    Supratip suture and skin scoring technique in the patient with thick skin in rhinoplasty

    © 2022 Elsevier Inc.Objective: The aim of this study is to demonstrate a new surgical technique to achieve ideal nasal tip rotation and projection with nasal supratip angle in patients with thick skin. Method: Supratip suture and skin scoring technique was used in 24 patients who underwent open technique septorhinoplasty operation between the years 2019 and 2022. In this technique the nose skin was thinned with scissors. Scoring and supratip sutures were applied to the skin. Kenacort (Triamcinolone 8 mg) was injected into the supratip area without closing the skin. Fourteen of the patients were female and 10 were male. Result: The mean age of the patients was 22. The mean follow-up time of the patients was between 6 and 18 months. No complications were observed due to this technique. Satisfactory results were obtained in the postoperative period after surgery. Conclusion: A new surgical approach has been proposed to create the desired dimensions with nasal tip rotation and projection and supratip angle in patients with thick skin using the supratip suture and skin scoring technique.

    Background: Mometasone furoate, one of the second generation intranasal corticosteroids, is currently used in suspension form due to its poor solubility. However, this is not favorable for nasal application because of the rapid elimination of the instilled drug from the nasal cavity by mucociliary clearance and delayed onset of action due to the slow dissolution of drug in suspension.

    Objective: Thyroid surgery is one of the frequent surgeries in daily operation schedules more often since the thyroid gland is very vascular Hematomas after thyroidectomy may cause airway obstruction, which is life-threatening and requires immediate surgical intervention. Therefore, many hemostatic devices and methods have been developed. Tranexamic acid is a synthetic lysine analog with low molecular weight. This study was planned to demonstrate the effect of intraoperative topical tranexamic acid on postoperative drainage output for thyroid surgery.

    Objective: Systemic absorption of steroids is reasonable because of the lipophilic nature of steroids and high vascularity of the middle ear mucosa. To date, however, of the clinical trials designed to investigate this subject, to the best of our knowledge, none has been based on objective methods. This study aims to demonstrate whether the intratympanic administration of dexamethasone has any systemic effects.

    The Effect of Types of Nasal Septum Deviation on the Eustachian Tube Function

    Objective: Nasal septum deviations are divided into six different subtypes in the literature. Because obstruction and nasal airflows in the nasal passages that these types form are different, their effects on the eustachian tube can be different. The effect of different nasal septum deviations on eustachian tube function was investigated in our study. Methods: A total of 80 patients with six different septum types and 15 healthy volunteers were included in the study. We tested eustachian tube function with P1, P2, and P3. P1 is the tympanometric measurement while resting. P2 is the tympanometric measurement after the Toynbee maneuver. P3 is the tympanometric testing after the Valsalva maneuver. To evaluate the functionality of the eustachian tube, we used the P1-P2 >10 daPa or P-max-P-min >15 daPa criteria. Measurements were performed before and 6 months after surgery. Results: Before surgery, there was no difference between types 1, 2, 3, and 5 and healthy volunteers in terms of eustachian tube dysfunction. Before surgery, type 4 and 6 had significant eustachian tube dysfunction compared with healthy volunteers. Type 4 and 6 showed significant improvement in the eustachian tube functions at 6th month after operation. Conclusion: According to the results of our study, applying septoplasty only to patients with type 4 and 6 nasal septum deviations before middle ear surgery would reduce unnecessary cost and increase the success of middle ear surgeries by preventing morbidities.
