    Heat stroke is the outcome of over heat stress that results in multiorgan dysfunction with a tendency for central nervous system damage. Brain is very sensitive to hyperthermia, especially the cerebellum that has selective vulnerability to heat stroke. There is complex interaction between heat cytotoxicity, coagulation disorder, cytotoxine - mediated systemic inflammatory response causing multiorgan failure, metabolic derangement, and circulatory insufficiency. We reviewed the literature and discussed brain MRI and MR spectroscopy findings of heat stroke, detailed the pathophysiology underlying brain involvement and proposed excitotoxic injury as an alternative mechanism of brain damage in heat stroke.

    MRI and MRS findings in fucosidosis; a rare lysosomal storage disease

    Fucosidosis is a rare lysosomal storage disorder caused by deficient activity of the enzyme L-fucosidase in all tissues. We presented magnetic resonance imaging [MRI] and MR spectroscopy [MRS] findings of a 4-year-old boy with genetically proven fucosidosis. He had a history and clinical findings of recurrent sinopulmonary infections, hypertonicity on lower extremities, gingival hypertrophy, bilateral ptosis, angiokeratoma corporis diffusum, and dysostosis multiplex. He had no organomegaly and urine glycosaminoglycan analysis were normal. MRI revealed abnormalities within the globus pallidus and periventricular and subcortical white matter. MRS showed a peak at the 3.8-3.9 ppm as a result of accumulating carbohydrate containing macromolecules and another peak at 1.2 which was doublet and inverted on TE 135, suggesting fructose peak. A final diagnosis of fucosidosis was proved by mutational analysis of FUCA1 gene which is responsible for the Fucosidosis phenotype. Two recent reports of MRS of two patients demonstrated that MRS is specific for fucosidosis. In this case, we aim to discuss fucosidosis with MRI and MRS findings accompanied by the literature.     Imaging Spectrum of Breast Papillary Lesions: With Special Emphasis on Atypical Appearances

    Papillary breast tumors are rare breast tumors. Presentation of papillary breast lesions varies clinically and radiologically. Standard diagnostic work-up for papillary breast lesions includes various radiological modalities such as mammography, galactography, ultrasound and MRI. Papillary lesions often have a wide spectrum of appearance on different radiological modalities, so that optimal differentiation of papillary lesions is not easy with various imaging methods. The purpose of this review article is to describe the different imaging appearances of benign and malignant papillary lesions of the breast with special emphasis on atypical appearances.

    Diffusion tensor imaging findings of the brainstem in subjects with tonsillar ectopia

    We aimed to evaluate the differences between apparent diffusion coefficient (ADC), fractional anisotropy (FA), axial diffusivity (AD), and radial diffusivity (RD) values obtained from different cranial sites in subjects with Chiari I Malformation (CM-I) and borderline tonsillar ectopia (BTE), and to determine correlations between diffusion tensor imaging (DTI) metrics and the severity of tonsillar ectopia. A total of 73 subjects with CM-I and BTE and 35 control underwent MRI and DTI. In our study, ADC values measured from the level of medulla oblongata and the RD values measured in middle cerebellar peduncles, thalamus, and globus pallidus were higher in CM-I patients than in controls. FA values at the medulla oblongata level and AD values at the medulla oblongata and pons level higher in patients with CM-I. ADC and AD values measured at the pons level were higher in BTE subjects than in controls. Compared with BTE, the CM-I subjects- ADC values at the medulla oblongata and AD values at the pons level were higher. In addition, FAs at the pons and medulla oblongata level were higher. At the medulla oblongata level, a positive correlation was observed between ADC and the size of tonsillar ectopia. AD and FA values measured at the level of medulla oblongata and pons were positively correlated with the size of tonsillar ectopia. These findings may be related to the severity of microstructural changes involving neuronal tracts at the brainstem level due to tonsillar ectopia. DTI may be useful in determining the extent of microstructural changes at the tissue level in subjects with tonsillar ectopia.
