    Hip Fracture Mortality: Is It Affected by Anesthesia Techniques?

    We hypothesized that combined peripheral nerve block (CPNB) technique might reduce mortality in hip fracture patients with the advantage of preserved cardiovascular stability. We retrospectively analyzed 257 hip fracture patients for mortality rates and affecting factors according to general anesthesia (GA), neuraxial block (NB), and CPNB techniques. Patients- gender, age at admission, trauma date, ASA status, delay in surgery, followup period, and Barthel Activities of Daily Living Index were determined. There were no differences between three anesthesia groups regarding to sex, followup, delay in surgery, and Barthel score. NB patients was significantly younger and CPNB patients- ASA status were significantly worse than other groups. Mortality was lower for regional group (NB + CPNB) than GA group. Mortality was increased with age, delay in surgery, and ASA and decreased with CPNB choice; however, it was not correlated with NB choice. Since the patients- age and ASA status cannot be changed, they must be operated immediately. We recommend CPNB technique in high-risk patients to operate them earlier.

    Circadian changes in cortical excitability in restless legs syndrome

    Various investigations have revealed a widespread and somewhat controversial pattern of cerebral, cerebellar and brainstem involvement in the pathophysiology of restless legs syndrome (RLS). However, several studies which investigated functional or structural aspects indicated cortical involvement in RLS. In this study, we aimed to analyze circadian changes of cortical excitability in idiopathic RLS patients by means of transcranial magnetic stimulation (TMS). Eleven idiopathic RLS patients and eight healthy age and sex matched subjects were investigated using single-pulse TMS and motor nerve conduction studies during early afternoon when there were no symptoms and late at night (22:00-23:00) when the symptoms reappeared. Central motor conduction time, latencies and amplitudes of scalp and cervical motor evoked potentials, resting and active motor thresholds, and cortical silent period were measured. Measured parameters were similar between RLS patients and healthy subjects during the daytime. At night, cortical silent periods tended to shorten, and motor thresholds tended to decrease in the RLS group, whereas in controls they tended to increase.

    Factors Related to Sense of Coherence Among Dental Students at Istanbul University

    The salutogenic model suggests that generalized resistance resources (GRRs) promote the development and maintenance of a strong sense of coherence (SOC), which is a health-promoting factor. This study examined associations between GRRs (sociodemographic characteristics, social support, stress, health practices, and self-rated health) and a strong SOC among dental students at Istanbul University. Data were collected from a convenience sample of 566 students using questionnaires including a sociodemographic section, the SOC scale, the Perceived Stress Scale, the Multidimensional Scale of Perceived Social Support, self-rated health, and health practices. Data were analyzed using descriptive statistics, t-test, chi-square test, and binary multiple logistic regression. Students were classified into a low or strong SOC group by a median split procedure, based on their total score on the SOC scale (median=56, range=22 to 91). Univariate statistics showed that students with a strong SOC reported lower stress levels, higher social support levels, better self-rated health, brushing their teeth more frequently, lower sugar intake between meals, and less frequent smoking than students with a low SOC. Multivariate statistics indicated that being a nonsmoker and having better self-rated health, higher social support levels, lower stress levels, and lower sugar intake between meals were the most important predictors of strong SOC. Taking into account the factors that reinforce a strong SOC may help dental educators develop health promotion programs and create supportive environments for maintaining dental students- health.

    Aims: Angiotensins were shown to have some role in the development of panic disorder (PD). In this study, we aimed to determine the frequency of polymorphisms in two angiotensin-related genes, angiotensin I-converting enzyme (ACE) and angiotensin II type I receptor (ATr1), in a sample of Turkish patients with PD and to evaluate their association with PD development.
