    The effects of different mechanisms on the development of post-ERCP pancreatitis in an ERCP model in rats

    Background/aims: To investigate the effects of different mechanisms on the development of pancreatitis after endoscopic retrograde cholangiopancreatography. Material and Methods: 40 male rats were randomly divided into four groups. After laparotomy, in Group 1, only duodenum was reached by a 24G cannula without performing any other procedure. In Groups 2, 3, and 4, biliopancreatic duct was cannulated transduodenally. Group 2 received no additional intervention after the cannulation. Group 3 received saline, whereas Group 4 received contrast agent into the duct. After a period of 24 hours, all rats were sacrificed. Laboratory tests for blood samples were performed and pancreatic tissue was also evaluated histopathologically. Results: Leukocyte, blood sugar, serum glutamic oxaloacetic transaminase, lactate dehydrogenase, amylase, C-reactive protein, and base excess parameters were evaluated. The values in Groups 2, 3, and 4 were found to be significantly higher than those in the control group, except for leukocyte count and base excess (p=0.551, p=0.031, p=0.0001, p=0.0001, p=0.0001, p=0.0001, p=0.683, respectively). Histopathological results demonstrated significant differences between the groups. Highest pathological damage scores were observed in Groups 3 and 4. Conclusion: Among different theories for the pathogenesis of post-endoscopic retrograde cholangiopancreatography pancreatitis, elevated intraductal hydrostatic pressure was observed to be the main underlying cause.

    Psychometric properties of the Turkish version of the Yale Food Addiction Scale among bariatric surgery patients

    Objective: The aim of this study was to examine validity and reliability of Turkish version of Yale Food Addiction Scale (YFAS) among Turkish bariatric surgery patients. Methods: The YFAS scale was administered to obese patients (n=171) who were seeking or underwent bariatric surgery. Construct validity of the scale was evaluated with factor analysis and reliability was evaluated with item-total score correlation and repeatability were tested by intraclass correlation (ICC) analysis between test-retest results. Results: Internal concistency was found adequate Kuder-Richardson Formula 20 coefficient (KR-20) 0.822, and Cronbach-s alpha 0.859 for the entire 25-item YFAS. As Bartlett-s Test of Sphericity was significant, the factor model developed in the present study was decided appropriate. Factor analysis extracted six factor in Turkish YFAS that explained for 67.51% of the total variance. Item total correlation coefficients of scale ranged from 0.214-0.666. Conclusion: Our findings support the use of the Turkish YFAS as a reliable measure of food addiction among bariatric surgery patients.

    Spontaneous Perforation of Pyometra; a Rare Cause of Acute Abdomen: Report of a Case

    Pyometra is defined as pooling of purulent material in the uterine cavity. We report a perforated pyometra presented with acute abdomen who was initially diagnosed as gastrointestinal perforation. A 68 year old woman was admitted as an emergency having developed sudden lower abdominal pain. Plain abdominal X-ray revealed free air in the peritoneal cavity. At laparotomy, enlarged perforated uterus of 12x7x4 cm in size was revealed. Hysterectomy and bilateral salpingooophorectomy was performed. Postoperative period was uneventful. Patient was discharged at thirteenth day. Although rare, perforation of pyometra should be considered in the differential diagnosis in an elderly woman with an acute abdomen.

    Remission of ulcerated necrobiosis lipoidica diabeticorum after bariatric surgery.

    A 32-year-old woman with type 2 diabetes mellitus suffering from morbid obesity with BMI 45,14 kg/m2 was operated on. Not only the type 2DM but also one of its complication known as necrobiosis lipoidica diabeticorum remitted postoperatively. Obesity should no longer be regarded simply as a cosmetic problem affecting certain individuals but an epidemic that threatens global well-being. It causes or exacerbates many health problems, and in particular, it is associated with the type 2 diabetes. Necrobiosis lipoidica is a granulomatous skin disease of unknown etiology, associated mainly with diabetes mellitus. We presented in this paper a morbid obese case of necrobiosis lipoidica diabeticorum with dramatic good response to bariatric surgery.

    Effect of probiotic supplementation after laparoscopic sleeve gastrectomy on constipation and gastrointestinal quality of life

    Aim: In this study, we aimed to investigate the early effect of probiotic supplementation after Laparoscopic Sleeve Gastrectomy (LSG) on constipation and gastrointestinal quality of life compared to control group. Methods: This study was a prospective, randomized clinical trial. Participants were recruited to Bariatriklab Obesity and Metabolic Surgery Center for LSG. All patients were divided into 2 groups as probiotic and control by using simple randomization. The probiotic group consumed Bifidobacterium animalis lactis BB-12 strain as a probiotic supplement during 6 weeks after LSG. Gastrointestinal Symptom Rating Scale (GSRS), Constipation Severity Instrument (CSI), Patient Assessment of Constipation Quality of Life Scale (PAC-QOL), Bristol Stool Form Scale (BSFS), Gastrointestinal Quality of Life Index (GIQLI) of the patients were recorded before LSG and at the 2nd, 4th, 6th weeks after LSG. Results: The probiotic group had an average age of 37.00±8.92 years (18 female, 12 male), the control group had an average age of 41.03±11.29 years (23 female, 7 male). CSI (16.50 ± 14.76 vs. 31.37 ± 15.34), PAC-QOL (58.53 ± 12.59 vs 72.30 ± 19.70), GSRS (26.83 ± 9.14 vs. 37.93 ± 16.59) and total score mean were lower compared to the control group, GIQLI total score average (147.50 ± 11.79 vs 136.87 ± 18.98) was found higher (p <0.05) in probiotic group. Conclusions: Probiotic supplementationimproved the constipation and gastrointestinal quality of life in the early post LSG-period in the brobiotic group compared to the control group.

    Assessment of the Correlation Between Weight Status and the Frequency of Dietician Interviews in Sleeve Gastrectomy Patients

    Purpose The aim of this study was to evaluate the relationship between the weight status of sleeve gastrectomy patients during the first two postoperative years and the frequency of dietician interviews. Materials and Methods This was a retrospective cross-sectional study, and the subjects were patients who had surgery at a special obesity clinic, Bariatriklab, in Istanbul, Turkey, between February 01, 2014, and February 01, 2018.The mean age +/- standard deviation of the participants was 38.8 +/- 11.9 years. The mean body mass index (BMI) +/- standard deviation was 44.2 +/- 6.1 kg/m(2)preoperatively. The preoperative (pre-op) and 12-, 18-, and 24-month postoperative (post-op) weights and the frequency of interviews with dieticians for 2 years after the operation were collected, and statistical analysis was performed using SPSS ver. 23.0. Results A total of 247 laparoscopic sleeve gastrectomy (LSG) patients, 161 women (65.2%) and 86 men (34.8%), were evaluated in this study. As the frequency of dietician interviews increased in the first year, the amount of excess weight loss (EWL) increased in the second year (p < 0.01). In addition, as the frequency of dietician interviews increased in the second year, the amount of EWL in the second year decreased (p < 0.01). The frequency of dietician counseling among participants who did not experience weight loss between 12 and 24 months was significantly higher than that among those who experienced weight loss (p < 0.05). Conclusion It should not be forgotten that bariatric surgery requires teamwork, and patients should be cared for with an interdisciplinary approach. Dieticians play an important role in changing nutritional habits and making them sustainable.

This prospective randomized study aimed to compare the effects of SLR methods on early postoperative complications and gastrointestinal symptoms in patients undergoing LSG for morbid obesity.
