    p53 expression and relationship with MDM2 amplification in breast carcinomas

    Carcinoma of the breast, like other malignancies, is a genetic disease with multiple genetic events leading to the malignant phenotype. p53 mutations are the most common genetic events in human cancer. Inactivation of p53 can be a result of mutation in gene sequence. One of the main structures that regulate p53 stabilization is MDM2. It suppresses p53 transcriptional activation by recognizing transactivation domain of p53. The loss of MDM2 function on p53 regulation results in deprivation of p53 tumor suppressor ability. Single nucleotide polymorphisms (SNP309 T->G exchange) or MDM2 amplification has been proposed to play a role in this issue. In the present study, our aim is to analyze p53 and MDM2 status and investigate their interactions in human sporadic breast carcinoma. The study groups were separated according to their molecular classifications. In each group, histologic type of the tumor, conventional prognostic parameters, p53, and MDM2 interactions were compared statistically. Tumors are divided into 4 subtypes due to estrogen and progesterone receptor status, HER-2, and Ki-67 proliferation index results. According to this classification, 23 cases are in the luminal A, 32 cases are in the luminal B, 15 cases are in the HER-2 positive, and 22 cases are in the triple-negative group, with a total of 92 cases. p53 expression is low in luminal breast carcinomas than HER-2 and triple-negative subtypes. MDM2 amplification frequency was found to be 5.4% in total. MDM2 gene amplification does not have a significant role in breast carcinogenesis, but other possible mechanisms may play a role in its inactivation. (C) 2016 Elsevier Inc. All rights reserved.

    A Six Sigma Trial For Reduction of Error Rates in Pathology Laboratory

    Objective: The aim of this study is to determine differences of levels of empathy among undergraduates in each year of their four-year programs of physiotherapy. Methods: During the 2014-2015 academic school year, 381 physiotherapy students were enlisted from two universities in Istanbul, one a foundation and the other a government university. The Turkish version of the Jefferson Scale of Physician Empathy was administered. Students were asked to indicate interest in particular physiotherapy specialties as well as their region of origin in Turkey. The Kruskal-Wallis analysis was used to determine differences among the four study years, and also to measure relationships between specialty interest, home-region, and empathy scores of the students. Empathy scores were also compared according to gender. Results: The difference of empathy scores between the students of the two universities was borderline significant (p=0.057). Empathy scores in both universities increased to a significant degree after school entrance and decreased in the final year. Levels of empathy did not change according to gender, specialty interest, or home-region (p=0.722, 0.524, and 0.309, respectively). Conclusions: This study points to the need for physiotherapy curricula that would enhance empathy and give students practice in exhibiting this valuable attribute. Additional studies are needed that would include larger study populations and track the same students year by year as to how and why their empathy levels change during their training.

    Pneumatized septal turbinate

    Intumenscentia septi nasi anterior is an anatomical variation in the nasal cavity, first described by Morgagni in 1662; septal turbinate and tuberculum septi nasi anterior are among the various other names attributed to this anatomical structure. It is an expandable vascular network which develops from the septal mucosa. Computed tomography, following history-taking and physical examination, is a valuable tool in diagnosing turbinate pneumatisation. This paper reports the case of a 23-year-old female with nasal stuffiness as her primary complaint, who was found to have pneumatization of the septum mimicking the septal concha. Pneumatization of the septal turbinate may also be a cause of septal enlargement. The symptomatology, diagnosis and treatment options for cases of septal turbinate are discussed. The surgical interventions performed in the presented case are briefly described. The presented patient had a pneumatization of the septal turbinate, like those of the conchae. It is reported for the first time in medical literature, and the histological and functional similarities of the septal turbinate and of the nasal conchae are pointed out.     Incidental detection of prostate-specific antigen-negative metastatic prostate cancer initially presented with solitary pulmonary nodule on fluorodeoxyglucose positron emission tomography/computed tomography

    A 71-year-old male patient with solitary pulmonary nodule underwent fluorodeoxyglucose positron emission tomography/computed tomography (FDG PET/CT) showing slightly increased FDG uptake in this nodule. In addition, PET/CT detected hypermetabolic sclerotic bone lesions in the right second rib and 7(th) thoracic vertebrae, which were interpreted as possible metastases, and mildly increased FDG uptake in the prostate gland highly suspicious of malignancy. The patient's prostate-specific antigen (PSA) level was within normal range (3.8 ng/dL). The histopathological examination of the lung nodule and right second rib lesion proved metastases from prostate cancer, then the prostate biopsy-confirmed prostate adenocarcinoma. The unique feature of this case is to emphasize the importance of performing PET/CT for solitary pulmonary nodule in detecting PSA-negative metastatic prostate cancer. This case indicated that it should be kept in mind that, even if the PSA is negative, a lung metastasis of prostate cancer may be an underlying cause in patients evaluated for solitary pulmonary nodule by FDG PET/CT.

    Schwannomas are the rare benign tumors of the neck. Patients developing from the brachial plexus and cervical truncus and extending to the posterior mediastinum have been reported, but for the first time the following case report describes the extension of a vagal origin schwannoma from the anterior and middle mediastinum and its extraction with thoracotomy without sternotomy.

    This animal study compares different methods of performing an osteotomy, including using an Erbium-doped Yttrium Aluminum Garnet laser, histologically, radiologically and biomechanically. A total of 24 New Zealand rabbits were divided into four groups (Group I: multihole-drilling; Group II: Gigli saw; Group III: electrical saw blade and Group IV: laser). A proximal transverse diaphyseal osteotomy was performed on the right tibias of the rabbits after the application of a circular external fixator. The rabbits were killed six weeks after the procedure, the operated tibias were resected and radiographs taken.

    Xenotransplantation of human cryopreserved parathyroid tissue isolated from parathyroid adenomas to normocalcemic rabbits

    Objective: Parathyroid allotransplantation is a new method for the treatment of permanent hypoparathyrodism. Adenoma cells are not used for transplantation because of the potential for functional or histopathologic transformation. In this study, we transplanted human adenomatous parathyroid cells to rabbits. Material and Methods: Parathyroid adenoma tissue taken from a male patient was cryopreserved and transplanted into seven New Zealand white rabbits (mean weight, 3700±220 g; mean age, 4.5 months) under immunosuppression. The levels of parathormone, calcium and phosphorus were measured before and after transplantation, and the parathyroid cells were observed histopathologically. Results: Mean parathyroid hormone level was 0.5 pg/dL before transplantation and 6.6 pg/dL after transplantation (p<0.05). Preoperative mean calciumlevel was 14.1 mg/dL, and mean phosporus level was 3.5 mg/dL before transplantation while these values were 14.4 mg/dL and 3.3mg/dL, respectively, after transplantation (p>0.05). Morphologic transformation was not observed in parathyroid cells after transplantation. Conclusion: In short-term observation, adenomatous parathyroid cells can function without malignant transformation. In the future, the preliminary methodology in this study may serve as a safe alternative for allotransplantation into patients with permanent hypoparathyroidism.     Somatostatin Analogs Regress Endometriotic Implants in Rats by Decreasing Implant Levels of Vascular Endothelial Growth Factor and Matrix Metaloproteinase 9

    Objective: To examine the effect of somatostatin analogs on surgically induced endometriosis in rat models. Study design: Endometrial tissue was implanted onto the abdominal peritoneum of 26 rats that were randomized into 3 groups. The rats in group 1(n = 9) were subcutaneously administered with 0.02 mg/kg/d of octreotide (a short-acting analog) for 28 days. The rats in group 2 (n = 8) were subcutaneously injected with 20 mg/kg of a single dose of a long-acting analogue lanreotide The rats in group 3 were given no medication and served as controls (n = 9). Results: Mean volume and histologic score of implants in groups 1 (P < .01 and P < .05, respectively) and 2 (P < .01 and P < .05, respectively) were significantly lower than that in group 3. There were significant reductions in vascular endothelial growth factor (VEGF) and matrix metalloproteinase 9 (MMP-9) immunoreactivities in group 1 (0.67 +/- 0.50 and 1.22 +/- 0.44, respectively; both P < .01) and group 2 (0.71 +/- 0.48 and 0.86 +/- 0.69, respectively; both P < .01) when compared with the control group (1.78 +/- 0.83 and 2.11 +/- 0.78, respectively). Conclusion: Somatostatin analogs has regressed significantly the size of the endometriotic implants and caused atrophy of these lesions in rats by decreasing explant levels of VEGF and MMP-9.

    Sternal metastasis from uterine leiomyosarcoma treated by near-total sternectomy and reconstruction with titanium sternal stabilization and fixation system

    Uterine leiomyosarcomas are rare soft tissue neoplasms. Although they have a tendency to metastasize to distant organs, most commonly to the lung, liver and brain, rarely, sternal metastasis from a uterine leiomyosarcoma can occur. In this article, we report a 50-year-old female patient whose uterine leiomyosarcoma was treated with total abdominal hysterectomy and bilateral salphingo-oopherectomy, and who developed sternal metastasis five years later, which was completely resected and successfully repaired during the same session.
