Publication Open Access A Radiographic Comparison of the Root Length and Area After Class II Treatment with Two Different Functional Appliances(1.10.2020) Seker, Elif Dilara; Yagci, Ahmet; Kurt Demirsoy, Kevser; Yuzuak, Ela NurObjective: The purpose of this study was to compare the changes in root lengths and root surface areas that occur after treatment with two functional appliances the Twin Block (TWB) and Crown Herbst appliances. Methods: Forty patients (12 boys, 28 girls) were included in this study. Half of them were treated with the stainless steel Crown Herbst appliance (with crowns placed on the first molars and the first and second premolars), and the other half were treated with the TWB appliance. Panoramic and cephalometric films were obtained before treatment (T1) and after the functional treatment (T2). All upper and lower teeth except second and third molars were analysed with the ImageJ software (version 1.37, National Institutes of Health, Bethesda) on panoramic films. Root length and area values were compared using t-tests. Results: The intragroup comparison showed that root length values were significantly decreased in right and left mandibular incisors and canines in the Crown Herbst group (12 years 7 months +/- 9 months). However, significantly increased root length was observed in right and left maxillary second premolars and right maxillary first premolar in the TWB group (11 years 1 month +/- 4 months). The intergroup comparison indicated that root length values were significantly decreased in right maxillary premolars, right and left mandibular laterals and left mandibular canine in the Crown Herbst group when compared with those in the TWB group. Conclusion: It was concluded that the Crown Herbst group showed a greater tendency for decreased root length than the TWB group. In orthopaedic correction of Class II malocclusions, in the absence of any contraindications, TWB appliance may be preferred for the promotion of root development.Publication Open Access A Rare Case: Mucous Membrane Plasmacytosis in the Larynx(2014-07-01) BÜYÜKPINARBAŞILI, NUR; ÇOBAN, GANİME; GÜCİN, ZÜHAL; Meriç, Ayşenur; BÜYÜKPINARBAŞILI, NUR; ÇOBAN, GANİME; GÜCİN, ZÜHALMucous membrane plasmacytosis is a rare reactive process characterized by plasma cell infiltration of mucous membranes for unknown reasons. We wanted to report this case, which we have not seen in the literature before, of a pica patient who consumed raw rice due to iron deficiency anemia. A 26-year-old female patient was brought to the internal medicine polyclinic by her mother with complaints of consuming a bowl of raw rice (200 gr) every evening. The patient was treated with parenteral ferik hydroxide + sucrose complex (2700 mg) for 5 days. After 15 days, when she came to control her will to eat raw rice had been lost completely. As a result, the attention of doctors and family members for pica due to treatable causes, such as anemia, will prevent many probable complications.Publication Open Access An Overwiev of the Neural Crest Cells and Tumor Metastasis(2016-08-01) Bayindir, NİHAN; Esrefoglu, MUKADDES; BAYINDIR, NİHAN; EŞREFOĞLU, MUKADDESNeural crest cells (NCCs) derived from neuroectoderm are multipotential cells. NCCs leave the neuroepithelium and migrate to various tissues by epithelial-mesenchymal transition. In this areas NCCs differentiate to variety of cells including melanocytes, glia cells, chromaffin cells. Cancer is a complex process which involves a dinamic interaction between tumor cells and surrounding micrenvironment. Cancer cells similar to neural crest cells leave their own environments and metastasize into a different tissue. The development of the neural crest and that of cancer progression share paralel morphological and molecular characteristics. Many signalling pathway and transcription factors are mutual for both processes. To investigate neural crest developmental mechanisms will provide a better understanding for cancer development, progression and metastasis.Publication Open Access Assessing and Managing Delirium in Surgical and Internal Medicine: an Overview(2016-12-01) Kirpinar, Ismet; KIRPINAR, İSMETDelirium is most common neuropsychiatric disorder that affects patients in surgical and internal medicine clinics. It is defined as a transient, usually reversible organic mental disorder that has a rapid onset and fluctuating course. Clinically, delirium is an acute or subacute organic mental syndrome that is characterized by a disturbance in attention (reduced ability to direct, focus, sustain, and shift attention), awareness (reduced orientation to the environment), and other cognitive abilities (e.g., memory deficit and disorientation) and is not better explained by preexisting, established, or other evolving neurocognitive disorders. Studies have found that between 10% and 20% of all adult inpatients are diagnosed with delirium and that it occurs in up to 50% of elderly inpatients. Delirium itself is not a disease but rather a clinical syndrome, which results from an underlying disease. There is evidence from the history, physical examination, or laboratory findings that the disturbance is a physiological consequence of an underlying medical condition, substance intoxication or withdrawal, use of a medication, or a toxin exposure, or it is a combination of these factors. Accurate identification of underlying causes and risk factors is essential in early stage diagnosis and treatment. Treatment of delirium requires the treatment of the underlying causes. Psychotropic medications are used to comfort patients or enable better patient management. The most common findings are acute impairment of consciousness, vertical gaze paresis, cognitive disturbances, and abnormal behavioral symptoms. In this paper, we report a case of bilateral thalamic infarction that resulted in abnormal behavior.Publication Open Access Boneless Maxillary Reconstruction(2020-01-01T00:00:00Z) Kelahmetoğlu, Osman; Güneren, Ethem; Kuzu, Ismail Melih; Yıldız, Kemalettin; KELAHMETOĞLU, OSMAN; GÜNEREN, ETHEM; YILDIZ, KEMALETTİNObjective: Although the midface is supported by maxillary bone, reconstruction of maxillary bony defects does not always require osseous reconstruction. A series of maxillary facial defects of moderate to severe sizes (Brown and Shaw 4-6) uniformly reconstructed with myocutaneous free flaps is presented. Methods: Retrospective analysis was done for consecutive 10 cases between 2013-18. Seven of the cases had defects caused by oncological resection and 3 defects caused by trauma. Free vertically oriented rectus abdominis (fVRAM) flap was used to reconstruct these defects. The mean age of patients was 42.9 years (range=20-71 years). All flaps survived and 2 flaps had small skin island problems. Rapid recovery, low morbidity, acceptable oral competency, and satisfactory cosmetic appearance were obtained with the reconstruction using fVRAM flaps. Conclusion: According to our limited experience, acceptable reconstruction of extensive maxillary defects can be done with using only soft tissue flap, which is versatile and safe.Publication Open Access Comparison Between Y- and S-Shaped Incisions for Open Reduction and Hook-Plate Fixation of the Mallet Finger(2017-04-01) Uzer, GÖKÇER; Yildiz, FATİH; Kapicioglu, MEHMET; Elmadag, Mehmet; Kara, DENİZ; Bilsel, Kerem; UZER, GÖKÇER; YILDIZ, FATİH; KAPICIOĞLU, MEHMET; KARA, DENİZ; BİLSEL, İSMAIL KEREMObjective: This study evaluates the results of open reduction and hook-plate fixation in the treatment of osseous mallet finger and compares S-and Y-shaped incisions used for the approach. Methods: Twenty-eight patients treated for osseous mallet finger using open reduction and hook-plate fixation were divided into two groups according to their incisions types (group I: S-shaped, n= 18; and group II: Y-shaped, n= 10). Functional results at the first year of follow-up and complications were evaluated. Results: All fractures healed. In group I, all patients had excellent results; in group II, nine patients had excellent results, and one patient had fair results. The numbers of complications were two in group I and six in group II. Conclusion: Hook-plate fixation is a successful treatment method for osseous mallet finger. The rate of complications can be decreased using an "S-shaped" incision with this technique. However, patients should be informed about possible skin problems and nail deformities.Publication Open Access Comparison of Intravenous, Intra-articular, and Combined Tranexamic Acid Use in Primary Total Knee Arthroplasty without a Tourniquet and a Drain(2020-04-01T00:00:00Z) Binlaksar, Ruwais; Ali, Jotyar; UZER, GÖKÇER; YILDIZ, FATİH; UÇAN, VAHDET; TUNCAY, İBRAHİMObjective: We assessed the effect of tranexamic acid (TXA) route of administration on the estimated blood loss (EBL) in patients undergoing primary total knee arthroplasty (TKA) without tourniquet and drain use. Methods: One hundred fifty three patients who underwent primary TKA with use of TXA, between December 2012 and February 2016 were evaluated retrospectively. The patients were divided into three groups according to the route of TXA use: group I, 2 g of intravenous (IV; n=50); group II, 2 g of intraarticular (IA, n=50); and group III, 1 g of IV and 1 g IA combined use (n=53). We recorded the body mass indexes, the platelet counts, haemoglobin, haematocrit levels, prothrombin time, partial thromboplastin time, and international normalised ratio, preoperatively and at 1st and 7th days, postoperatively. EBL was calculated using Meunier's formula. Results: On the first day, the mean EBLs for groups I to III were 286.3 +/- 128.8 mL, 342.7 +/- 176.0 mL, and 379.7 +/- 228.9 mL, respectively (p=0.029 for group I vs. group III). On 7th day postoperatively, they were calculated as 823 +/- 619.3 mL, 1175.1 +/- 970.5 mL, and 1092.2 +/- 766.7 mL (p=0.073) During the first 90 days postoperatively, we did not see any symptomatic thromboembolic complications, delayed haemorrhage, or surgical site or periprosthetic infections. Delayed or extensive dental treatment may need to be delivered under general anesthesia and the management of such treatments in these children may require advanced medical support including intensive care (IC). Two children with NPD type B who underwent dental treatment with general anesthesia and their post-operative follow-ups requiring in IC are were presented. Patients with NPD may present with fragile blood clots in extracted tooth socket and should be subjected to strict bleeding control standards; especially the ones that may require further respiratory assistance since oral intubation is an invasive application to the oral surgical site.Publication Open Access Development of Small and Large Intestine(2017-01-01) Esrefoglu, MUKADDES; Cetin, Asli; EŞREFOĞLU, MUKADDESEpithelial components of the organs of the digestive system are derived from endoderm whereas connective tissue and muscle components are derived from mesoderm. At the 3-4th of development, as a result of cephalocaudal and lateral foldings of the embryo, a portion of the endoderm-lined yolc sac cavity is incorporated in the embryo to from the primitive gut. Primitive gut is composed of four main regions which are pharyngeal gut, foregut, midgut, and hindgut; respectively. Intestines are derived from foregut, midgut and hindgut. The development of the intestines is characterised by lengthening, widening, thickening, histological and positional changes. In this manuscript we tried to review the morphological and functional development of the small and large intestines with the aid of pictures obtained from various stages of prenatal and postnatal development the intestines of rats. Previous reviews lack information on both histological and functional development of the organs.Publication Open Access Differences in the Prevalence of Giardia intestinalis as a Generalized Pruritus Etiologic Factor Between Geographic Regions in Turkey(2016-12-01) Cengiz, FATMA PELİN; Emiroglu, NAZAN; Su, Ozlem; Bahali, ANIL GÜLSEL; Onsun, NAHİDE; CENGİZ, FATMA PELIN; EMİROĞLU, NAZAN; BAHALI, ANIL GÜLSEL; SU KÜÇÜK, ÖZLEM; ONSUN, NAHIDEObjective: Giardia intestinalis is a protozoan parasite of the gastrointestinal tract that is widely distributed worldwide. Patients with G. intestinalis are usually asymptomatic; however, the presence of the parasite may lead to a variety of clinical manifestations, including skin lesions. Methods: In this study, 100 cases of generalized pruritus who were followed up in the dermatology outpatient clinics of Kars State Hospital and Bezmialem Vakif University between September 2013 and December 2013 were evaluated to retrospectively determine the incidence of G. intestinalis. Results: A total of 100 patients were analyzed in this study, and 38% of the patients in Kars were found to have the parasite. There were no cases of Giardia in Istanbul. There was a statistically significant difference in parasite prevalence between geographic regions (p<0.05). Overall, 37.9% of men and 38.0% of women tested positive for parasites. No significant difference was found in parasite prevalence by sex (p>0.05). The mean duration of generalized pruritus was 6.52±2.13 months. Conclusions: G. intestinalis is more common in the Eastern Anatolian region of Turkey than in other regions. Patients with generalized pruritus should be investigated for Giardia intestinalis infection, especially in this region.Publication Open Access Diffuse Large B-cell Lymphoma Transformed from MALT Lymphoma Presented by Acute Abdomen(2014-08-01) KARATOPRAK, CUMALİ; ÇETİN, GÜVEN; CIKRIKCIOGLU, M; KISKAÇ, MUHARREM; ÇETİN, GÜVEN; KARATOPRAK, CUMALİ; KISKAÇ, MUHARREMNon-Hodgkin lymphomas constitute 1% of gastrointestinal system tumors. Lymphomas can occur from the oral cavity to the rectum. Primary gastrointestinal lymphomas are most frequent in the stomach, followed by the small and large intestines. Here, we report a 41-year-old male patient who had been under chemotherapy following a diagnosis of diffuse large B-cell lymphoma transformed from MALT lymphoma and discuss new treatment options. He presented to us with an acute abdomen. To our knowledge, this was the first case in the literature that presented with acute abdomen and was diagnosed as diffuse large B-cell lymphoma that transformed from MALT lymphoma. In addition, for the treatment, we want to draw attention to the importance of surgery, in addition to chemotherapy.Publication Open Access Effects of Mandatory School-time Practice on the Preparation of Vocational Medical College Students for their Profession(2015-12-01) Basaranoglu, GÖKÇEN; Yilmaz, SİNAN; BAKAN, Mefkur; Basaranoglu, METİN; BAŞARANOĞLU, GÖKÇEN; YILMAZ, SİNAN; BAŞARANOĞLU, METİNObjective: The goal of this study was to evaluate the effects of mandatory school-time practice on the preparation of second-year anesthesia nurse students for their future profession. Methods: A questionnaire was filled out by second-year anesthesia nurse students for situational awareness evaluation at the beginning of the term. The questionnaire form consisted of 19 questions and was prepared according to the basic practice on anesthesia course. Certain practical anesthesia procedures were obliged to make a certain minimum number for anesthesia nurse students. At the end of the year, the same questionnaire form was filled out again. Results: At the beginning of the study period, the ratio of students who did not feel were ready for their future professional life was 60%, whereas this ratio decreased to 20% by the end of the study period. Conclusion: Application of mandatory practical procedures for anesthesia nurse students seem to be very important in preparing them for their future professional life.Publication Open Access Effects of the Valsalva Maneuver on Intraoperative Bleeding During Tonsillectomy(2019-01-01) ÇALIM, ÖMER FARUK; ÇALIM, ÖMER FARUKObjective: We investigated whether the Valsalva maneuver helps detect any further bleeding points following adequate hemostasis in tonsillectomy. Methods: This was a prospective study of consecutive patients who underwent tonsillectomy performed in a tertiary medical center. The data collected included age, gender, method of operation, and hemorrhage (if any) informations. Immediately after completing the surgery, hemostasis was performed by an anesthesiologist using the Valsalva maneuver to identify bleeding points. Intraoperative and postoperative hemorrhage, as well as the treatment applied. were recorded. Results: One hundred twenty patients underwent tonsillectomy (53 males and 67 females). Tonsillectomy was performed using cold dissection in 59 patients (26 males and 33 females) and bipolar diathermy in 61 patients (27 males and 34 females). The differences in intraoperative hemorrhage following Valsalva were significantly higher in the cold dissection group than in the hot (bipolar) group (p=0.044). Tonsillectomy using bipolar cautery had a statistically significantly higher postoperative hemorrhage rate than that using cold dissection (p<0.05). Conclusion: The Valsalva maneuver is useful for identifying subtle bleeding vessels in tonsillectomy wound bed. We suggest applying the procedure for final verification of hemostasis before ending the surgery.Publication Open Access Effects of Weight Loss with Bariatric Surgery on Platelet Count and Volume(2018-07-01) Ilhan, Muzaffer; KARAMAN, ÖZCAN; Yasin, Ayse Irem; TURGUT, SEDA; TAŞAN, ERTUĞRUL; KARAMAN, ÖZCAN; YASİN, AYŞE İREM; TURGUT, SEDA; TAŞAN, ERTUĞRULObjective: Obesity is a chronic metabolic disorder that leads to the increased risk of cardiovascular diseases. This study aims to investigate the effect of weight loss on the platelet count and volume, which is associated with cardiovascular diseases. Methods: In total, 56 obese patients were recruited for the study. The parameters were retrospectively evaluated before and after 6 months of surgery. Results: The mean weight of the patients was 126.2±23.1 kg before surgery and 91.8±20.5 kg after surgery (p<0.001). The mean platelet counts were 292.5±58.6×10³/µL before surgery and 246.8±59.1×10³/µL after surgery (p<0.001). The mean platelet volumes were 10.4±1.0 fL and 11.6±0.9 fL before and after surgery, respectively (p<0.001). The mean platelet counts before surgery were correlated with the mean platelet volume, mean weight, and mean body mass index (p<0.01, r=−0.39, p<0.01, r=0.35, p<0.01, r=0.41, respectively). The mean platelet counts after surgery were correlated with the mean platelet volume (p<0.001, r=−0.68). Conclusion: This study demonstrated decreased platelet counts and increased platelet volume at 6 months after surgery in obese patients. Further long-term and prospective studies are warranted to clarify these results and pathopsychological mechanisms involved.Publication Open Access Esmolol Administration for the Treatment of Refractory Ventricular Fibrillation(2016-12-01) Karaaslan, KAZIM; UMUTOGLU, Tarik; TOPUZ, Ufuk; Ay, YASİN; KARAASLAN, KAZıM; AY, YASİNVentricular fibrillation (VF) after releasing an aortic cross clamp in patients undergoing open heart surgery procedures is not rare. Ischemiareperfusion injury after release of the aortic clamp, increased adrenergic tone, and insufficient protection of the myocardium are the possible causes. Amiodarone, lidocaine, and beta blockers have been added to the cardioplegia solutions as a preventive measure for reperfusion VF. We report a case of life-threatening, shock-resistant VF during the weaning period of a cardiopulmonary bypass (CPB) in a 61-yearold male who underwent a mitral valve repair surgery for mitral valve regurgitation. After several defibrillation attempts, refractory VF was turned to normal sinus rhythm shortly after ultra-short acting, beta blocking agent esmolol administration. CPB was terminated successfully following this. In conclusion, VF is still a major problem for clinicians and the treatment of refractory VF is not well defined. In contrast with the absence of the sufficient randomized controlled human studies, theoretically beta blockers could be a choice alternative for shock refractory VF.Publication Open Access Estrogen Receptor Positive/Progesterone Receptor Negative Breast Carcinomas: A Subgroup Deserves Particular Interest(2015-08-01) Gucin, ZÜHAL; GECER, Melin Ozgun; Buyukpinarbasili, NUR; SONMEZ, Cavide; Ersoy, YELİZ EMİNE; MUSLUMANOGLU, Mahmut; GÜCİN, ZÜHAL; BÜYÜKPINARBAŞILI, NUR; ERSOY, YELIZ EMINEObjective: Breast carcinomas positive for the estrogen receptor (ER+) but negative for the progesterone receptor (PR−) have unfavorable prognostic features and are resistant to tamoxifen therapy. The goal of this study was to highlight the significance of PR− breast carcinomas. Methods: Therefore, 146 breast carcinomas comprising 87 ER+/PR+ and 59 ER+/PR− carcinomas were examined. These two groups were compared in terms of age; tumor type; tumor size; histologic grade; presence of an in situ component; lymphovascular and perineural invasion; and ER, PR, c-Erb B2, Ki-67, and epidermal growth factor receptor (EGFR) status. Results: While the number of metastatic lymph node and related pN2+pN3 tumors were found to be significantly higher in the ER+/ PR− group, the differences with respect to the tumor size, metastatic lymph node size, and frequency of lymphovascular invasion were nearly significant. Conclusion: ER+/PR− tumors have an unfavorable prognosis and show a clinical behavior closer to triple negative ones, although classified as luminal tumors. Revealing the mechanisms causing these differences will enhance the success of breast cancer therapy.
