    Intraoperative Complications and Conversion to Laparatomy in Gynecologic Robotic Surgery

    Introduction In this study our objective was to document complications encountered during our initial experience with the robotic system and also state the cases in which conversion to laparotomy was necessary. Material and Methods This study is a retrospective analysis of robotically performed gynecological and gynecologic oncology procedures at a single center from July 2016 to July 2018. Patient demographics and preoperative indications were obtained from the electronic medical records. Results The patients had a mean age of 53.6 years (range, 25-84 years). The operative time ranged from 1 h and 50 min to 9 h (mean, 5 h and 2 min). Most of the complications were managed within minutes and with robotic assisted suturing when necessary. Five patients out of 83 patients needed a surgical conversion from robotic surgery. Conversion rate was 6.02%. Conclusion During the study period we were able to manage complications uneventfully without requiring conversion to laparotomy most of the time. Vascular complications encountered during robotic surgery can be managed without requiring conversion to laparatomy.

    Family planning usage status in women aged 15-49 and the affecting factors: example of kahramanmaras province

    Family planning and education have an important role in the positive management of many health indicators such as reducing maternal and infant mortalities, healthy and controlled pregnancy, and postpartum maternal and baby health. This study was carried out to examine the knowledge level of women of reproductive age about family planning methods and to determine their usage status. This research is a cross-sectional descriptive study. Women aged 15-49 who applied to health institutions in Kahramanmaraş and needed to use the family planning methods constituted the population of the research. 1000 women were included in the research without sampling. The questionnaire which was prepared by the researchers and included questions about socio-demographic characteristics and family planning was applied to women. Data were analyzed in the SPSS 19.0 program. The data were given in the form of averages, percentages and distributions, and chi-square test was applied. According to analysis results, it was determined that 84.2% of women preferred at least one of the family planning methods. The ratio of using method significantly increased as the age increased (p=0.001). It was determined that the ratio of using method increased as the educational level increased although it was not statistically significant. It was determined that the women who participated in the research started to use method after the second child on an average. At the same time, the ratio of using method also increased significantly as the number of delivery of women increased (p=0.000).It was determined that the socio-economic levels of women are important factors in the usage and preferences of family planning which has an important place in terms of women and children's health.

    Total Surgical Excision by Ultrasound-Guided Wire Localization for Spontaneous Abdominal-Wall Endometriosis

    Background: Extrapelvic endometriosis is the most-common form of abdominal-wall endometriosis (AWE). This rare condition usually results from transport of endometrial tissue into incision sites of surgical operations or adjacent tissues. Spontaneous AWE also occurs. A common symptom is a painful and tender mass usually associated with menstruation. Noncyclic pain can occur in 45% of patients. Standard treatment is complete surgical excision. Case: This 44-four-year-old patient, gravida 0, presented with persistent cyclic pain in her right inguinal area, but she had no history of previous abdominal surgery Ultrasonography (USG) showed an obscure, heterogeneous-hypoechogenic mass of similar to 2.0 x 1.5 cm in the right lower quadrant of her abdominal wall. She underwent total surgical excision by ultrasound-guided-wire localization. Results: The nonpalpable mass that was localized via an ultrasound-guided wire was surgically excised. A diagnosis of AWE was confirmed by histopathologic examination of the mass. Two months postoperatively, this patient-s pain disappeared. Conclusions: Extrapelvic endometriosis can be located in almost any organ of the body, including the abdominal wall. The incidence of AWE is reported to be up to 0.03%-3.5%, and most cases of AWE commonly arise in an incision made during a cesarean section. Spontaneous AWE is less common, compared to cesarean-scar endometriosis. The diagnosis of AWE is difficult due to its rarity, subclinical presentation, and generally nonpalpable lesions. Ultrasound-guided-wire localization is a reliable, simple, and practical solution for localizing nonpalpable lesions.

    Analysis of preoperative and postoperative quality of life, sexual function, and sleep in patients with endometriosis: a prospective cohort study

    Purpose Endometriosis affects the quality of life, sleep, and sexual life of patients due to pain. This study compared the scores of endometriosis patients in these three areas before and after surgery. Methods Patients between the ages of 18 and 60 with a prediagnosis of endometriosis were enrolled. Postoperative histopathological diagnosis of endometriosis was confirmed in all patients. This study included 56 patients who completed pre- and postoperative (three months) evaluation of quality scale questionnaires: a visual analog scale for pelvic pain, the Pittsburgh Sleep Quality Index, Morningness-Eveningness Questionnaire, Endometriosis Health Profile-30 Questionnaire, and Female Sexual Function Index were administered prior to and 3 months after each patient-s surgery. Results Among the 56 female patients included in this study, statistically significant improvement was observed in pain scores, quality of life, sexual function, and sleep of all patients regardless of endometriosis stage. Conclusion Endometriosis is a disease that progresses, with increasing pain scores; it has negative effects on the quality of life, sexual function, and sleep of patients. Surgical or medical treatment can be performed considering the complaints and fertility status of the patients.
