve sosyal desteğin değerlendirilmesi için; Hastane anksiyete-depresyon ölçeği (HADÖ), beck umutsuzluk ölçeği (BUÖ), maslach tükenmişlik ölçeği (MTÖ), COVID-19 korkusu ölçeği ve çok boyutlu algılanan sosyal destek ölçeği (ÇBASDÖ) kullanıldı. Bulgular: HADÖ-anksiyete, COVID-19 korkusu ölçeği, MTÖ-duygusal tükenme, ÇBASDÖ-arkadaş ve özel arkadaş alt ölçek puanları kadınlarda erkeklere göre daha yüksekti (p<0,05). BUÖ (en yüksek asistan doktorlarda ve hasta bakıcılarda), HADÖ-anksiyete (asistan doktorlarda en yüksek), HADÖ-depresyon (hasta bakıcılarda en yüksek), MTÖ-duygusal tükenme ve duyarsızlaşma (asistan doktorlarda en yüksek) puanları olarak görüldü ve mesleğe göre farklılık göstermekteydi (p<0,05). COVID-19 polimeraz zincirleme reaksiyonu testi yaptırmayanlarla kıyaslandıklarında,yaptıranlarda BUÖ, HADÖ-anksiyete, HADÖ-depresyon, COVID-19 korkusu puanları daha yüksek ve ÇBASDÖ puanları daha düşüktü (p<0,05). ÇBASDÖ puanları ile HADÖ-anksiyete, HADÖ-depresyon, MTÖ-duygusal tükenme ve MTÖ-duyarsızlaşma puanları arasında negatif korelasyon vardı. Sonuç: Çalışmamızın sonuçları sağlık çalışanlarının psikososyal uyumunun cinsiyet, meslek ve hastane çalışma biriminden etkilendiğini göstermiştir.     Associated factors of legal child marriage in Turkey: Pregnancy and below-average intelligence

    © The Author(s), 2021. Published by Cambridge University Press.Turkish civil code permits child marriages at 16-17 years of age, but if a child is 16 years old judicial consent is needed. Before making a final decision on marriage consent, the judges refer these children to the doctor, and ask whether they are psychologically and physically ready for marriage. While the literature on child marriage in Turkey is rich, little is known about the underlying factors leading 16-year-old girls to request legal child marriage. This study examined the socio-demographic and clinical characteristics of girls applying to the courts for judicial marriage consent. Hospital records of girls admitted to a hospital in the Istanbul province of Turkey between January 2015 and December 2018 for the assessment of their physical and mental readiness for marriage were scanned. Of the 122 girls admitted to hospital for this purpose, eight were excluded as they had incomplete data, leaving 114 participating girls. All girls were Turkish citizens and had been referred from the law courts. Being pregnant/having a child (54.4%), having had a religious (non-legal) marriage (49.1%) and having an intelligence score of less than 90 (91.2%) were common among the participants. Among those who had a religious marriage, 78.6% were pregnant/had a child at the time of evaluation and 32.1% reported that they were pregnant before their religious marriage took place. None of the participants reported being forced to marry, but many had run away from home to marry (33.3%). Among those running away to marry, the major reason for deciding to do this was being pregnant (63.2%). Running away from home to marry was found to be related to lower parental educational levels (p<0.05). Contrary to the literature, the findings indicate that girls who request judicial consent for legal child marriage in Turkey are not being forced to marry. However, lower educational level seems to be an important factor. Below-average intelligence of the girl and her parents, running away from home to marry and early unsafe sexual intercourse, leading to child pregnancy, were found to be closely associated with legal child marriage in Turkey.

    The relationship between speech difficulties and brain laterality in Attention Deficit Hyperactivity Disorder and Specific Learning Disorder

    ÖZ Amaç: Bu çalışmada Dikkat Eksikliği Hiperaktivite Bozukluğu (DEHB) ve Özgül Öğrenme Bozukluğu (ÖÖB) tanıları olan çocuklarda beyin lateralitesinin davranışsal belirteçleri ve bunların konuşma problemleri ile ilişkisinin incelenmesi amaçlanmıştır. Yöntemler: Bu kesitsel çalışma DEHB ve/veya ÖÖB tanılarına sahip 7-18 yaşındaki hastalardan oluşan klinik bir örneklem ile yürütülmüştür. Hastaların sosyodemografik, gelişimsel ve medikal bilgileri klinik görüşme sırasında alınmıştır. Beyin lateralitesinin davranışsal belirteçleri çeşitli motor görevler ile değerlendirilmiştir. Bu görevler el tercihi için -kalem ile yazma-, göz tercihi için -teleskoptan bakma-, ayak tercihi için -topa vurma- veya -tek ayak üzerinde durma- idi. Bulgular: Çalışma DEHB ve/veya ÖÖB tanısı olan toplam 130 hasta ile tamamlanmıştır. Sol el-göz ve ayak tercihi ve çapraz lateralite ÖÖB (komorbid DEHB olsun veya olmasın) tanısı olan hastalarda sık iken sadece DEHB tanısı olanlarda değildi. Sol göz/ayak tercihi konuşma bozuklukları ile ilişkili idi (p<0,05). Sonuçlar: Bu çalışma beyin lateralitesini etkileyen diğer faktörlerin kontrol edilmesinden sonra bile DEHB’nin değil ama ÖÖB’nin sol-yan tercihi ve çapraz lateralite ile ilişkili olduğunu saptamıştır. DEHB ve ÖÖB olan hastalarda konuşma bozuklukları ve sol göz/ayak tercihi arasındaki ilişkiyi gösteren diğer bulgumuz ise bu bozukluklardaki beyin lateralitesinin gelişimine aslen konuşma gelişiminin aracılık ettiğine işaret etmektedir.     The first male case with fluoxetine induced hair loss and review of the literature

    Objective: Fluoxetine is the drug of choice in the treatment of depression. It is widely preferred due to fewer side effects and greater tolerability. Hair loss is a frequent adverse effect that may occur by psychotropic drug use and that can remit by its cessation. Patients and methods: We present the diagnosis and treatment of a 26-year-old male patient. He was diagnosed with depression without psychotic features according to the DSM-V criteria and was administered fluoxetine in a dose of 20 mg/day. Results: Six weeks after the initiation of the fluoxetine treatment, the patient reported hair loss in the frontal area of the skull. These complaints regressed after cessation of drug. Conclusions: Hair loss appears to be a rare side effect of fluoxetine-based treatment. Dermatologists and psychiatrists must be informed about this adverse side effect. There might be differences in the risk of hair loss between the various SSRIs and the risk might be higher in female than in male. This male patient was the first case as far as we know in the literature. It should be kept in mind that hair loss may be observed in patients treated with fluoxetine and should be questioned in both male and female.
