    Papillary Lesions of the Breast: Addition of DWI and TIRM Sequences to Routine Breast MRI Could Help in Differentiation Benign from Malignant

    Aim: We aimed to investigate the magnetic resonance imaging (MRI) features of benign, atypical, or malignant papillary breast lesions and to assess the additional value of diffusion-weighted imaging (DWI) and turbo inversion recovery magnitude (TIRM) sequences to routine breast MRI. Background: Differentiation between benign and malignant papillary breast lesions is essential for patient management. However, morphologic features and enhancement patterns of malignant papillary lesions may overlap with those of benign papilloma. Methods: Seventy-two papillary breast lesions (50 benign, 22 atypical or malignant) were included in the current study, retrospectively. We divided the patients into two groups: benign papillary breast lesions and atypical or malignant papillary breast lesions. Morphologic, dynamic, turbo inversion recovery magnitude (TIRM) values and diffusion features of the papillary lesions were compared between two groups. Results: Benign papillary lesions were smaller in size (p=0.006 and p=0.005, for radiologists 1 and 2 respectively), closer to the areola (p=0.045 and 0.049 for radiologist 1 and 2 respectively), and had higher ADC values (p=0.001 for two radiologists) than the atypical or malignant group. ROC curves showed diagnostic accuracy for ADC (AUC=0.770 and 0.762, p & lt; 0.0001 for two radiologists), and showed a cut-off value of <= 957 x 10-6 mm(2/s) (radiologist 1) and <= 910 x 10-6 mm(2/s) (radiologist 2). Conclusion: MRI is a useful method for differentiation between benign and malignant papillary breast lesions. Centrally located, lesser in size, and higher ADC values should be considered benign, whereas peripherally located, larger in size, and lower ADC values should be considered malignant.

    Comparison of MRI Features of Invasive Pleomorphic and Classical Lobular Carcinoma: Differentiation Is Possible?

    To evaluate breast MRI and DWI and demographic features of pleomorphic invasive lobular carcinoma (pILC) and classic invasive lobular carcinoma (cILC). Invasive lobular (ILC) is the second most common breast malignancy after invasive ductal carcinoma (IDC) and constitutes the 8-14% of all invasive breast cancers. ILC morphologically can be classified into the classic, alveolar, solid, tubulolobular, and pleomorphic subtypes according to WHO. This study was performed retrospectively. The MRI and demographic features of 18 patients with 23 pILC were compared with those 22 consecutive patients with 27 cILC. There was no significant difference in demographic features of patients, MR appearance, kinetics, and ADC values between two groups. pILC, an aggressive subtype of ILC, cannot be differentiated from cILC with breast MRI.

    Puberte Prekoks ve Prematür Telarş ile Serum İrisin Düzeyi İlişkisi

    Amaç: İrisin, -Fibronectin type III domain containing 5- (FNDC5) adı verilen bir transmembran proteinin yıkımı ile ortaya çıkan yenikeşfedilmiş bir adipomyokindir. Daha önce prepubertal dönemde ve puberte evrelerinde irisin düzeylerinde değişiklik olduğu saptanmıştır.Puberte başlangıcını tetikleyen mekanizmalar halen tam olarak açıklanamamakla beraber periferik yağ dokusu ve oradan salgılananadipokinlerin santral sinir ağını uyararak puberte başlangıcını tetikleyebileceği hipotezi vardır. Çalışmamızın amacı puberte prekokslu,prematür telarşlı ve prepubertal çocuklarda irisin düzeylerini karşılaştırmak, bu adipomyokinin bir belirteç olarak kullanılıpkullanılamayacağını incelemekti.Gereç-Yöntem: Çalışmaya 33’ü puberte prekoks, 31’i prematür telarş, 30’u kontrol grubunda olmak üzere toplam 94 kız çocuğu dâhil edildi.Yaş ve vücut kitle indeksleri (VKİ) standart deviasyon skoru (SDS) benzer gruplar oluşturuldu. Gruplar arasında irisin düzeyleri karşılaştırıldı.Ayrıca irisin ile yaş, VKİ-SDS, boy-SDS, ağırlık-SDS, kemik yaşı, uterus uzun aksı, sağ over boyutu, sol over boyutu, bazal FSH, bazal LH, pikFSH, pik LH ve östradiol (E2) arasındaki korelasyonlar incelendi. Çoklu regresyon analizi ile irisin düzeyini etkileyen en önemli faktörlerbelirlendi.Bulgular: Puberte prekoks, prematür telarş ve kontrol grubunu karşılaştıran tablo aşağıdadır (Tablo 1). İrinsin ile VKİ-SDS, boy-SDS, ağırlık-SDS, kemik yaşı, uterus uzun aksı, sağ over boyutu, sol over boyutu, bazal FSH, bazal LH, pik FSH ve pik LH arasında pozitif korelasyonsaptanmıştır. Çoklu regresyon analizinde irisin ile ilişkili en önemli faktörün pik LH düzeyi olduğu sonrasında sırası ile yaş ve VKİ-SDS olduğugörülmüştür. Yapılan ROC analizinde eşik değer 631 ng/mL alındığında puberte prekoks tanısı için sensivite ve spesifitesinin çok yüksekolduğu gösterildi.Sonuçlar: Puberte prekoks grubunda irisin düzeyleri prematür telarş ve kontrol grubundan anlamlı derece yüksek bulunmuştur. İrisinyüksekliğinin puberte başlangıcı üzerine etkili olabileceği hipotezine destek veren bir bulgu olmakla beraber bu çalışma ile bu hipotezikesinleştirmek mümkün değildir. Puberte sırasında artmış kas ve yağ dokusunun irisin düzeylerinin artışında rolü olabileceği dedüşünülebilir. Tüm bunlara karşın irisin düzeylerindeki bu artışın puberte prekoks tanısında faydalı olabileceği ve hasta sayısı daha yüksekçalışmalar yapıldıktan sonra bir marker olarak kullanılabileceği kanaati oluşmuştur.

    Development of Myocardial Infarction in a 12-Year-Old Female after the Use of Inhaled Salbutamol

    Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) accounts for 0.023% of all cases reported in pediatric patients. According to literature, only a handful of ALCAPA patients are able to reach adulthood. Clinical manifestations of ALCAPA range from fatigue during exercise to sudden death in adulthood. Herein, we described a 12-year-old symptomatic patient with ALCAPA who had severe chest pain after using salbutamol treatment for presumed asthma. ALCAPA is one of the curable versions of myocardial ischemia and infarction in childhood. Due to clinical findings in conjunction with electrocardiogram and echocardiography, a computed tomography scan with coronary angiography was performed and the diagnosis of ALCAPA was confirmed. We presented this case because ALCAPA-related myocardial ischemia and infarction in children are rare with only sporadic cases reported. This case illustrated the need for close monitoring and surgery as the best treatment for ALCAPA associated with myocardial infarction.

    A Rare Cause of Pulmonary Hypertension in a 4-Year-Old Toddler: Association of Cor Triatriatum Sinister and Pulmonary Arteriovenous Malformation

    Cor triatriatum sinister is a rare congenital cardiac anomaly. The anomaly is caused by a fibromuscular membrane that divides the left atrium into two cavities. This membrane can lead to the obstruction of left atrial flow and also create pulmonary venous hypertension. Pulmonary arteriovenous malformation (PAVM) is notorious for its aberrant blood flow between the pulmonary arteries and veins. Herein, we report a case of a 4-year-old toddler who had a unique form of pulmonary hypertension presenting with cor triatriatum sinister and diffuse PAVM. After the surgical treatment of cor triatriatum sinister, both pulmonary arteriovenous malformation and pulmonary hypertension disappeared.

    Balanced Double Aortic Arch Causing Persistent Respiratory Symptoms Mimicking Asthma in an Infant

    Double aortic arch (DAA) is a common form of complete vascular ring. The state of the condition leads to upper airway impediment and compression of the esophagus. A balanced DAA is an extremely rare anomaly. The anatomical aberration cannot be easily diagnosed with echocardiography when an infant has chronic respiratory distress. Herein, we present the case of an 11-month-old girl who had chronic respiratory distress and a balanced DAA.
