The differentiation of the etiology is of utmost importance as the pathophysiological basis will dictate the choice of appropriate treatment. We retrospectively evaluated 154 (99 females and 55 males) patients who received therapeutic plasma exchange (TPE) due to a presumptive diagnosis of TMA, who had serum ADAMTSI3 activity/antiADAMTS13 antibody analysis at the time of hospital admission. The median age of the study cohort was 36 (14-84). 67 (43.5%), 32 (20.8%), 27 (17.5%) and 28 (18.2%) patients were diagnosed as thrombotic thrombocytopenic purpura (TTP), infection/complement-associated hemolytic uremic syndrome (IA/CAHUS), secondary TMA and TMA-not otherwise specified (TMA-NOS), respectively. Patients received a median of 18 (1-75) plasma volume exchanges for 14 (153) days. 81 (52.6%) patients received concomitant steroid therapy with TPE. Treatment responses could be evaluated in 137 patients. 90 patients (65.7%) achieved clinical remission following TPE, while 47 (34.3%) patients had non-responsive disease. 25 (18.2%) non-responsive patients died during follow-up. Our study present real-life data on the distribution and follow-up of patients with TMAs who were referred to therapeutic apheresis centers for the application of TPE. (C) 2018 Elsevier Ltd. Severe bleeding can be seen after liver injury. Various methods have been described in the literature for stopping liver bleeding such as compression, Pringle maneuver, and other hemostatic agents have been used. In this study, the use of anti-inflammatory, antioxidant, antimicrobial, analgesic, and anticoagulant agents described in the literature were evaluated for hemostatic efficacy following liver injuries. Materials and Methods: In this study 28 Wistar albino female / male rats from out-bred production were used. Animals were divided in 4 groups. After liver damage model creation, various herbal substances were applied to the liver. Hematocrit and platelet counts were measured. In addition, for histopathologic changes in the liver parenchyma, the rats were sacrificed, and the liver was resected. Results: The amount of bleeding (p: 0.001) and the preoperative and postoperative hematocrit changes (p: 0.009) were statistically significant between the groups. There was no significant difference in connective tissue growth (p: 0.065) and necrosis (p: 0.062). There were significant differences in inflammation and without karyorrhexis (p: 0.003). Discussion: Histopathological examinations showed that the groups using active agents had more connective tissue and karyorrhexis. Therefore these tissues could help to stop bleeding.
- PublicationOpen AccessA Case Report of Tongue Edema due to Laryngeal Mask with Introducer(1.10.2018) Daskaya, Hayrettin; Ciftci, Taner; Idin, Kadir; Gul, Kursat; Topuz, Ufuk; Olculu, HakkiAirway management using a laryngeal mask is an especially preferred noninvasive technique because of its achievement of hemodynamic stability and ease of application in surgeries that have short surgery time and do not require specific positions such as the prone position. Although it is easily performed, serious complications may manifest rarely because of lack of experience and inappropriate choice of instrumentation. In this case report, clinical management and treatment options of tongue edema that developed because of the forgotten introducer in laryngeal mask application are presented.
- PublicationOpen AccessA Radiographic Comparison of the Root Length and Area After Class II Treatment with Two Different Functional Appliances(1.10.2020) Seker, Elif Dilara; Yagci, Ahmet; Kurt Demirsoy, Kevser; Yuzuak, Ela NurObjective: The purpose of this study was to compare the changes in root lengths and root surface areas that occur after treatment with two functional appliances the Twin Block (TWB) and Crown Herbst appliances. Methods: Forty patients (12 boys, 28 girls) were included in this study. Half of them were treated with the stainless steel Crown Herbst appliance (with crowns placed on the first molars and the first and second premolars), and the other half were treated with the TWB appliance. Panoramic and cephalometric films were obtained before treatment (T1) and after the functional treatment (T2). All upper and lower teeth except second and third molars were analysed with the ImageJ software (version 1.37, National Institutes of Health, Bethesda) on panoramic films. Root length and area values were compared using t-tests. Results: The intragroup comparison showed that root length values were significantly decreased in right and left mandibular incisors and canines in the Crown Herbst group (12 years 7 months +/- 9 months). However, significantly increased root length was observed in right and left maxillary second premolars and right maxillary first premolar in the TWB group (11 years 1 month +/- 4 months). The intergroup comparison indicated that root length values were significantly decreased in right maxillary premolars, right and left mandibular laterals and left mandibular canine in the Crown Herbst group when compared with those in the TWB group. Conclusion: It was concluded that the Crown Herbst group showed a greater tendency for decreased root length than the TWB group. In orthopaedic correction of Class II malocclusions, in the absence of any contraindications, TWB appliance may be preferred for the promotion of root development.

It is also well known that congenital hypothyroidism may be associated with other congenital malformations, especially with cardiac heart diseases, skeletal abnormalities, Central nervous system and eye malformations. In this article, 11-years-old-girl with fibula agenesis associated with congenital hypothyroidism because of thyroid gland hypoplasia is presented. In our knowledge, both congenital hypothyroidism and isolated fibula agenesis have not been published before in the literature.

DOÇ. MEHMET ALİ ÇIKRIKÇIOĞLUAim: The presence of many blood count and biochemical parameters are known to show mortality in acute decompensated heart failure. In our study, we aimed to find the best markers showing mortality.Method: 176 patients hospitalized due to ADHF were enrolled. Treatment modalities and comorbidities influencing leukocyte counts were excluded.Hemogram, ProBNP, D-Dimer, cardiac troponins, biochemistry, sensitive C-reactive protein, urinanalysis and echocardiography were obtained. Cardiovascular deaths at the end of the first year were determined.Results: WBC and absolute neutrophil count were significantly higher and absolute lymphocyte and absolute eosinophil counts were significantly lower in the deceased patients than patients survived. Groups were similar in terms of monocyte counts. BMI, ferritin, uric acid, free T3 (FT3), D-dimer, ProBNP, ejection fraction (EF), albumin, systolic dysfunction, mitral regurgitation, hypotension, hyponatremia, acute renal failure were significantly different among dead and surviving patients. Logistic regression analysis employing these variables showed that low BMI, low albumin, low EF, hyponatremia, low absolute eosinophil count, and low FT3 as a whole were responsible from the 81.8% of cardiovascular deaths. Death rate among patients with an absolute eosinophil count of ?20/mm3 was 4.8 fold higher than the patients with an absolute eosinophil of >20/mm3.Conclusion: In our study, in ADHF, six levels associated with prognosis were worth significantly more than others: the EF Low, low BMI, hypoalbuminemia, free T3, hyponatremia, eosinopeni. Eosinopenin is thought to be due to increased sympatho-adrenarjik activty.
- PublicationOpen AccessOsteoid osteoma of the cuboid bone: a rare cause of foot pain.(2011-01-01) Gürkan, VOLKAN; ORHUN, H; BÜLBÜL, M; KAYAHAN, S; GÜRKAN, VOLKANOsteoid osteoma, a common bone lesion of benign nature, is more rarely seen in feet. It most commonly involves the talus yet rarely the cuboid. The atypical symptoms of foot involvement may delay the diagnosis. Differential diagnosis most commonly includes ankle sprain, monoarticular arthritis, anterior impingement syndrome, tarsal spur, osteomyelitis, stress fracture, eosinophilic granuloma. The delay in diagnosis and treatment of osteoid osteoma in the foot may be a cause of chronic foot pain. In this study, we present a 17-year-old boy with osteoid osteoma in his right cuboid bone. The patient was undiagnosed during the first year of his symptoms. After surgical removal of the tumor, his complaints were resolved. The pathological examination confirmed the diagnosis of osteoid osteoma. Osteoid osteoma is an unusual bone tumor of the foot. It should be included in the differential diagnosis of patients exhibiting foot pain. In speculative cases with no obvious radiographic findings, further imaging studies, such as CT, should be considered.

Lumbar disc herniation surgery is commonly performed today. With radiological and clinical findings; such as low back and lower limb pain which not heal with medical treatment, motor and sensorial deficit and reflex amendments are indications of surgery. In differential diagnosis of lumbar disc herniation, rare pathologies such as Leriche syndrome should be considered. This syndrome is an occlusive thrombotic lesion of proximal part of bifurcation of abdominal aorta. The characteristic symptoms include inability to maintain penile erection, fatigue of both lower limbs, intermittent bilateral claudication with ischemic pain and absent or diminished femoral pulses along with pallor or coldness of both lower extremities. This patient was operated before with diagnose of lumbar disc herniation because of overlapping of symptoms and after 40 days Leriche syndrome was diagnosed. Vascular lesions should be considered in preoperative differential diagnosis in cases with radiologically and clinically diagnosed lumbar disc herniation. Symptoms should be depend on Leriche syndrome, findings of two disease should be together.

Materials and methods: A scientific committee of 25 experts consisting of six rheumatologists and 19 physical medicine and rehabilitation specialists was formed by TLAR. Recommendations were based on the 2006 ASsessment in Ankylosing Spondylitis International Working Group (ASAS)/European League Against Rheumatism (EULAR) recommendations and a systematic review of associated publications between January 2005 and September 2010. A Delphi process was used to develop the recommendations. Twelve major recommendations were constructed for the management of AS. Voting using a numerical rating scale assessed the strength of each recommendation. Results: The 12 recommendations include patient assessment, patient follow-up along with pharmacological and nonpharmacological methods. Some minor additions and changes have been made to the ASAS/EULAR recommendations. All of the recommendations had sufficient strength. Conclusion: National recommendations for the management of AS were developed based on scientific evidence and consensus expert opinion. In a hospital-based case-control study in Istanbul, 209 patients with histologically confirmed cervical cancer were compared with 1050 controls, who were admitted to the different departments of the same hospital. Odds ratios (OR) and 95% confidence intervals (Cl) were obtained from multivariate logistic regression analysis, fitted by the method of maximum likelihood.

Material and Methods: Fifteen healthy neonates as control group and 15 neonates with various congenital malformations were included study. Erythrocyte cataly (CAT) which is an antioxidant enzyme, erythrocyte carbonic anydrase (CA) enzyme activities which play a important role in acid-base balance, serum copper (Cu), lead (Pb), zinc (Zn), cadmium (Cd), iron (Fe), manganese (Mn) and magnesium (Mg) were measured from venous blood samples of each subject. Results: Erythrocyte CAT enzyme activity, serum Zn and Fe levels were statistically significantly decreased (P<0.05) in the study group while serum Cu and Cd levels were statistically significantly high (P<0.05)in the study group when compared to the control group. A negative correlation between erythrocyte CA activity and serum Pb levels was found in the study group. In the control group, negative correlations were found between erythrocyte CAT activity and serum Pb levels; serum Mg and Cu levels; and serum Cd and Mn levels. In addition, in the control group a positive correlation between serum Zn and Cu levels; erythrocyte CA activity and serum Mn levels was found. Conclusion: Our results suggest that erythrocyte CAT enzyme activity and serum Cu, Zn, Fe and Cd levels may play a role in pathogenesis of congenital malformations. The oxidative stress that rise as a result of an imbalance between prooxidant and antioxidant reactions seems to be associated with congenital malformations. Further studies are needed to confirm the association of antioxidant condition and mineral-heavy metal levels in the pathogenesis of congenital malformations.
- PublicationOpen AccessLeiomyoma of the extrapleural chest wall: an atypical location.(2011-04-01T00:00:00Z) Zıyade, Sedat; Ugurlucan, M; Soysal, O; Cemil, Akdemir; ZİYADE, SEDAT; SOYSAL, ÖMERTumours of the thoracic wall constitute a wide histological spectrum. However, the literature includes very few reports of a leiomyoma occurring at the extrapleural chest wall. In this report we present our experience together with a review of the literature. Our patient was a 33-year-old woman who was diagnosed with leiomyoma of the chest wall and treated accordingly and successfully. To the best of our knowledge, this is the seventh report in the literature defining the clinical entity.

The aim of this study was to evaluate differences in the structural parameters of the hip in patients with osteopenia and osteoporosis in the hip region and to assess their relationship with osteoporotic fracture risk, age and gender.

In LNSS, choristomas and colobomas are frequently seen signs, but complex choristomas are rare. We describe a case of a 3-year-old boy with a yellow alopecic plaque on his scalp, face and neck as well as a mass in the episcleral region since birth. He also had epileptic seizures since one year old. Histopathological examination of episcleral specimen revealed choristoma. Magnetic resonance imaging of the brain identified hemimegalencephaly, hemicerebral atrophy, and lateral ventricular enlargement (Turkderm 2011; 45: 155-7)
