    Ultra-High Performance Liquid Chromatography Mass Spectrometry-based Metabolic Profiling in Urine for Bipolar Disorder with Manic Episode

    ÖzetGiriş ve Amaç: Bipolar bozukluk, yineleyen mani ve depresyon ataklarıyla seyreden karmaşık bir psikiyatrik hastalıktır. Yaygın olmasına rağmen, altta yatan patofizyolojisi hâlâ tam olarak anlaşılamamıştır ve tanı için nesnel biyobelirteçler bulunmamaktadır. Metabolomiks, belirli duygudurum dönemleriyle ilişkili biyolojik değişiklikleri araştırmak için umut verici bir yaklaşımdır. Bu çalışma, manik fazdaki bipolar bozukluğu olan hastalar ile sağlıklı kontrol grubu arasındaki idrar metabolomundaki farklılıkları araştırmayı amaçlamıştır.Yöntem: İdrar örnekleri, 22 manik bipolar bozukluk hastasından ve 31 sağlıklı kontrolden toplanmıştır. Örnekler Ultra Yüksek Performanslı Sıvı Kromatografi Kütle Spektrometresi kullanılarak analiz edilmiştir. Veriler MZmine ve TidyMass kullanılarak işlenmiştir. Ardından MetaboAnalyst 5.0’da istatistiksel ve çok değişkenli analizlere tabi tutulmuştur. Önemli Kat Değişimi gösteren metabolitler daha sonra Alıcı İşletim Karakteristiği analizi kullanılarak değerlendirilmiştir.Bulgular: İki fosfatidilkolin türevi olan 1.2-dioleoyl-sn-glycero-3-fosfatidilkolin ve 1-stearoyl-2-linoleoyl-sn-glycero-3-fosfokolin düzeyleri manik grupta önemli ölçüde daha yüksek bulunmuştur (p<0,05). Alıcı İşletim Karakteristiği analizi, bu metabolitlerin ayrı ayrı değerlendirildiğinde sınırlı ayırt edici performansa sahip olduğunu ortaya koymuştur.Sonuç: Bulgular, bipolar bozukluğun manik evresinin idrar lipidiyle ilişkili metabolitlerdeki değişikliklerle ilişkili olduğunu göstermektedir. Tanımlanan metabolitler tek tek bakıldığında tanı değeri açısından sınırlı olsa da bipolar bozuklukla ilişkili evreye özgü metabolik değişiklikleri yansıtma potansiyeline sahiptir. Despite its prevalence, the underlying pathophysiology remains poorly understood, and there is still a lack of objective biomarkers for diagnosis. Metabolomics is a promising approach for exploring biological alterations associated with specific mood states. This study aimed to investigate differences in the urinary metabolome between patients with bipolar disorder in the manic phase and healthy controls. Methods: Urine samples were collected from 22 manic bipolar disorder patients and 31 healthy controls. Samples were analyzed using UltraHigh Performance Liquid Chromatography Mass Spectrometry. The data were processed using MZmine and TidyMass, and then subjected to statistical and multivariate analyses in MetaboAnalyst 5.0. Metabolites showing significant fold changes were then evaluated using a receiver operating characteristic analysis.Results: The levels of two phosphatidylcholine derivatives, 1.2-dioleoylsn-glycero-3-phosphatidylcholine and 1-stearoyl-2-linoleoyl-snglycero-3-phosphocholine, were significantly higher in the manic group (p<0.05). Receiver Operating Characteristic analysis revealed that these metabolites had limited discriminatory performance when evaluated individually. Conclusion: The findings suggest that the manic phase of bipolar disorder is associated with alterations in urinary lipid-related metabolites. While the identified metabolites exhibited modest diagnostic value individually, they could potentially reflect phase-specific metabolic changes relevant to bipolar disorder. These exploratory results warrant further investigation in larger longitudinal studies that include different mood states.     Evaluation of Edible Mushroom Cantharellus lutescens as Potential Cholinesterase Inhibitors

    ABSTRACTObjectiveAlzheimer’s disease is one of the most common forms of dementia which affects the cognitive, physical, psychological, and social functions of individuals. Cholinesterase enzymes are known to play a pivotal role in the occurrence and progression of Alzheimer’s disease. This study aims to evaluate the anticholinesterase and butyrylcholinesterase inhibitory activities of the mushroom Cantharellus lutescens, which can be found in İstanbul and its surrounding regions.MethodsExtracts of Cantharellus lutescens mushroom were prepared by maceration with hexane, ethyl acetate, and methanol at room temperature. Acetylcholinesterase and butyrylcholinesterase inhibitory activities of these three extracts were determined using the Ellman method.ResultsWhile Cantharellus lutescens hexane extract was determined as the most active extract with 48.07±0.51% acetylcholinesterase inhibition at a concentration of 800 μg/mL, ethyl acetate extract showed the highest activity against butyrylcholinesterase with 29.20±0.80% inhibition at the same concentration.ConclusionThis study evaluates the anticholinesterase activity of an edible mushroom that could be a potential therapeutic agent in the future.Keywords:Cantharellus lutescens, yellow foot, anticholinesterase, Alzheimer’s disease

    Therapeutic Delay and Biologic Response in Hidradenitis Suppurativa: A Real-World Cohort Study

    Background: Hidradenitis suppurativa (HS) is a chronic inflammatory disease with a progressive course that may lead to scarring and impaired quality of life. Early initiation of biologic therapy has been proposed to improve outcomes; however, real-world evidence remains limited. Objectives: To assess treatment outcomes of patients with HS in a tertiary care center, with a particular focus on the effect of treatment delay and the potential association between early biologic therapy and clinical response, evaluating the concept of a therapeutic \"window of opportunity\" in an observational real-world study. Methods: This retrospective cohort included 152 patients with HS treated between December 2022 and March 2025. Disease severity was assessed using the International Hidradenitis Suppurativa Severity Score System (IHS4) and Hurley stage. Patients were stratified by treatment type (biologic vs. nonbiologic). Demographic characteristics and comorbidities were systematically recorded. Statistical analyses included Mann–Whitney U tests, Wilcoxon signed-rank tests, Spearman correlation, and Cox regression to evaluate treatment delay as a predictor of response. Results: Among 152 patients (71.1% male; mean age: 35.9 ± 12.2 years), 88 (57.9%) received biologic therapy. Patients treated with biologics presented with significantly more severe disease (mean IHS4: 13.7 ± 7.9 vs. 3.8 ± 4.4; p < 0.001) and achieved greater absolute improvement (ΔIHS4: −7.6 ± 9.0 vs. −2.2 ± 5.0; p < 0.001). Cox regression demonstrated that therapeutic delay inversely predicted response (p = 0.045). Hurley stage strongly correlated with IHS4 (Spearman r = 0.904; p < 0.001). Male patients represented 83.0% of the biologic therapy group compared to 54.7% in the nonbiologic group (p < 0.001). Smoking prevalence was 75% but did not significantly affect treatment response. Conclusions: Earlier initiation of biologic therapy may be associated with greater clinical improvement in HS; however, this finding should be interpreted with caution due to the retrospective design, baseline imbalances, and potential confounding. The concept of a therapeutic \"window of opportunity\" remains hypothesis-generating and requires confirmation in prospective studies with standardized outcome measures.

    The effect of metformin on inferior alveolar nerve regeneration following crush-type injury

    Peripheral nerve injuries often cause significant sensory and motor dysfunction, but current treatments provide limited neurosensory recovery. Beyond its antidiabetic use, metformin shows promise as a neuroprotective and regenerative agent by modulating autophagy and reducing inflammation. This study aimed to investigate the potential therapeutic effects of metformin on inferior alveolar nerve (IAN) regeneration following crush-type injury in a rat model. Thirty male Sprague–Dawley rats were randomly divided into metformin, placebo, and control groups. A crush injury was performed on the IAN in all but the control group. The metformin group received daily intraperitoneal 5 mg/kg injections for 10 days. Regeneration was assessed on day 28 by histological and immunohistochemical analyses, and functional recovery was evaluated using the von Frey test at day 0 (preoperative baseline) and on postoperative days 1 (T1), 7 (T7), 10 (T10), and 28 (T28). Histologic results showed reduced inflammation, oedema, fibrosis, and degeneration, and greater regenerated nerve fibre density in the metformin group. Immunohistochemical analysis showed a marked elevation in GAP-43 expression in the metformin group compared to the placebo and control groups (P < 0.05). Von Frey results showed gradual improvement in mechanical sensitivity in the metformin group, suggesting better recovery. Metformin showed neuroregenerative effects on crushed IAN in rats by promoting repair, increasing GAP-43, and reducing inflammation. These findings support its therapeutic potential in peripheral nerve injury and warrant further research for clinical application.
