Alcohol Consumption and Views on Alcohol Research and Control Policies in Turkey: A Qualitative Study

This study aims to determine the culture of alcohol consumption and elicit the views of drinkers, former drinkers, and nondrinkers on alcohol consumption, alcohol research, and alcohol control policies. The data were collected through semi-structured interviews and analysed using Thematic Framework analysis. Alcohol consumption was described as a means for socialising and relaxing by the drinkers, whereas unhappiness, anxiety and depression by the non-drinkers. The participants have argued that the responses in alcohol studies are unreliable due to social characteristics and that alcohol research should be conducted by institutions, not the state. Interviewers were suggested to have come across as objective, non-judgmental, and likeable with a reassuring attitude and to have used body language appropriately. The participants support a positive approach in control policies by increasing awareness, and prohibition is not believed achievable in practice or to be an effective alcohol control policy as it instead makes drinking attractive. Therefore, the recommendation is to target the whole population by increasing facilities with social activities and encouraging social life. In conclusion, the study has provided valuable hints for planning alcohol research and control policies, and it is especially useful for understanding the background of alcohol consumption culture in Turkey.

Lack of policy-oriented research in Turkey could be attributed to the perceptions of researchers on existence of an alcohol use problem in the population, methodological difficulties of conducting alcohol research, and the perceived sensitivity of the topic in the population. This descriptive quantitative study was carried out before the conduct of the International Alcohol Control Study (IAC) in Turkey, with an aim to assess the knowledge, attitude, and behaviors on alcohol research of 142 adults aged 18 years and above, who have the experience of consuming alcohol, to determine their preference for interviewer characteristics, interview place, and research institution. More than half of the participants (54.9%) stated that alcohol consumption studies were necessary, and a majority (74.7%) was willing to participate in such a study. The participants believed that the alcohol consumers from their own social environments would express their drinking status openly (72.5%) and report their consumption accurately (70.4%). Use of political or religious symbols by the interviewers did not seem to influence responses for more than half of the study subjects, although a non-negligible group have not agreed. The participants favored World Health Organization as the research institution (53.2%) and medical students (62.9%) as the interviewers.

Public Health research should aim to influence policies, and therefore researchers should determine the issues and highlight the salient problems and possible solutions in their area of interest. It is suggested that delaying the onset of alcohol consumption among youth has a positive impact on health and social development. This study aims to review existing school surveys with regard to the availability of information which can influence policies, so as to guide future research. A wealth of cross-studies was found on harmful habits including alcohol use conducted among 18-year-old youths and under. Twenty-eight scientific articles and one report met the inclusion criteria and these were reviewed. A prevalence of lifetime drinking was found to range between 4.4% and 65.0%. This literature review emphasizes the importance of well-designed studies on alcohol consumption in this young age group. Temporal and spatial comparisons as well as individual and population level determinants of early-onset and harmful drinking should be explored in order to formulate effective policies for youths.
