Neopterin and soluble CD14 levels as indicators of immune activation in cases with low anti-HCV reactivity and true HCV infection

Neopterin and soluble CD14 (sCD14) are detected at high levels in hepatitis C virus (HCV) infections. We aimed to evaluate the role of these plasma immune activation biomarkers, for the indirect assessment of immune activation status of patients with low anti-HCV reactivity and a HCV infection. Low anti-HCV reactivity group (LRG, n: 70), true positive HCV infection group (THG, 30) and healthy control group (HCG, 30) were analyzed in this study. We have used ELISA, HCV RIBA/LIA and HCV-RNA methods. Mean neopterin levels were significantly lower in LRG than THG (p 0.05). Mean sCD14 were significantly higher in LRG than THG and HCG (p <0.05, p <0.001). Values of 3.95 mu g/ml and 5.36 nmol/l for sCD14 and neopterin resulted in the maximum area under the receiver operating characteristic curves (ROC), which were 0.859 (95% CI, 0.745 to 0.935; <0.0001) and 0.788 (95% CI, 0.663 to 0.883; <0.0001), respectively. These cut-offs corresponded to a sensitivity of 73.3% and a specificity of 73.3% for neopterin and of 100% and 76.7% for sCD14. Our results suggest that a specific immunoactivation might be caused by true positive HCV infection. Due to the significant results sCD14 in LRG might be non-specifically affected by some underlying atypical immunohematological pathologies. Only neopterin might be used to exclude low anti-HCV reactivity from a true HCV infection. The use of neopterin but not sCD14 in combination with fourth-generation EIA/CMIA combo tests will be useful when nucleic acid tests are not available for screening blood donors at blood banks.

Serum Bone Markers Levels and Bone Mineral Density in Familial Mediterranean Fever

[Purpose] The aim of this study was to measure bone mineral density, serum and urinary bone turnover parameters, and to evaluate the influence of demographic and genetic factors on these parameters in FMF patients. [Subjects and Methods] Twenty-seven attack-free patients who were diagnosed with FMF (in accordance with Tel Hashomer criteria) were recruited at outpatient rheumatology clinics. We investigated whether there were any differences between the FMF patients and a control group in terms of lumbar and femur bone mineral density (BMD), standard deviation scores (Z scores and T scores) and bone markers. [Results] In terms of the median values of lumbar BMD (p = 0.21), lumbar T (p = 0.098) and Z (p = 0.109) scores, femoral neck BMD, femoral T and Z scores and total femur BMD, T (p = 0.788) and Z scores, there were no significant differences. [Conclusion] In our study, no statistically significant differences were found between FMF patients and a control group in terms of osteoporosis. The 25-OH vitamin D was found to be significantly lower in FMF patients than in the control group.

Electrocardiographic P-wave characteristics in patients with psoriasis vulgaris

Purpose. Psoriasis vulgaris is one of the most common skin disorders. Patients with psoriasis carry an excessive risk of atrial fibrillation (AF). The differences between the maximum (Pmax) and the minimum (Pmin) P-wave duration on ECG are defined as P-wave dispersion (PWD). Prolongation of PWD is an independent risk factor for the development of AF. The aim of this the study was to investigate P-wave duration and PWD in patients with psoriasis. Methods. Sixty-one adult patients with psoriasis vulgaris (group 1) and 58 age and sex-matched healthy individuals (group 2) were included in this study. ECG recordings were obtained, and the P-wave variables were calculated. Results were reported as mean ± standard deviation and percentages. Continuous variables were analysed using Student’s t test. A value of P < 0.05 was considered statistically significant. Results. Pmax and PWD were significantly higher in group 1 than in group 2 (108.8 ± 21.3 ms versus 93.3 ± 13.0 ms, P < 0.001; 67.4 ± 22.9 ms versus 45.0 ± 19.6 ms, P < 0.001, respectively). Also, Pmin was significantly lower in group 1 (41.3 ± 12.3 ms versus 48.3 ± 14.3 ms, P = 0.04). The psoriasis disease activity score and hsCRP correlated with PWD (P < 0.01). Conclusions. Atrial conduction of sinus impulses was impaired in patients with psoriasis vulgaris. It was more prominent in patients with severe disease. Physicians caring for patients with psoriasis vulgaris should screen them for AF development.

Can PET-CT replace bone marrow biopsy for lymphoma staging? Retrospective analysis of 198 Hodgkin and non-Hodgkin lymphoma cases

Aim: Lymphoma staging is important from the prognostic and therapeutic point of view and bone marrow biopsy performed for this purpose makes up a large part of current hematopathology practice. PET-CT is the currently preferred method to evaluate bone marrow involvement by lymphoproliferative disorders as it is non-invasive and practical. The aim of this study was to compare trephine biopsy and PET-CT results as regards determining bone marrow involvement in various lymphoma subtypes. Materials and Methods: A total number of 198 bone marrow biopsies and 185 PET-CT images of cases consisting of various lymphoma subtypes were included in the study. The results of both methods evaluated for bone marrow infiltration were grouped as positive, negative, and suspicious to compare consistency. Statistical agreement was calculated with the kappa coefficient. The sensitivity, specificity, and the positive and negative predictive values were calculated as the diagnostic test measures. Results: Twenty-six cases (15.8%) had lymphoma involvement in trephine biopsies and 36 cases (21.2%) had positive findings for lymphoma involvement of the bone marrow on PET-CT. The two methods had weak statistical agreement (κ= 0.21). Biopsy and PETCT results were similar in 132 cases (71.20%). PET-CT showed false negative results in 11 cases in which infiltration was observed with biopsy. Twenty-six cases that were negative for lymphoma involvement on biopsy were accepted as positive on PET-CT and 16 of these cases were classical Hodgkin-s lymphoma. One case with suspicious bone marrow biopsy was positive on PET-CT, while four cases with suspicious PET-CT results were positive on biopsy. Conclusion: The results showed that both of the methods have advantages and disadvantages as regards lymphoma staging. However, histopathology is globally accepted as the gold standard for a definite diagnosis. We believe that the complementary use of the two methods is more beneficial for correct guidance during clinical practice.
