Abstract
Protective Effect of Micronized Purified Flavonoid Fraction on Renal Ischemia Reperfusion Injury in Experimental Ischemic Reperfusion Model and Partial Nephrectomy Rats Objective: Partial nephrectomy is the standard treatment for kidney tumors smaller than 4 cm.In this study, we aimed to investigate the protective effect of purified micronized flavonoid fraction (SMFF) on I-R injury after partial nephrectomy performed in solitary kidney and bilateral kidney. Materials and Methods: Forty-eight male Sprague-Dawley rats weighing 200-250 g were randomly assigned to a solitary kidney model (1st- 2nd and 3rd groups) and bilateral kidney model (4-5th and 6th groups) were divided into 2 main groups. These 2 main groups were divided equally into 6 groups, namely Sham group (1st and 4th group), ischemia-reperfusion (I-R) group (2nd and 5th group) and treatment group (3rd and 6th group). Right nephrectomy was performed on the rats in the first 3 groups to establish a solitary kidney model. We waited for 1 month for the developement of compensatory hypertrophy. After entering the abdomen in the Sham groups, the left renal pedicle was dissected and closed without renal ischemia. In the I-R groups and the treatment groups, partial nephrectomy of the lower pole was performed within 45 minutes of ischemia and post-ischemic 24-hour reperfusion period completed. Prior to surgery, for 3 days, Once a day and about 1 hour before the operation, 80 mg / kg (1 ml / kg) SMFF treatment was given to treatment groups (groups 3 and 6) by oral gavage. In the I-R groups (2nd and 5th groups), %0.9 NaCl was given at 1 ml / kg for the same time and same amount as SMFF. Rats were sacrificed at the end of 24 hours after blood samples from the heart and left kidney tissues were taken. Results: Change in the oxidative stress markers and inflammatory markers (TAK, TOS, OSI, PON1, ARES, CAT, MDA, IL-1β and TNF-α) measured in tissue and serum in the I-R groups was found to be significant in relation to the Sham group. However, despite the improvement in values in the treatment groups compared to the I-R groups, this improvement was not statistically significant. The changes in PCO (p <0.05) and DNA damage (p <0.01) values were statistically significant in the treatment groups according to I-R groups. Renal histopathological damage score was found to be significantly lower in the Sham group than in the other groups, although there was a relative improvement in the histopathological damage score in the treatment groups compared to the I-R group, it was not statistically significant. There was no difference between the Sham groups (group 1 and 4), I-R groups (group 2 and 5) and treatment groups (group 3 and 6) when bilateral kidney and solitary kidney groups were compared with each other. Conclusion: In our study, which we aimed to evaluate the effect of SMFF treatment in renal injury associated with warm ischemia-reperfusion occurred during partial nephrectomy, SMFF treatment did not protect the renal tissue from ischemia-reperfusion injury to the extend we expected, SMFF treatment was found to be successful in preventing DNA damage in the renal tissue in I-R, It has been concluded that SMFF treatment can reduce renal I-R damage to a certain extent, regardless of whether it is solitary or bilateral, but when histopathological evaluations were considered, we did not achieve the expected positive effects.
