Publication Metadata only Dependence of grip strength on shoulder position and its implications for ergonomics practice(2019-05-01T00:00:00Z) Eryigit, Sevim; Ugurlu, Umit; UĞURLU, ÜMITGrip strength (GS) variability due to positional changes in the upper extremity joints is of importance while designing workstations and work methods. This study was conducted to analyze the GS variations due to positional changes at shoulder joint when some important variables were under control. The GSs of dominant and nondominant hands were measured in eight shoulder (0 degrees, 45 degrees, 90 degrees, and 135 degrees of flexion and abduction) and standard test positions (STP). One hundred and thirteen subjects 20-30years old completed the study. At the dominant side, no significant difference was observed in the pairwise comparisons between STP and the others. Maximum and minimum GSs were obtained in 0 degrees abduction and 45 degrees flexion and abduction, respectively. At the nondominant side, GSs were significantly lower (p<0.001) in the corresponding test positions and demonstrated more variability. İlerleyici cilt tutulumu eklemlerde yaygın kontraktürlere, kas zayıflıklarına ve günlük yaşam aktivitelerinde belirgin kısıtlanmalara neden olabilir. Bu vaka raporunda lokalize skleroderma tanısı alan 33 yaşındaki bir kadın hastanın rehabilitasyon süreci anlatılmaktadır. Hastanın sol üst ekstremitesinde skapulayı da içine alan bir bölgede lokalize sklerotik değişiklikler ve buna bağlı ileri düzeyde eklem kontraktürleri vardı. Tutulum bölgesinde aktif eklem hareket açıklıkları (EHA), kas, el tutma ve kavrama kuvvetleri ölçüldü. Üst ekstremite fonksiyonellik seviyesini belirlemek için The Disabilities of the Arm, Shoulder and Hand (DASH) anketi kullanıldı. Terapinin temel unsurları nemli ısı uygulaması, aktif, germe ve güçlendirme egzersizleri ve dirsek fleksiyon kontraktürü için statik progresif germe ortezi kullanımıydı. Egzersizler haftada 3 defa terapist kontrolünde yaptırıldı. Hasta diğer günlerde ev egzersiz programı uyguladı ve dirsek ortezini kullanmaya devam etti. Terapinin etkinliğini belirlemek için başlangıçta ve 1 ay sonra yapılan değerlendirme sonuçları karşılaştırıldı. EHA’larında genel bir artış elde edildi. Bu artışlar omuz abdüksiyonu ve dirsek ekstansiyonunda 40 dereceye ulaştı. El tutma kuvvetlerinde bir miktar azalma görülmesine karşın kavrama kuvveti 5, 6 kg arttı. DASH anketi puanında belirgin ilerleme görüldü. Bununla birlikte manuel kas testi sonuçlarında herhangi bir değişiklik gözlenmedi. Bu raporda, statik progresif germe ortezi kullanımıyla desteklenen geleneksel rehabilitasyon yöntemlerinin lokalize sklerodermalı hastalarda rijid kontraktürlerin düzeltilmesinde ve fonksiyonel durumun iyileştirilmesinde etkili olabileceği gösterilmiştir.
