    Assessment of electronic media use in children and adolescents with attention deficit hyperactivity disorder

    Objective: Electronic media use is an emerging area of research interest, however its relationship with Attention Deficit hyperactivity disorder (ADHD) is neglected in Turkish literature. We therefore aimed to examine its relationship with certain variables and ADHD subtypes. Methods: A total of 360 participants aged between 6-18 years, who attended Bezmialem University, Child and Adolescent Psychiatry Outpatient Clinic between January 2016 and October 2016, were screened with the Schedule for Affective Disorders and Schizophrenia for School Age Children K-SADS) interview to ascertain diagnoses and a sociodemographic information tool was used to collect the relevant data. Results: The mean age of the sample was 9.47 +/- 2.67 years, and 81.9% of them consisted of males. 20.6% of the sample was reported to spend between 2 and 4 hours a day using electronic media gadgets (TV viewing, cell phone and/or tablet/computer use), while 4-6 hours of use was reported in 15.8%, 6-8 hours in 11.4%, 8-10 hours in 9.4% and more than 10 hours/day in 18.2% of the total sample. Heavy users of electronic media (> 6 hours/day) reportedly had significantly higher academic achievement and lower socioeconomic status as compared to those who used it at medium (2-6 hours/day) and low levels (< 2 hours/day). Gender, the content of TV viewing and total electronic media use within the household also significantly differed between the heavy, medium and low intensity users. Regression analyses revealed that total daily electronic media use for 10 hours and above was significantly and independently associated with the current family psychiatric illness and low academic achievement of the child. Conclusion: To the best of our knowledge, this is the first study reporting use of a variety of electronic media gadgets in a Turkish clinical sample of children and adolescents with ADHD. Results are in line with the existing international literature and highlights the excessive electronic media use in this clinical population. We recommend a routine screening for electronic media exposure in ADHD minors and increase awareness in their families and schools. Longitudinal and methodologically more robust studies are needed to examine cause-effect relationships.

    Psychiatric comorbidity in children with psychogenic and functional breathing disorders.

    Background: The present study aims to assess psychiatric diagnoses in children with psychogenic and functional breathing disorders (PFBD), which consist of children with psychogenic cough, throat-clearing tics, and sighing dyspnea, and compare them to a control group without any diagnosis of chronic medical problems. Methods: The participants consist of 52 children with PFBD and 42 children without any chronic medical problems. Psychiatric diagnoses were assessed via semistructured psychiatric interviews in both groups. Results: The two groups did not differ on age (PFBD group 11.25 ± 2.61, control group 11.17 ± 2.58; t = 0.14, P = .88) or sex (48.1% of the PFBD group were female, 61.9% of the control group were female; χ2 = 1.79, P = .18). 55.8% of the PFBD group and 28.6% of the control group had at least one psychiatric diagnosis according to the semistructured interviews (χ2 = 6.99, P = .008). The most common psychiatric diagnoses in the PFBD group were attention deficit hyperactivity disorder (ADHD; 17.3%), tic disorders, (15.4%), and specific phobia (15.4%). 11.5% of the cases in the PFBD group were diagnosed with somatic symptom disorder and more than half of the patients (n = 27 (51.9%)) showed clinical characteristics of tic disorders. Conclusion: Psychiatric diagnoses are common in children with PFBD, and teamwork involving child psychiatrists may be essential for the management of children with PFBD.

    Psychometric Properties of the Parent Version of the Revised Child Anxiety and Depression Scale in a Clinical Sample of Turkish Children and Adolescents

    The Revised Child Anxiety and Depression Scale-Parent version (RCADS-P) is a self-report questionnaire that assesses dimensions of DSM-based anxiety and depressive disorders in children and adolescents. The present study examined the psychometric properties of the Turkish version in a clinical sample of 483 children and adolescents. The child and parent versions of the RCADS, parent versions of the Screen for Child Anxiety Related Emotional Disorders, the Strengths and Difficulties Questionnaire and Adolescent Symptom Inventory-Depression Scale were administered. Current psychiatric diagnoses were assessed via the Schedule for Affective Disorders and Schizophrenia for School-Age Children, Present Version. The RCADS-P demonstrated high internal consistency and test-retest reliability, and good convergent, divergent, and discriminant validity. Confirmatory factor analysis supported the DSM-related six-factor structure. With its demonstrated favorable psychometric properties, the Turkish RCADS-P is currently the only validated parent-report instrument that assesses DSM-based anxiety and depressive disorders in children and adolescents in Turkey.

    Anti-N-Methyl-d-Aspartate (Anti-NMDA) Receptor Encephalitis Rapid and Sustained Clinical Improvement With Steroid Therapy Starting in the Late Phase

    Anti-N-methyl-d-aspartate (anti-NMDA) receptor encephalitis is an autoimmune/paraneoplastic encephalitis, with neurologic and psychiatric symptoms. Early and aggressive therapy has been shown to improve prognosis although problems with executive functions and memory have continued for several years. A 15-year-old girl had a history of initial symptoms including behavioral difficulties, poor attention, and frequent seizures progressing to a catatonia-like state, 2.5 months after onset of initial symptoms. Anti-NMDA receptor antibodies were detected in serum and cerebrospinal fluid. Subsequent to treatment with methylprednisolone starting 3 months after onset, motor skills, responsiveness, self-care, and speech improved rapidly. Her neuropsychologica profile assessed after 2 months showed global difficulties predominantly in attention, executive functions, memory, and visual perception, which moderately recovered in the 7th and 24th months, respectively. Contrary to current literature supporting the positive impact of early immunomodulatory therapy, a dramatic resolution of major neurologic and psychiatric symptoms was detected with steroid treatment given in the late phase.

    Prevalence of psychiatric disorders and suicidality among children and adolescents with thalassemia major-A Turkish sample

    The current study examined psychiatric diagnoses and suicidality in children with transfusion-dependent thalassemia major via a semi-structured interview (K-SADS). Although 4 subjects were diagnosed with major depressive disorder, 24 subjects (40.7%) had depressed mood and irritability, whereas anhedonia and boredom were detected in 23 (39.0%) subjects. Suicidal thoughts were expectedly more prevalent (28.8%) than suicidal intention (5.1%) and suicide attempts (1.7%). Children with thalassemia major may be an under-risk population for subclinical forms of depression and suicidality. Offering routine screening for mental health problems can lead to early identification and treatment.

    Peer victimization in children with attention-deficit/hyperactivity disorder

    This study examined the correlation of peer victimization to psychosocial adjustment in a sample of children diagnosed with attention-deficit hyperactivity disorder (ADHD). A total of 303 files of youth who received a psycho-educational assessment were reviewed; of these, 116 had an ADHD diagnosis. The data collected included the Child Behavior Checklist (which includes items assessing peer victimization), Conner's Parent Rating Scale, Revised Children's Manifest Anxiety Scale and Children's Depression Inventory. Peer victimization was positively correlated with parent reports of anxiety, depression, social problems, delinquent behavior and aggressive behavior. Children with a comorbid psychiatric diagnosis with ADHD reported higher rates of peer victimization than those without a comorbid diagnosis. Children diagnosed with ADHD along with a comorbid externalizing psychiatric diagnosis experienced higher rates of peer victimization than those with a comorbid internalizing psychiatric diagnosis. The implications of this study concerning peer victimization and psychosocial adjustment in children with ADHD are discussed.

In this present study, we aimed to examine the psychometric properties of the Turkish version of the RCADS-CV in a clinical sample of children in Turkey.
