    Does ozone administration have a protective or therapeutic effect against radiotherapy-induced testicular injury?

    Objective: We investigate the protective and therapeutic effects of ozone therapy (OT) in radiotherapy (RT)‑induced testicular damage. Methods: Thirty healthy adult male Wistar rats divided into five groups consisting of six animals each as follows: (1) Control (C), (2) RT, (3) OT, (4) OT + RT, and (5) RT + OT group. Histopathological findings, Johnsen scores, thiobarbituric acid‑reactive substances (TBARS), glutathione (GSH), superoxide dismutase (SOD), catalase, and glutathione peroxidase (GPx) levels were evaluated. Results: RT caused a significant decrease in testicular weight and Johnsen score compared to the control group. In addition, TBARS level was significantly higher, whereas GSH, SOD, catalase, and GPx levels were significantly lower in the RT group when compared to the control group. Pre and postRT OT significantly increased GSH, SOD, catalase, and GPx levels and decreased TBARS level. Furthermore, testicular weight and Johnsen score were increased with OT. Conclusions: The present study showed that OT is protective and therapeutic in radiation‑induced testicular damage. OT may be beneficial to the patients who underwent RT.

    Outcome of Daily Cisplatin with Thoracic Chemoradiotherapy in Advanced Non-small Cell Lung Cancer Patients with Comorbid Disorders: a Pilot Study

    Background: Lung cancer is the most common cancer in males worldwide. The principal mode of treatment in the early stage of non-small cell lung cancer (NSCLC) is surgery. However, five-year survival is only about 15% for all stages. The aim is to investigate the effect of daily low dose cisplatin concurrently with radiation therapy in advanced NSCLC patients with poor performance status. Materials and Methods: Ten patients diagnosed as inoperable Stage IIIB NSCLC with comorbid disease were assessed retrospectively in Bezmialem Vakif University, Faculty of Medicine, Department of Radiation Oncology, between 2011 to 2013. ECOG performance status was between 3 and 4. Cisplatin was administered at 6mg/m2 daily, for 5 days a week concurrently with radiotherapy using 160-200 cGy daily fractions, 54 Gy being the lowest and 63 Gy being the highest dose. Results: Complete response at the primary tumour site was obtained in 20% patients. Grade I esophagitis was seen 70 percent of patients, and the grade II haematological toxicity rate was 20 %. Median survival time was 7 months. Conclusions: Median survival time was reasonable, despite the patients ECOG performance status of 3-4, which is similar to groups even without comorbid disorders in comparison to other published papers in the literature. Acceptable toxicity, high response rates and quality of life of patients are the other favourable features.

    Objective: There is an increase in the incidence of cancer, and consequently in mortality rates, both in the world and in Turkey. The increase in the incidence and mortality rate of cancer are more prominent in our country as well as in other developing countries. The aim of this workshop was to determine the current status on prevention, screening, early diagnosis and treatment of cancer in our country, to identify related shortcomings, specify solutions and to share these with health system operators, and to aid in implementation of these systems. Developments on palliative care were also evaluated. Materials and methods: The current situation in the practice of clinical oncology, related drawbacks, problems encountered during multidisciplinary approach and their solutions were discussed under several sub-headings during a 3-day meeting organized by the Turkish Ministry of Health (Türkiye Cumhuriyeti Sağlık Bakanlığı-TCSB) with participation of 16 scientists from Turkey and 6 from abroad, and the conclusions were reported. Results: It is expected that the newly established Turkish Health Institutes Association (Türkiye Sağlık Enstitüleri Başkanlığı-TÜSEB) and the National Cancer Institute (Ulusal Kanser Enstitüsü) will provide a new framework in the field of oncology. The current positive findings include the increase in the number of scientists who carry out successful trials in oncology both in Turkey and abroad, the implementation of the national cancer registry program by the Cancer Control Department and the breast cancer registry program by the Turkish Federation of Breast Diseases Societies (Türkiye Meme Hastalıkları Dernekleri Federasyonu-TMHDF), and introduction of Cancer Early Diagnosis, Screening, and Training Centers (Kanser Erken Tanı, Tarama ve Eğitim Merkezi-KETEM) for the application of community-based cancer screening programs. In addition to these, obvious shortcomings related to education, implementation, management and research issues were also determined, and policy and project proposals to address these issues were presented. Collaboration with relevant organizations in the implementation of these studies was supported. Conclusion: Both the incidence and mortality rates of cancer are increasing in Turkey. The widespread deficiencies in population-based screening and in effective treatment lead to an increase in delay in diagnosis and mortality. Despite improvements in data recording, screening and treatment over the last 10 years, extensive, organized, population-based screening programs and fully equipped early diagnosis and treatment centers are required. Enhancement of basic cancer epidemiologic, translational, genetic and molecular research studies is essential in our country. Improvements on pain treatment and palliative care of patients with chronic and terminal cancer are also required.

    THE EVALUATION OF CELLULAR AND HUMORAL IMMUNITY CHARACTERISTICS IN GASTROINTESTINAL AND HEPATOCELLULAR CANCERS

    Introduction: Gastrointestinal cancers (GI Ca) are a global health problem leading to high morbidity and mortality all over the world. Human organism presents a well organised defense mechanism against cancer cells by humoral and cellular immune system. Nowadays, cancer immunotherapy is an essential and promising research field of modern immunology. In this study, we aimed to evaluate the characteristics of the humoral and cellular immune systems in gastrointestinal and hepatocellular cancers (HCC) patients. Methods: Cellular and humoral immune parameters (CD3, CD4, CD8, CD16 / CD56 and CD 19 lymphocytes respectively) were investigated in the peripheral blood samples of 71 gastric Ca, 50 pancreatic Ca, 27 colorectal Ca, 50 HCC patients and of 30 healthy persons as control group by flow cytometry method. One way Anova and post hoc Dunnett tests were used to compare mean lymphocyte values of cancer groups with the control group. Significance level is determined as p: 0.05. Results: CD3 and CD4 lymphocyte levels were not significantly different in all cancer groups compared to the control group (Table-1). CD8 lymphocyte levels were significantly increased in pancreatic and colorectal cancer groups (p < 0.05) whereas CD19 lymphocytes were significantly lower in the same groups (p < 0.01-0.05). Natural killer (CD16 / CD56) lymphocytes were significantly higher in gastric Ca, colorectal Ca and HCC patients compared to the control group (p < 0.01-0.05).

    Bilateral carotid paraganglioma: surgery and radiotherapy.

    Background: Paragangliomas are relatively rare vascular tumors that develop from the neural crest cells of carotid bifurcation. They usually present as slow-growing, painless unilateral neck masses; bilateral presentation is rare and is mostly associated with familial forms. Bilateral total resection is not always possible for high-grade bilateral tumors, and radiotherapy is a good alternative, with cure rates similar to surgery. Case report: A 35-year-old female patient was admitted with a chief complaint of a bilateral, painless mass located on her neck. Subsequent magnetic resonance imaging (MRI) and angiographic imaging revealed bilateral hypervascular masses surrounding her carotid at 360°, and they were interpreted as stage 3 carotid paragangliomas according to the Shamblin classification protocol. Surgery was carried out on the left carotid paraganglioma and the mass was totally resected. It was thought that the patient could not tolerate bilateral surgery. Primary radiotherapy was planned on the right carotid paraganglioma: 59.8 gray (Gy) conformal, Linac-based multileaf collimator radiotherapy with a 180 cGy daily dosage, and five fractions per week were planned. Results: Follow-up at 3 months following the conclusion of radiotherapy revealed no significant regression. A follow-up MRI 6 months and 24 months later revealed 59% regression. Grade 2 esophagitis and minimal neck edema were the only complications noted during the course of radiotherapy and during the 24-month follow-up period. No complications or relapse were observed except for edema following neck surgery.

    Effects of Low-Dose Radiation on the Survival of Lung Cancer Patients

    Objective: Some previous studies have shown that low-dose radiotherapy (RT) can increase tumor invasion and metastasis. Multiple RT fields are usually used to prevent the damage to the organs at risk. We performed a clinical study with a concern that low-dose RT might increase invasion or metastasis. Ionizing radiation (IR) can enhance the potential tumor micro-environment by modifying the host micro-metastatic cancer cells.

    Importance of Performing Radiotherapy and Chemotherapy in the Same Clinic and Bad Prognostic Factors for Small-cell Lung Cancer Patients

    Objective: We evaluated different treatment results reported and showed the effect of treatment at single and multiple clinics for small-cell lung cancer (SCLC). We attempted to show a decreasing impact of chemotherapy (CT) and thoracic radiotherapy (RT) treatment results when implemented at different clinics for SCLC compared to the treatment results at a clinic. Methods: We conducted a retrospective study on 54 non-metastatic SCLC patients who underwent treatments at various clinics. Patients underwent 1–12 courses of CT before they came to the clinic for thoracic RT. RT was performed at 180–400 cGy dose per fraction for a total of 30–52 Gy doses, and patients were followed for 12–60 months. Results: When the study was reviewed, the results showed that the median disease-free survival and survival rates were 8 and 9 months and that the 2-, 5-, and 5-year survival rates were 8%, 6.3%, and 1.8%, respectively. The median progression-free survival rates for 2 and 3 years were 4%, and for 5 years, it was 1.8%. Weight loss for disease-free survival (p=0.01) and superior vena cava syndrome for overall survival (p=0.02) were considered as bad prognostic factors. In this study, acceptable toxicity values were found when the results were compared with those from other studies. Conclusion: We obtained worse results than those from literature data on our SCLC patients who came to our clinic after the progression of their disease. The main causes were identified as insufficient staging and different treatment protocols applied at different clinics. Therefore, we argue that CT and thoracic RT for SCLC must be performed at the same clinic and that the same protocols and staging methods must be used.
