    Indirect Effect Of A Dentistry And Medical School Hospital Before And After Covid-19 On The Environment

    Dünya Sağlık Örgütü (WHO)’ne göre küresel ısınma tüm dünyada insanların karşılaştığı ciddi sorunlardan biridir.(WHO 2011) Gelecek nesiller için dünyanın uzun bir süre daha var olması önem taşımaktadır. Dolayısıyla bir diş hekimi olarak çevre dostu bir yöntem kullanarak doğanın ko runmasına yönelik katkı sağlamak bizim sorumluluğumuzdur. Küresel ısınmanın en önemli sebeplerinden biri de çevresel kirliliktir. Yapılan çeşitli araştırmalara göre, diş hekimliğinin çevreye etkisi değerlendirilmiştir. Birleşik Krallık Ulusal Sağlık Sistemi (NHS)’ne göre karbon ayakizine en büyük katkıyı dental servislere yapılan giriş çıkışlar ve tedarik süreci oluşturmaktadır. Diş hekimliği klinikleri, diğer sağlık kuruluşlarına kıyasla daha az atık üretiyor olsa da, son on yılda dental atıklarda önemli bir artış gözlenmektedir. Çeşitli ülkelerde yapılan akademik araştırmalarda, Covid-19 sürecinden bağımsız olarak değerlendirilen kliniklerden esinlenerek Bezmialem Vakıf Üniversitesi Diş Hekimliği ve Tıp Fakültesi Hastanelerinde bir çalışma yapılmıştır. Bu çalışma, Covid-19 öncesi ve sonrası bir diş hekimliği ve tıp fakültesi hastanesinin kullandığı ve harcadığı materyallerin (2019-2020 yıllarının Haziran-Aralık ayları arasındaki dezen-fektan, sabun, kağıt, hasta sayısı, doğalgaz, elektrik ve geri dönüşüm atıkları) derlenmesiyle sağlık sektörünün karbon ayakizinin bu süreçte hangi yönlerde değişime uğradığını belirtmektedir. Matery-allerin belirlenen süreçlerdeki hem yıllık değişimi hem de hasta başına düşen birim değişimi dikkate alınmıştır. Sağlık sektöründeki bireyler olarak bu süreci nasıl değerlendirmiş olduğumuzu gösteren bu çalışma, gelecekte benzer pandemilerin yaşanması durumunda öngörülecek durumları ve dikkat edilmesi gereken noktaları göz önüne sermiştir

    Temporary materials: comparison of in vivo and in vitro performance

    Objective The aim of this investigation was to compare clinical performance and in vitro wear of temporary CAD/CAM and cartridge crowns. This study is an approach to estimate the influence of in vivo use and laboratory simulation on temporary crowns. Materials and methods A total of 90 crowns were fabricated from each temporary CAD/CAM or cartridge material. Also, 10 crowns of each material were clinically applied for 14 days, and 80 identical duplicate restorations were investigated in the laboratory after storage in water (14 days; 37 degrees C) and subsequent thermal cycling and mechanical loading (TCML, 240.000 x 50N ML, 600 x 5 degrees C/55 degrees C). After in vivo application or in vitro aging, facture force, superficial wear (mean and maximum), surface roughness (Ra, Rz), thermal weight loss (TGA), and heat of reaction (DSC) were determined for all crowns. Statistics: Bonferroni post hoc test; one-way analysis of variance (ANOVA);alpha = 0.05). Results The fracture resistance of the temporary materials varied between 1196.4 (CAD in vivo) and 1598.3 N (cartridge crown in vitro). Mean (maximum) wear data between 204.7 (386.7 mu m; cartridge in vitro) and 353.0 mu m (621.8 mu m; CAD in vitro) were found. Ra values ranged between 4.4 and 4.9 mu m and Rz values between 36.0 and 40.8 mu m. DSC and TG analysis revealed small differences between the materials but a strong influence of the aging process. Conclusions Comparison of in vivo and in vitro aging led to no significant differences in fracture force and wear but differences in roughness, DSC, and TGA. SEM evaluation confirmed comparability. Comparison of CAD/CAM and cartridge temporary materials partially showed significant differences.

    Urogenital myiasis caused by Psychoda albipennis in a child

    Urogenital myiasis results when flies lay their eggs near the exit of the urethra and the larvae proceed upward along the urogenital tract. In this case report, a 10 year-old female patient diagnosed with urogenital myiasis was reported. The patient presented with complaints including painful and frequent urination, genital pruritus and moving larvae in urine. The patient had received Enterobius vermicularis treatment previously for two times. A 24-hour urine sample was collected and two black larvae were found in the urine. It was found that these larvae were fourth-stage larvae of Psychoda albipennis. Although there was no risk factor, the patient was affected with this rare parasitological disease. This case was presented to draw attention to myiasis in children. Myiasis may be observed in individuals with a favourable hygiene status and a high socioeconomical level. If a detailed history is not taken and appropriate laboratory tests are not performed, the diagnosis may be missed.

    Natural Compound Cudraflavone B Shows Promising Anti-inflammatory Properties in Vitro

    Cudraflavone B (1) is a prenylated flavonoid found in large amounts in the roots of Morus alba, a plant used as a herbal remedy for its reputed anti-inflammatory properties. The present study shows that this compound causes a significant inhibition of inflammatory mediators in selected in vitro models: Thus, 1 was identified as a:potent inhibitor of tumor necrosis factor a (TNF alpha) gene expression and secretion by blocking the translocation of nuclear factor kappa B (NF-kappa B) from the cytoplasm to the nucleus in macrophages derived from a THP-1 human monocyte cell line. The NF-kappa B activity reduction resulted in the inhibition of cyclooxygenase 2 (COX-2) gene expression. Compound Cads as a COX-2 and COX-1 inhibitor with higher selectivity toward COX-2 than indomethacin. Pretreament of cells by 1 shifted the peak in an regulatory gene zinc finger protein 36 (ZFP36) expression assay. This natural product has noticeable anti-inflammatory properties suggesting that 1 potentially could be Used for development as a nonsteroidal anti inflammatory drug lead

    Palliative Care in High and Low Resource Countries

    Palliative Care (PC) is defined by the World Health Organization (WHO) as a support provided by multiple disciplines in order to improve the quality of life of both patients and their caregivers, throughout the disease course, from diagnosis to end- of-life. PC aims to prevent and treat symptoms and side effects of the disease and its treatment. PC is well developed in most high-income countries; however in most low-income settings, where approximately 80% of patients with cancer requiring PC care for advanced disease live, PC services are still uncommon. Health indicators monitoring global PC development are policy, education, use of medicines, service provision and professional activity. Globally, PC development may be categorized as Group 1 (no known hospice-PC activity), Group 2 (capacity-building activity), Groups 3a Isolated PC provided, 3b Generalized PC provided, 4a hospice-PC services at a stage of integration into regular service provision, and 4b hospice-PC services at a stage of advanced integration into regular service provision. Spirituality is an essential element of patient-centered PC. The use of Complementary and Traditional Medicine (CTM) in Middle Eastern countries is widespread. There are wide discrepancies in cancer care and PC in many regions of the world. The Individualized Care Planning and Coordination (ICPC) Model is designed to facilitate the advance care planning with continuity of all the measures like symptom control or emotional, social and spiritual care of both the patient and the family during the disease steps like relapse or end of life.

    State and trait anxiety among medical staff during the first month of COVID-19 pandemic: A sample from Turkey

    Objectives During the COVID-19 pandemic, excessive workload, a rapidly changing workplace environment, the danger of carrying the virus and transmitting the disease to their families, relatives and those they live with creates stress for the medical workers. In our study, we aimed to evaluate the state and trait anxiety levels of healthcare professionals who encounter patients with suspected COVID-19 infection and related factors. Method Data were collected from healthcare professionals working with patients diagnosed or suspected with COVID-19 via online self-report questionnaire between 9-19 April 2020. The state (STAI-S) and trait anxiety (STAI-T) scale was used to measure anxiety. Results A total of 291 healthcare professionals, 216 women and 75 men, participated in the study. Women-s state and trait anxiety were significantly higher than men-s. 11 participants without any lifetime psychiatric illness experienced psychiatric symptoms and consulted to a psychiatrist. The state anxiety of those who have children, nurses and those working in branches directly related to the pandemic (Infectious Diseases, Respiratory Diseases, Emergency Medicine, Internal Medicine, Radiology, Anesthesiology and Reanimation) was higher than others. The state anxiety of those who thought they were not protected with personal protective equipment and those who did not stay in their own home was higher than others. Conclusions At the forefront of the fight against COVID-19, there are medical personnel who pay a serious psychological cost. Especially in terms of anxiety, we should pay attention to women, workers with children, nurses and people working in branches that are directly related to pandemics.

    Impact of training on practical skills of Iraqi health providers towards integrated management of neonate and child health - a multicentre cross sectional study

    Background:This study aims to assess the mandatory practical skills of caregivers towards the implementation of the Integrated Management of Neonatal and Childhood Illness (IMNCI) strategy in primary health care (PHC) centers of Salah al-Din governorate in Iraq.Methods:A cross-sectional comparative study was conducted from January to May 2014. An equal sample of 42 trained and 42 non-trained caregivers who are working in 20 PHC centers in Tikrit city and other districts of Salah al-Din governorate of Iraq were included. The study tool was a semi-structured questionnaire with 20 questions covering different required practical skills that caregivers should have. The total score was 100 and in a range of 4- 6 points for each question. An independent sample t-test was used to compare the means of numerical variables.Results:The mean age of total respondents were (33.18 ± 5.82Years), and the vast majority (63, 75.0%) were females. More than two-third (58, 69.0%) were paramedical compared to 26 (31.0%), who were doctors. Trained caregivers had statistically significant better practice (73.48 ± 13.46) compared to non- trained caregivers (63.95 ± 17.44). Trained doctors had statistically significant better practice (88.15 ± 2.70) compared to trained paramedical staff (66.90 ± 10.84). Trained caregivers from Tikrit city had statistically significantly better practice (80.26 ± 7.38) compared to trained caregivers from districts (67.89± 14.85). The highest proportion (97.5%) of trained caregivers felt the child for fever or body hotness appropriately, and the lowest proportion (59.5%) of them recorded age, height, and weight correctly.Conclusion:This study showed that training has a positive influence on the implementation of IMNCI interventions. IMNCI-trained caregivers were more likely to correctly classify illnesses than non-trained caregivers.

    Brachial plexus injury complicating the labor: mother is the victim at this time

    Objective: Postpartum nerve injuries are rare complications of labor and the lower extremity peripheral nerves are the frequently affected ones. In this case report, we are presenting a patient who developed brachial plexus injury after a forceful vaginal delivery.

    Bir Sosyal Değişim Katalizörü Olarak Sosyal Girişimcilik

    Bu çalışmanın amacı, sosyal girişimcilik kavramını incelemek, tartışmak ve uygulama örnekleri ile sosyal girişimcilik alanında farkındalık yaratmaktır. Bu amaçla, çalışmada ilk olarak literatür incelenmiş ve sosyal girişimcilik kavramının farklı anlamları tartışılmıştır. Ayrıca, sosyal girişimciliğin amacı, önemi, boyutları ve faaliyet alanları açıklanmıştır. Ardından, geleneksel girişimcilik ve sosyal girişimcilik arasında karşılaştırma yapılmıştır. Sosyal girişimciliğe yönelik yapılan eleştiriler, sosyal girişimcilik eğitimi ve toplumda oluşan sosyal etki incelenmiştir. Sonrasında ise, Türkiye’deki sosyal girişimcilik uygulamaları detaylı anlatım ve örneklerle açıklanmıştır. Elde edilen sonuçlar, Türkiye’deki sosyal girişimcilik faaliyetlerinin 2000 yılından sonra artış gösterdiğini fakat kavramın herkes tarafından bilinmediğini ve bir kavram karmaşası yaşandığını göstermektedir. Son olarak, günümüzde sosyal girişimcilik, işletmeler ve bireyler bazında desteklenmektedir. Yapılan projelerle birlikte sosyal girişimcilik alanı daha da gelişmekte ve başta gençler olmak üzere sosyal girişimciliğe yönelen girişimci sayısında artış görülmektedir.

    BERNARD SOULİER SENDROMLU ÇOCUKTA GENEL ANESTEZİ ALTINDA DİŞ TEDAVİSİ

    Giriş Bernard Soulier sendromu ilk kez 1948’de bildirilmiş, nadir görülen (yaklaşık 1/1milyon) kalıtsal bir pıhtılaşma bozukluğudur. Bildirilmiş birkaç otozomal dominant geçiş dışında otozomal resesif geçişli, dev trombositler, trombosit sayısında düşüklük ve uzamış kanama zamanı ile karakterizedir. Ortalama teşhis yaşı 16’dır. Etkilenmiş bireyler, kanamanın aşırı olmasına ve spontan ekimozlara eğilimlidirler. Bu sunumda biz, genel anestezi altında diş tedavisi yapılacak 14 yaşında bir olguyu paylaşmak istedik.OlguTanısı 6 aylıkken geçirdiği spontan intrakraniyal kanama sonrasında konulmuş 14 yaşında ve 60 kg ağırlığında erkek hasta dört adet diş çekimi ve dört adet diş tedavisi için ameliyathaneye alındı. Bernard Soulier sendromuna ek olarak faktör 7 eksikliği, intrakraniyal kanama sonrasında aralıklı devam eden epileptik nöbetleri, hafif mental retardasyonu, iletişim kurmada güçlüğü, mitral valv prolapsusu, atriyal septal defekti ve sol pulmoner arterde darlığı mevcuttu. Preoperatif dönemde pediyatrik nöroloji ve kardiyoloji ile konsülte edilen hasta için ek hastalıkları nedeniyle yoğun bakım yatağı rezerve edildi. Kan testleri normal sınırlarda olduğu için preoperatif kan ya da kan ürünü replasmanına ihtiyaç duyulmayan hastaya ameliyathaneye inmeden yarım saat önce endokardit profilaksisi için 1 gr ampisilin intravenöz olarak uygulandı. Yine öneriler doğrultusunda 1 ünite aferez trombosit hazırlandı. Ayrıca pediyatrik nöroloji önerileri doğrultusunda antiepileptik ilaçları düzenlendi. İletişimi sınırlı ve inaktif olan hastaya premedikasyon uygulanmadı. Hastanın anestezisi ve postoperatif derlenme dönemi sorunsuz seyretti. Cerrahi işlem sırasında kanama eğilimi, beklendiği gibi, normal hastalara göre fazlaydı. Diş çekimi en sona bırakıldı. Çekim yapılan alanlara emilebilir hemostatik jelatin sünger konuldu. Bu alanlardaki kanama nedeniyle hemostatik süngerler 3-4 kez değiştirildi. Bu süreç esnasında baskı uygulayarak hemostaza yardımcı olmak için fındık boyutunda gaz kompres uygulandı ve kanama aralıklı olarak kontrol edildi. Derlenme ünitesinde 1 saat gözlendikten sonra sorun olmaması üzerine servise alındı. Ameliyatın ertesi günü pediyatrik nöroloji ve kardiyoloji tarafından tekrar değerlendirilen hasta taburcu edildi.Tartışma ve sonuçBernard Soulier sendromlu hastalarda klinik seyir farklı olabildiğinden, bu hastalar preoperatif dönemde ayrıntılı bir şekilde incelenmeli ve gerekli bölümlerle konsülte edilmelidir. Kanama eğilimi nedeniyle peroperatif yakın hemodinamik takip önemlidir. Ayrıca komorbiditelerin olması durumunda postoperatif dönemde yoğun bakım takibine ihtiyaç duyulabileceği akılda tutulmalıdır.
